Principles of septal correction.

The operation of septoplasty is often given to junior surgeons to perform and dismissed as a simple procedure. This can lead to unsatisfactory results with unnecessary morbidity for the patient and disillusionment for the surgeon. Trainee surgeons feel that the operation of septoplasty is poorly taught. Some common problems encountered during septal surgery are described and a variety of surgical solutions are offered.

The Effect of Intraarticular Insulin on Chondral Defect Repair.

OBJECTIVE: The aim of this study is to evaluate the effects of intraarticular insulin on the treatment of chondral defects. DESIGN: Twenty-four mature New Zealand rabbits were randomly divided into 3 groups as control (Group 1), microfracture (Group 2), and microfracture and insulin (Group 3). Four-millimeter full-thickness cartilage defects were created to the weight-bearing surface on the medial femoral condyles of each rabbit. In the first group, any additional interventions were not performed. Microfracture was performed on defects in groups 2 and 3. Additionally, 10 IU of insulin glargin

[Analysis of the sonographic patterns in Traumatic Patellar Dislocation].

UNLABELLED: The diagnosis of osteochondral fractures and medial patellar retinaculum injuries that originate as a result of Traumatic Patellar Dislocation (TPD) appears to be crucial for establishing the treatment strategy. Those injuries may be efficiently recognized by means of ultrasonography (USG). MATERIAL AND METHODS: The analysis of the USG patterns in 22 knees after TPD in 21 patients treated between 2004 and 2007 was proceeded. In all children knee USG examination followed its clinical examination and X ray imaging. The standard USG knee assessment was supplemented by patellar articu

Triradiate cartilage injury. Report of two cases and review of the literature.

Acetabular fractures in children are not uncommon, but traumatic disruption of the acetabular triradiate cartilage occurs infrequently and may be associated with acetabular dysplasia and subluxation of the hip. The triradiate cartilage is at risk from birth to closure. The earlier the age at the time of injury, the greater the chance of acetabular dysplasia. In two cases of triradiate cartilage injury, one had premature closure of the triradiate cartilage with subsequent acetabular dysplasia requiring acetabular reconstruction. A review of the literature reveals ten other cases of acetabular d

[Etiopathogenesis and biodynamics of closed laryngo-tracheal injuries].

The blunt laryngotracheal trauma, once quite a rare phenomenon, has become quite relevant today, particularly because of the great increase in automotive traffic. It was, therefore, felt knowledge of the etiopathogenesis and biodynamics of this form of trauma is important for proper diagnosis and treatment. After having noted that the literature in this regard is generally incomplete and fragmentary, the authors felt it worth compiling the various etiopathogenesis and biodynamic factors involved; characteristics of blunt laryngotracheal trauma. Three basic elements of this form of trauma were

A meta-analytic review of the frequency and patterning of laryngohyoid and cervical fractures in cases of suicide by hanging.

The frequency and distribution of fractures are commonly utilized to assist in interpreting the manner of death. In cases of alleged suicide by hanging, however, the evidence base for the frequency and patterning of laryngohyoid and cervical vertebrae fractures resulting from such blunt force traumatic events is limited and so fractures cannot be reliably used to assist in interpreting the manner. Using meta-analytic techniques, this study aimed to estimate frequency and distribution of fractures in the context of relevant intrinsic and extrinsic variables. A systematic review of the literatur

Editorial Commentary: Haven't We Seen This Somewhere Before? Laying the Foundation for Cartilage Restoration in Hip Preservation.

Since its inception in the early 1980s, the microfracture procedure has been successfully used throughout the body to treat isolated full-thickness cartilage lesions. Although treatment of such injuries can be challenging, and outcomes variable, microfracture has afforded surgeons the ability to treat cartilage lesions in a single-stage fashion at the time of treatment for concomitant injuries. Whereas most research relating to the use of microfracture has focused on managing lesions in the knee, there continues to be interest in applying the same principles in other regions of the body. Magnetic resonance imaging of the wrist: bone and cartilage injury.

Magnetic resonance imaging (MRI) is particularly useful for imaging the wrist due to its superior soft tissue contrast and ability to detect subtle bone marrow changes and occult fractures. A high field (1.5T or greater) strength, dedicated wrist coil, and high in-plane and through-plane resolution must be utilized to successfully visualize the relatively thin cartilage of the wrist. MRI can be used to detect occult carpal bone fractures, identify complications following scaphoid fractures, and assess for avascular necrosis in the setting in Kienböck's and Preiser's disease. MRI is useful to i

Comparison of post-operative outcomes of graft materials used in reconstruction of blow-out fractures.

BACKGROUND: Trauma to the face caused by assault or impact may cause internal orbital fracture. Increased intraorbital pressure without disruption of soft tissue integrity or causing a fracture line in orbital rims or orbital floor fractures described as 'orbital blow-out fracture'. Such fractures have been categorized as 'pure blow-out fractures' in which only the orbital floor is affected, and 'impure blow-out fractures' in which other maxillofacial bones such as zygoma, maxilla and nasoethmoid are also affected. Physical examination reveals periorbital edema and ecchymosis, subconjunctival

Immunolocalization of IL-17A, IL-17B, and their receptors in chondrocytes during fracture healing.

Fracture healing in long bones is a sequential multistep cascade of hemostasis, transient inflammation, chemotaxis of progenitor cells, mitosis, differentiation of cartilage, and replacement with bone. This multistep cascade is orchestrated by cytokines and morphogens. Members of the interleukin (IL)-17 family, including IL-17B, have been identified in cartilage, but their expression during fracture healing is unknown. In this study, we determined the immunolocalization of cytokines IL-17A and IL-17B, along with the IL-17 receptor (IL-17R) and IL-17 receptor-like protein (IL-17RL), during the

Effects of human amniotic fluid on fracture healing in rat tibia.

BACKGROUND: Human amniotic fluid (HAF), including hyaluronic acid (HA) and several growth factors, has been used experimentally in tendon, nerve, and cartilage regeneration and in bone defects because of its positive stimulating effects on regeneration potential. This study was performed to investigate whether HAF was effective on fracture healing. MATERIALS AND METHODS: We created 36 tibial fractures in 20-week-old Wistar rats that were divided into three groups. In group 1, fracture lines were instilled with HAF collected at 18th week of the gestation and, in group 2, fracture lines were in

Overlay grafting for lateral nasal wall concavities.

One of the most difficult maneuvers in rhinoplasty is achieving a straight dorsum in the patient with a crooked nose. Often this deformity is a result of trauma involving the bony and cartilaginous vaults, resulting in nasal bone fractures and avulsion of the lateral cartilages into the nasal airway. Reduction of nasal bone fractures is fairly straightforward, but the repair of nasal cartilage trauma is more problematic. The cartilage of the nasal vault possesses fibrous attachments, which when disturbed, are difficult to reestablish. Overlay grafts are known to be effective in filling these d

Disease-specific clinical problems associated with the subchondral bone.

The subchondral bone is involved in a variety of diseases affecting both the articular cartilage and bone. Osteochondral defects in distinct locations and of variable sizes are the final results of different etiologies. These include traumatic osteochondral defects, osteochondritis dissecans, osteonecrosis, and osteoarthritis. Traumatic osteochondral defects are caused by osteochondral fractures, separating an osteochondral fragment that includes articular cartilage and both subchondral and trabecular bone from the joint surface. In osteochondritis dissecans, the disease originates in the subc

Long-term results of tibial plafond fractures treated with open reduction and internal fixation.

In this study a series of 41 consecutive cases of operatively treated fractures of the tibial plafond was retrospectively analysed. The fractures were classified into types I (9.75%), II (41.5%) and III (48.75%) as defined by Rüedi. The 10-year average follow-up revealed 66% good and 24% fair objective results overall. The recent literature uniformly reports good results in operatively treated type-I and -II fractures. In this series, these essentially low-velocity injuries resulted in a satisfactory outcome in 86% of cases. Controversy exists in reports of operative treatment of type-III frac

[Surgical treatment of nasal septum fractures in children with acute trauma].

Statistics are provided on the prevalence, age-related aspects and results of conventional treatment of nasal septum fractures (NSF) in 15% of children treated in hospitals. An original approach to surgical treatment of NSF is proposed. It consists in open endonasal reposition of the osteochondrous base of the nasal septum with removal of small fragments, sparing resection of the quadrangular cartilage along the fracture line with perforation of the cartilage for reliable fixation by targeted formation of the scars. Possibility of one-stage correction of NSF and nasal bones fractures, advantag

Computed tomography of the injured larynx.

Computed tomography (CT) permitted a much more detailed appraisal of laryngeal dysfunction in patients with blunt laryngeal trauma (8 cases) and iatrogenic injury caused by radiation therapy (7 cases), surgery (2 cases), or intubation (1 case). In thyroid cartilage fractures, the fragments may be widely displaced. The cricoid ring breaks in two places, frequently involving the signet. CT facilitates diagnosis of the mechanism of injury based on the site and extent of fractures. The patterns of distortion and fibrosis of the laryngeal cartilage following radiation therapy may be manifested as e
