Gait Analysis in Cervical Spondylotic Myelopathy.

STUDY DESIGN: Gait analysis of patients with cervical spondylotic myelopathy (CSM) by using a sheet-type gait analysis system. PURPOSE: The aim of this study was to compare the gait patterns of patients with CSM, evaluated by the Nurick grades, and to determine the threshold values of gait parameters predicting the occurrence of a fall by using a gait recorder. OVERVIEW OF LITERATURE: Gait disorder due to CSM may progress to severe paraplegia, following even a minor trauma such as a fall. The indications for the surgery of CSM without severe paralysis remain controversial. The quantitative g…

[Gait disturbances and normal pressure hydrocephalus].

Normal pressure hydrocephalus must be considered when gait disturbance, cognitive impairment and sphincter disorders are associated. Symptoms of normal pressure hydrocephalus, especially gait disturbance, are potentially curable by surgery. Our paper provides a summary review of gait disorders associated with normal pressure hydrocephalus detailing their characteristics and the best assessment methods. Although the pathogenic mechanisms underlying normal pressure hydrocephalus remain poorly understood, advances in imaging have enabled considerable progress in our fundamental knowledge of the c…

Gait variability: methods, modeling and meaning.

The study of gait variability, the stride-to-stride fluctuations in walking, offers a complementary way of quantifying locomotion and its changes with aging and disease as well as a means of monitoring the effects of therapeutic interventions and rehabilitation. Previous work has suggested that measures of gait variability may be more closely related to falls, a serious consequence of many gait disorders, than are measures based on the mean values of other walking parameters. The Current JNER series presents nine reports on the results of recent investigations into gait variability. One novel …

[Gait analysis--a new diagnostic tool].

Three-dimensional gait analysis is a systematic measurement, description, and assessment of human gait. Gait analysis is established as a useful diagnostic tool in patients with gait problems, as it is not possible to obtain an adequate and detailed understanding of such a complex mechanism as gait in a conventional clinical examination. The method has provided a better understanding of both normal gait and abnormal gait patterns; it is a suitable instrument for evaluation of treatment results as well as for scientific work. The first gait laboratory for clinical use in Norway was established …

Gait training in hemiparetic stroke patients.

The major goal of neurological rehabilitation includes restoration of mobility. In mobility we include walking, standing up, sitting down, weight shifting from one leg to the other, turning around, initiating and stopping locomotion, as well as climbing stairs. The therapeutic procedures include: different concepts of physiotherapy stressing different features, like force exercise, reduction of spasticity, gait symmetry, utilization of equilibrium reflexes, stepping automation, endurance training, repetition of rhythmic movements etc. The spectrum of available therapies was recently widened by…

Typical features of cerebellar ataxic gait.

BACKGROUND: Although gait disturbance is one of the most pronounced and disabling symptoms in cerebellar disease (CD), quantitative studies on this topic are rare. OBJECTIVES: To characterise the typical clinical features of cerebellar gait and to analyse ataxia quantitatively. METHODS: Twelve patients with various cerebellar disorders were compared with 12 age matched controls. Gait was analysed on a motor driven treadmill using a three dimensional system. A tandem gait paradigm was used to quantify gait ataxia. RESULTS: For normal locomotion, a significantly reduced step frequency with a …

Neuromuscular evaluation using rat gait analysis.

We have developed a rat gait analysis model to evaluate if ankle angle and other associated gait parameters could consistently define normal peroneal nerve and anterior tibialis muscle function. The second part of the study was designed to determine if such a model would be useful to measure recovery of function after a peroneal nerve crush injury (NCI). A clear plexiglas tunnel was designed for high-speed frame videotaping and subsequent computergraphic gait measurement and analysis. Normal gait patterns for ankle angle, back height, step and stride lengths and the stance and swing times were…

Dementia, gait disturbance, incontinence and hydrocephalus.

Normal pressure hydrocephalus is frequently emphasised as a cause of reversible dementia, but is seldom encountered. Over a 2 year period, 5 patients presented with cognitive decline, and gait disturbance, with or without incontinence. CT head examination revealed hydrocephalus. Although none had symptoms of raised intracranial pressure on initial presentation, there were clinical or CT signs of raised intracranial pressure in 4 of the 5. Underlying pathologies were meningeal lymphocytic lymphoma, idiopathic meningeal fibrosis, periaqueductal glioma, basilar aneurysm and basilar invagination. …

Biomechanics of below-knee amputee gait.

Sagittal plane biomechanical and EMG analyses from eight below knee (B/K) amputee trials demonstrate considerably modified motor patterns from the residual muscles at the hip and knee. Five SACH fittings, two Uniaxial and one Gressinger prostheses were analysed. Moments of force and mechanical power were analysed on all eight trials and EMG profiles are reported for three of the amputees fitted with SACH prostheses. The findings can be summarized as follows: 1. All eight trials had similar internal moment of force patterns at the ankle. A dorsiflexor moment commenced at heel contact and contin

Gait changes in adult onset hemiplegia.

Multiple parameters of gait were evaluated in 50 adult acquired hemiplegic patients and 30 control patients with no history of gait abnormality and no deviation from normal gait by observational analysis. Findings in the hemiplegic group include: 1) increase in the proportion of the gait cycle spent in stance and double-limb support phases in both the normal and affected limb; 2) consistent deviation from normal gait pattern by observation and objective pattern analysis; 3) abnormal phasic activity of specific muscles and muscle groups in the affected limb; and 4) a consistent electrogoniometr…

Biped gait stabilization via foot placement.

It is shown that stable biped gaits can be achieved by discrete foot placement based on feedback of information available at the time of foot placement. The model, developed by Townsend (1981, J. Biomechanics 14, p. 727) to evaluate the coordinations of torso motions, subsumes most of the salient body members and motions. The modeling yielded a generalized inverted pendulum with a movable support point which physically defines lateral foot placement. The principal result is that stable gaits can be defined by foot placements which are a linear function of the system center of mass position and…

Antisynthetase syndrome causing progressive gait disturbance.

Antisynthetase syndrome accounts for one-third of cases of idiopathic inflammatory myopathy. It is characterised by the presence of serum antibodies against aminoacyl-tRNA synthetase and most often manifests as interstitial lung disease, myositis and/or arthritis. We report a 67-year-old woman with gait disturbance and muscle weakness, which worsened over 7 years, especially during intermittent illnesses with fever. Extensive neurophysiological and immunological examination led to a diagnosis of antisynthetase syndrome. She improved with corticosteroids and methotrexate.

Gait Analysis Characteristics in Relapsed Clubfoot.

BACKGROUND: Relapse rates of clubfoot deformity after initial correction range between 19% and 68% regardless of treatment approach. Most studies focus on relapse before age 4. Little research has focused on late clubfoot relapse. The purpose of this study was to compare the gait characteristics of children with late clubfoot relapse (age ≥5 y) following treatment with the Ponseti method only compared with intra-articular and extra-articular surgeries. METHODS: A retrospective review was conducted of all patients with idiopathic clubfoot ≥5 years old who underwent computerized gait analysis f…

How groove in music affects gait.

Rhythmic auditory stimulation (RAS) is a gait intervention in which gait-disordered patients synchronise footsteps to music or metronome cues. Musical 'groove', the tendency of music to induce movement, has previously been shown to be associated with faster gait, however, why groove affects gait remains unclear. One mechanism by which groove may affect gait is that of beat salience: music that is higher in groove has more salient musical beats, and higher beat salience might reduce the cognitive demands of perceiving the beat and synchronizing footsteps to it. If groove's effects on gait are d…

Dissecting parkinsonism: cognitive and gait disturbances.

Parkinsonism is usually designated a movement disorder. However, cognitive impairment comprises a major part of many parkinsonian syndromes, … ces and associated clinical features often help differentiate between parkinsonian syndromes. Human gait requires complex synchronisation at every level of the nervous system, yet occurs with minimal conscious effort on behalf of the walker. Deviations from the normal gait pattern can result from otherwise unnoticeable insults to the body, both intrins …

Flexible Piezoelectric Sensor-Based Gait Recognition.

Most motion recognition research has required tight-fitting suits for precise sensing. However, tight-suit systems have difficulty adapting to real applications, because people normally wear loose clothes. In this paper, we propose a gait recognition system with flexible piezoelectric sensors in loose clothing. The gait recognition system does not directly sense lower-body angles. It does, however, detect the transition between standing and walking. Specifically, we use the signals from the flexible sensors attached to the knee and hip parts on loose pants. We detect the periodic motion compon…
