Male-to-Female (MtoF) gender affirming surgery: Modified surgical approach for the glans reconfiguration in the neoclitoris (M-shape neoclitorolabioplasty).

PURPOSE: The aim of this article is to describe our modified surgical technique for the reconfiguration of the glans in the clitoris and the labia minora, known as the "M-shape neoclitorolabioplasty". METHODS: The glans with all its neurovascular bundle is isolated from the corpora cavernosa, incised in Y-shape mode and spread in order to obtain an M-shape glandular flap. The "belly" of the M-shape glans will constitute the triangular neoclitoris meanwhile the lateral flaps will constitute the labia minora. The inferior apex of the neoclitoris is fixed to the superior apex of the previously s

Surgical and Patient-Reported Outcomes of Gender-Affirming Surgery in Lower- and Middle-Income Countries: A Systematic Review and Meta-Analysis.

BACKGROUND: Access to gender-affirming surgery (GAS) is contingent on availability of procedures, affordability, legality, and social norms. Regions with higher accessibility to GAS, typically high-income countries, may report more surgical and patient-reported outcomes (PROs) data related to GAS, while limited research has characterized outcomes in low- or middle-income countries (LMICs). This meta-analysis characterizes and synthesizes the peer-reviewed literature on GAS-related outcomes in LMICs. METHODS: Electronic databases (MEDLINE, Embase, Web of Science, CINAHL, and PsycINFO) were sea

The Evolution of Phalloplasty for Masculinizing Gender Affirming Surgery: A Systematic Review of Surgical Techniques and Complications Over the Past 25 Years.

BACKGROUND AND OBJECTIVES: Gender affirming phalloplasty is aimed at reducing the deleterious effects of gender dysphoria in the trans male population. Despite being first described in 1945, there is no gold standard for gender affirming phalloplasty. Our objective is to systematically review the phalloplasty literature regarding surgical technique, complication profiles, and temporal trends to understand the evolution of phalloplasty. MATERIALS AND METHODS: A literature review was conducted using the electronic databases PubMed and Medline separately, Scopus, and Embase. Studies were include

Gender-affirming facial surgery in racially, ethnically, and socioeconomically diverse individuals: Variations and consistencies in procedures and outcomes in a large cohort.

BACKGROUND: Gender-affirming facial surgery (GAFS) is an integral part of the gender-affirming care continuum, but access to surgery is limited, and only 7% of transgender women have undergone GAFS. The intersection of race/ethnicity and insurance coverage significantly impacts access to GAFS. Many insurance plans exclude GAFS, and a vast majority of GAFS operations in the United States have been performed on White patients. Accordingly, there are limited data on GAFS in diverse individuals. METHODS: A retrospective chart review identified transgender women who underwent GAFS at the Universit

A Comparative Analysis of Full and Partial Gender-Affirming Facial Surgeries Surgical Complications and Trends: Insights from a NSQIP Study.

BACKGROUND: Gender-affirming facial surgery (GAFS) can be performed as a full, single-staged procedure (F-GAFS) or in multiple, partial stages (P-GAFS). P-GAFS addresses either the upper, middle, or lower facial thirds, while F-GAFS includes procedures performed on multiple facial regions during a single anesthetic event. This study assesses the safety profile of GAFS, comparing surgical complications between F-GAFS and P-GAFS. METHODS: Data from the NSQIP databases spanning 2012-2022 were analyzed. Surgical complications included return to the operating room, hospital readmission, urinary tr

Power, policy, and transgender identities: A case study of gatekeeping by mental health professionals in accessing gender affirming surgeries in India.

BACKGROUND: Transgender individuals seeking gender-affirming surgeries (GAS) are often denied or delayed by mental health professionals (MHPs). Studies on the gatekeeping of GAS have been mainly conducted in the Global North and primarily focus on the perspectives of health professionals. This case study from India incorporates health professional, community, advocate, and activist perspectives to contribute new evidence about MHP gatekeeping in GAS. The study aims to examine the role of power and gender in MHP gatekeeping of GAS in India. METHODS: A qualitative multi-method case study includ

Reduction in Mental Health Treatment Utilization Among Transgender Individuals After Gender-Affirming Surgeries: A Total Population Study.

OBJECTIVE: Despite professional recommendations to consider gender-affirming hormone and surgical interventions for transgender individuals experiencing gender incongruence, the long-term effect of such interventions on mental health is largely unknown. The aim of this study was to ascertain the prevalence of mood and anxiety disorder health care visits and antidepressant and anxiolytic prescriptions in 2015 as a function of gender incongruence diagnosis and gender-affirming hormone and surgical treatment in the entire Swedish population. METHODS: This study used the Swedish Total Population

The Bilateral Inferior Pedicled Anterior Intercostal Artery Perforator-Sparing Flap for Correcting Funnel Chest Deformity in Gender-affirming Masculinizing Chest Surgery.

Pectus excavatum (PE) is the most common congenital anterior chest wall anomaly, often requiring surgical correction due to both functional and aesthetic concerns. Gender-affirming surgery has seen a significant increase in recent decades, with mastectomy being the primary procedure for transgender men. For transmasculine patients with PE seeking chest masculinization, the possibility of addressing both conditions in a single surgery has not been well explored. We report the first known case of a 33-year-old transgender patient with PE undergoing a combined procedure of masculinization top sur

The Impact of a Breast Cancer Risk Assessment on the Decision for Gender-Affirming Chest Masculinization Surgery in Transgender and Gender-Diverse Individuals: A Pilot Single-Arm Educational Intervention Trial.

BACKGROUND: Persons assigned female or intersex at birth and identify as transgender and/or gender-diverse (TGD) may undergo gender-affirming chest masculinization surgery (GACMS); however, GACMS is not considered equivalent to risk-reducing mastectomies (RRM). This study aimed to estimate the prevalence of elevated breast cancer (BC) risk in TGD persons, compare self-perceived versus calculated risk, and determine how risk impacts the decision for GACMS versus RRM. METHODS: A prospective single-arm pilot educational intervention trial was conducted in individuals assigned female or intersex

Systematic Review and Meta-analysis of Facial Anthropometric Variations Among Cisgender Females of Different Ethnicities: Implications for Feminizing Facial Gender Affirming Surgery.

BACKGROUND: Feminizing Facial Gender-Affirming Surgery (FFGAS) is gaining popularity among the diverse population of patients impacted by gender incongruence. However, most studies examining facial femininity are based on Caucasians. Thus, it is unclear if ethnic differences exist in anthropometric measures relevant to FFGAS procedures. This study aims to analyze ethnic anthropometric variations in the cisgender female face to identify differences that are potentially relevant to FFGAS. METHODS: A systematic review and meta-analysis of the PubMed, EMBASE, and Cochrane databases was performed
