Permanent reversal of essential tremor following a frontal lobe stroke.

Following a right prerolandic stroke, a 76 year old woman with bilateral upper extremity essential tremor (ET) recovered permanently from the latter contralaterally to the affected hemisphere. The ischemic stroke likely interrupted the cortical component of the ET network. Given the magnitude of neuronal loss, post-stroke cortical reorganization was unable to restore this important component of the ET network, accounting for the permanent nature of ET suppression.

Pediatric frontal lobe epilepsy: white matter abnormalities and cognitive impairment.

OBJECTIVES: Cognitive impairment is frequent in children with frontal lobe epilepsy (FLE). Its etiology remains unknown. With diffusion tensor imaging, we have studied cerebral white matter properties and associations with cognitive functioning in children with FLE and healthy controls. METHODS: Thirty children aged 8-13 years with FLE of unknown cause and 39 healthy age-matched controls underwent neuropsychological assessment, structural and diffusion-weighted brain MRI. Patients were grouped as cognitively impaired or unimpaired, and their white matter diffusion properties were compared wit…

New onset left frontal lobe seizure presenting with ictal asystole.

Ictal asystole is a presumably rare but potentially fatal complication of seizures, most often of temporal lobe origin. It is believed that … stole, eliminating the need for permanent pacemaker placement. We describe a case of new onset left frontal lobe epilepsy in a young athletic patient who presented with near-syncopal episodes but whose comprehensive work-up revealed frequent events of ictal bradycardia and asystole. He responded well to monotherapy using oxcarbazepine, avoiding a permanent pac …

Frontal lobe dementia and subcortical vascular dementia: a neuropsychological comparison.

We compared the performance of 40 patients with frontal lobe dementia to that of 40 patients with subcortical vascular dementia (80 patients including, 46 men and 34 women) in a set of tasks assessing attentional, executive, and behavioural tasks. The frontal lobe dementia represents an important cause for degenerative disruption and is increasingly recognised as an important form (up to 25%) of degenerative dementia among individuals of late-middle-age. The main involvement is the frontal-subcortical pathway, which is the final target of impairment even in subcortical vascular dementia. A wid…

Hereditary spastic paraplegia with frontal lobe dysfunction: a clinicopathologic study.

The authors report an unusual family with hereditary spastic paraplegia (HSP) with frontal lobe dysfunction having the onset in the sixth decade. All the patients showed hypoperfusion in the frontal lobes and thalami on SPECT. Neuropathologic findings revealed thin corpus callosum and degeneration in the thalamic dorsomedial nuclei as well as degeneration of the corticospinal tracts. This family was likely affected by a novel form of HSP characterized by frontal lobe dysfunction caused by thalamic degeneration.

Frontal lobe disinhibition, Rett syndrome and attention deficit hyperactivity disorder.

Motor cortex hyperexcitability in Rett Syndrome (RS) has been ascribed to the failure of an inhibiting system arising from the frontal lobes, a system needed in childhood and early adolescence. It is also presumed that, on a virtually dysfunctional (nonstructural) basis, motor cortex hyperexcitability in ADHD is also due to insufficient frontal lobe inhibition ("lazy frontal lobe"). A series of schematic illustrations is added for the sake of easier comprehension of the presented concept.

Frontal lobe degeneration: novel ubiquitin-immunoreactive neurites within frontotemporal cortex.

Ubiquitin-protein conjugates have been identified in filamentous inclusions in various neurodegenerative disorders. In frontal lobe degeneration (FLD) no distinctive histological features have been reported with the exception of some ubiquitin-positive intraneuronal inclusions in cases associated with motor neuron disease. In the present study, we investigated five FLD cases without motor neuron disease using immunohistochemistry. A constant feature in all cases consisted of ubiquitin-positive neurites in layers I-III of the frontotemporal neocortex. These neurites were not argyrophilic, and c…

Autosomal dominant nocturnal frontal lobe epilepsy. A distinctive clinical disorder.

The disorder of autosomal dominant nocturnal frontal lobe epilepsy has recently been identified, and is now delineated in detail. A phenotypically homogeneous group of five families from Australia, Britain and Canada, containing 47 affected individuals, was studied. The largest family contained 25 affected individuals spanning six generations. This disorder is characterized by clusters of brief nocturnal motor seizures, with hyperkinetic or tonic manifestations. Subjects often experienced an aura, and remained aware throughout the attacks. Seizures occurred in clusters (mean eight attacks/nigh…

Clinical picture of frontal lobe degeneration of non-Alzheimer type.

Frontal lobe degeneration of non-Alzheimer type (FLD) is the second most common primary degenerative dementia in southern Sweden. Clinical findings in 30 FLD cases with postmortem-verified diagnoses are described. FLD starts in the presenium with a mean disease duration of 7.5 years (range 3-17 years). Clinical onset is insidious and slow and the early stage is dominated by personality changes with lack of insight and judgement and signs of disinhibition. A typical feature is progressive loss of expressive speech with stereotyped phrases, late mutism and amimia. Restlessness, changes of oral/d…

Deficits in strategy application following frontal lobe damage in man.

A quantitative investigation of the ability to carry out a variety of cognitive tasks was performed in 3 patients who had sustained traumatic injuries which involved prefrontal structures. All 3 had severe difficulties in 2 tests which required them to carry out a number of fairly simple but open-ended tasks over a 15-30 min period. They typically spent too long on individual tasks. All patients scored well on tests of perception, language and intelligence and 2 performed well on a variety of other tests of frontal lobe function. Explanations for their difficulty on the multiple subgoal tasks …

Cerebral neurosyphilis mimicking acute frontal lobe infarction: a case report.

INTRODUCTION: Syphilis, caused by Treponema pallidum, the "great imitator," has re-emerged as a public health concern. It presents with diverse manifestations, ranging from asymptomatic meningitis and psychiatric changes to meningovascular syphilis presenting as ischemic stroke. We aim to report a case of an elderly male with cerebral neurosyphilis mimicking an acute frontal lobe infarction, emphasizing the importance of considering syphilis in patients with atypical risk factors or clinical presentations. CASE PRESENTATION: A 77-year-old male presented with subacute personality changes and e…

When judgment and initiative fail: dilemmas in frontal lobe lesions.

A key part of neurosurgery is decision-making; not only by the neurosurgeons but also by the patients. The patient needs to decide: shall I do the operation? Now or later? With which doctor? At which medical center? In which country? But what if the exact condition that needs an operation causes you problems with making decisions, such as a frontal lobe lesion? Your decision-making capacity may need to go to others [1]. But what if those others may not act in your best interest? What if your decisions carry weight not just for you, but for someone else who depends on you, e.g. as a caregiver, …

Effect of elastic oral appliance chewing on frontal lobe activity.

OBJECTIVES: Chewing increases frontal lobe activity, resulting in improved memory, learning ability, and response reaction time. This study aimed to assess the effects of elastic oral appliance chewing on the activities and functions of the frontal lobe. METHODS: The study participants were 15 healthy men with full dentulous (mean age, 27.4 ± 4.1 years). A prospective crossover design was used to assess frontal lobe activities and functions. Changes in frontal lobe activities were measured with near-infrared spectroscopy (NIRS). At baseline, the participants were assessed in the resting state…

The self and self-knowledge after frontal lobe neurosurgical lesions.

BACKGROUND: Evidence suggests that damage to the frontal lobes can be associated with changes in cognitive and behavioral functioning and reduced awareness that such changes have occurred. In the current study, the Cognitive Awareness Model was used as a theoretical framework to explore knowledge of the self in people with acquired frontal lesions. METHODS: Fifteen individuals with focal frontal lobe lesions (FFL) and their nominated informants were compared with fifteen healthy matched control-informant dyads on questionnaire measures designed to assess awareness of difficulties. Questionnai…

Enhanced mind-matter interactions following rTMS induced frontal lobe inhibition.

A major barrier to acceptance of psi is that effects are small and hard to replicate. To address this issue, we developed a novel neurobiological model to study this controversial phenomenon based upon the concept that the brain may act as a psi-inhibitory filter. Our previous research in individuals with frontal lobe damage suggests that this filter includes the left medial middle frontal region. We report our findings in healthy participants with rTMS induced reversible brain lesions. In support of our a priori hypothesis, we found a significant psi effect following rTMS inhibition of the le…

Decision-making under ambiguity after frontal lobe resection for epilepsy.

PURPOSE: Decision-making is crucial to daily life and can impact our society as well as economic conditions. Although the frontal lobes have been identified as important for decision-making, this capacity has only been studied to a limited extent in frontal lobe epilepsy and not at all after frontal lobe resection (FLR) for epilepsy. This study aimed to explore decision-making under ambiguity after FLR for epilepsy. METHODS: Fourteen patients having undergone FLR for epilepsy completed the Iowa Gambling Task (IGT) which is a widely used tool to measure decision-making under ambiguity. Iowa Ga…
