Case studies on second-generation anticoagulant rodenticide toxicities in nontarget species.

Specimens from 10 cases of second-generation anticoagulant rodenticide poisoning in dogs and cats were submitted to the Texas Veterinary Medical Diagnostic Laboratory during 1986 and 1987. The clinical signs most frequently observed were lethargy, dyspnea, and ventral hematomas; common necropsy findings included hemoperitoneum, hemothorax, and pulmonary hemorrhage. In the instances when histopathological examination of the tissue was done, it supported a diagnosis of coagulopathy. The presence of anticoagulants in serum or liver was confirmed by high pressure liquid chromatography, gas chromat

Methemoglobinemia Secondary to Inhalation of Automobile Emissions with Suicide Motivations.

BACKGROUND: Methemoglobinemia (MetHb) is a rare and potentially severe dyshemoglobinemia that can be induced by exposure to oxidizing agents, decreasing the functional capacity of the hemoglobin molecule to transport and release oxygen into the tissues. MetHb can originate from gases with oxidizing capacity generated by internal combustion engines, although since the universalization of catalyst converters in automobiles, a tiny proportion of MetHb poisoning is due to exposure to engine gases and fumes. Within this group, only two cases due to suicidal motivations have been reported in the las

Unique regulatory mechanism of sporulation and enterotoxin production in Clostridium perfringens.

Clostridium perfringens causes gas gangrene and gastrointestinal (GI) diseases in humans. The most common cause of C. perfringens-associated food poisoning is the consumption of C. perfringens vegetative cells followed by sporulation and production of enterotoxin in the gut. Despite the importance of spore formation in C. perfringens pathogenesis, the details of the regulation of sporulation have not yet been defined fully. In this study, microarray and bioinformatic analyses identified a candidate gene (the RNA regulator virX) for the repression of genes encoding positive regulators (Spo0A an

Recording acute poisoning deaths.

Recording deaths from acute poisoning/substance abuse is not straightforward. The International Classification of Diseases (ICD), used to code mortality statistics, is aimed towards recording the underlying cause of death such as suicide or drug dependence rather than gathering data on poisoning per se. Despite the inherent difficulties clear trends can be observed from the data available for England and Wales. There have been marked changes in the compounds featuring in suicidal poisoning in the last 35 years reflecting changes in the availability of poisons, notably carbon monoxide and presc

Too much of a good thing: Bicarbonate toxicity following treatment of sodium channel blocker overdose.

Although sodium bicarbonate can be a life-saving antidote for patients with overdoses resulting in sodium channel blockade, there has been a concerning rise in cases referred to the Poisons Information Centre where inappropriately large doses of bicarbonate have been used resulting in iatrogenic harm. We present a series of three clinical cases where excessive bicarbonate was used to treat poisonings and discuss our approach to managing cardiotoxicity secondary to sodium channel blockade. Serial blood gas analysis should be performed when using bicarbonate to ensure pH targets are met and seve

[Treatment strategies for acute smoke inhalation injury].

Most fatalities from fires are not due to burns, but are a result of inhalation of toxic gases produced during combustion. Fire produces a complex toxic environment, involving flame, heat, oxygen depletion, smoke and toxic gases such as carbon monoxide and cyanide. As a wide variety of synthetic materials is used in buildings, such as insulation, furniture, carpeting, electric wiring covering as well as decorative items, the potential for poisoning from inhalation of products of combustion is continuously increasing. The present review describes the pathophysiologic effects from smoke inhalati

[Acute poisoning by inhalation of metallic vapors].

Seven cases of acute intoxications by inhalation of metallic vapors are reported. The metals involved were zinc in three cases, tin in one case and mercury in three cases. The severity of the respiratory symptoms was dependent from the inhalated metal. The inhalation of zinc or tin vapors produced an acute limited pneumonitis. A lesional pulmonary oedema and acute interstitial fibrosis with lethal outcome was observed in two cases of intoxication by mercury vapors. The treatment by dimercaptopropanol used in mercury intoxications, increased urinary mercury excretion, but did not improve the pu

Oral Ethanol for Methanol Poisoning: A Case Report.

Methanol poisoning usually occurs due to accidental ingestion of distilling and fermenting errors or beverage contamination. The typical presentation includes visual disturbances with severe metabolic acidosis and neurological damage. A 32-year-old male presented with blurring of vision for 2 days, with a history of local alcohol consumption 2 days prior, with severe metabolic acidosis on arterial blood gas analysis, and was clinically diagnosed and treated as methanol poisoning in the absence of a serum methanol assay. He responded favorably to the treatment with timely oral ethanol via Nasog

Acute Neurological Deficit With New-Onset Atrial Fibrillation in a Patient With Carbon Monoxide Poisoning: A Stroke Mimic.

We present the case of a middle-aged male from the Middle East with no prior history of cardiac disease who presented with syncope, nausea, and dizziness following smoking and barbeque in the desert outdoors. On arrival, he was found to have atrial fibrillation (AF), with a heart rate of 150-170 bpm. A diagnosis of carbon monoxide (CO) poisoning was made based on venous blood gas (VBG) analysis, showing a carboxyhemoglobin level of 33.9%. Treatment was initiated with a non-rebreather mask, followed by high-flow nasal cannula therapy, which resolved the symptoms and arrhythmia within a day. Car

'Hidden' suicides amongst deaths certified as undetermined intent, accident by pesticide poisoning and accident by suffocation in Taiwan.

OBJECTIVE: To identify cause-of-death categories in which suicides might be misclassified in Taiwan. METHODS: We plotted secular trends (1971-2007) in sex- and method-specific rates of deaths classified as suicide, undetermined intent and accident for the Taiwanese population aged 15+ and compared the sex, age and marital status profiles of deaths in these three categories by method of death. RESULTS: The demographic profiles of registered suicides generally resembled those for deaths of undetermined intent and accidents by pesticide poisoning/suffocation but differed from those for accident

Effect of repeated administration of 4-methylpyrazole on renal function and lipid peroxidation products in rat kidney after ethylene glycol poisoning.

Toxic effects of ethylene glycol (EG) and its metabolites are mainly related to metabolic acidosis and kidney damage. EG biotransformation involving CYP2E1 affects the oxidant-antioxidant balance. The study assessed the effect of repeated administration of 4-methylpyrazole (4MP, 15mg/kg b.w. after 2h, followed by 10mg/kg b.w. every 12h) on renal function (creatinine, urea and urinary protein levels) as well as products of kidney's lipid peroxidation (MDA and TBARS levels) in rats poisoned with EG (5745mg/kg b.w.). Serum EG and glycolic acid (GA) concentrations were measured throughout the expe

[Pulmonary edema of toxic origin. Hemodynamic data].

The inhalation of toxic gases or vapours is capable of resulting in pulmonary oedema (P.O.), the mechanism of which corresponds, on the basis of a number of hemodynamic studies carried out, to that which characterises the so-called "lesional" pulmonary oedema, which is different from so-called "hemodynamic" oedema. Classically PAP, PCP and P wedge pressure have virtually normal values (normalisation of pulmonary arterial hypertension by correction of hypoxemia). CI and SWILV are normal or increased and pulmonary resistances are virtually normal. The origin of the oedema is thus related to an i

Fatal phosphine poisoning after accidental aluminium phosphide exposure: a forensic case report.

Aluminium phosphide is a widely used fumigant pesticide that releases phosphine (PH₃), a highly toxic gas responsible for severe and often fatal poisoning. Phosphine intoxication is characterized by rapid clinical deterioration, severe metabolic acidosis, cardiovascular collapse, and multiorgan failure. From a forensic perspective, the diagnosis may be challenging because autopsy findings are frequently nonspecific. We report a fatal case of phosphine poisoning following accidental exposure to aluminium phosphide (Phostoxin) in a 41-year-old electrician. Clinical, biochemical, autopsy, and tox

Transformation of Arsenic from Poison into Active Site by Construction of Unique AsOx/CeO2 Interface for Stable NOx Removal.

Arsenic in the flue gas has been widely reported as a common poison for SCR catalysts; however, an appropriate coping strategy is still lacking to improve the arsenic resistance performance. Herein, a unique AsOx/CeO2 interface is constructed to transform arsenic from poison into active site with balanced acid-redox property, successfully achieving efficient NOx removal. The optimized AsOx/CeO2 exhibits high NOx removal efficiency, four times that of the As-poisoned V2O5/TiO2 catalyst, and even comparable to the state-of-the-art SCR catalysts. It was found that the As-O-Ce interfacial sites in

Brain tissue levels in a fatal case of neonatal mepivacaine (Carbocaine) poisoning.

A fatal case of mepivacaine poisoning in a newborn infant is reported. Regional brain tissue concentrations of mepivacaine were determined by a sensitive and specific gas chromatographic-mass spectrometric method. The brain tissue levels in this patient were higher than those previously reported, possibly due to alkalosis occurring several hours prior to the infant's death.

A general strategy for design SCR catalysts with enhanced resistance to multiple poisons through constructing highly dispersed paired acid-base sites.

The complex composition of solid waste in co-processing within industrial kilns leads to the coexistence of multiple poisons in flue gas, which accelerates the deactivation of SCR catalysts. In this work, solid superacid modified CeO2 with highly dispersed acid-base sites was constructed, achieving the capture of acidic/basic poisons. Moreover, the designed catalyst retained active sites for the adsorption and activation of NH3 after poisoning. The sulfated ZrO2/CeO2 exhibited 2.0, 1.1, 2.3, 1.2, and 2.6-fold increase of NOx conversion than commercial V2O5-WO3/TiO2 when Pb, As, Zn, K, and Cd l
