For almost 40 years, diphenylhydantoin has been the preferred drug in the treatment of seizure disorders. Soon after the drug was introduced into clinical practice, gingival enlargement was noticed as a side effect and, despite much research effort, the etiology of this condition remains unknown. More recently, diphenylhydantoin-induced osteomalacia and the teratogenic effects of the drug have been reported. Recent advances in our knowledge of these conditions will be discussed and future directions for research will be outlined.

A review of cases indicated that gingival fibromatosis occurs in a variety of genetic entities. High risk for epilepsy and oligophrenia is associated if hypertrichosis is present. The side effects of numerous drugs may lead to pathological changes or functional disturbances in the ENT region. In order to explain the causes of presenting symptoms, the ENT doctor should always take in consideration the various drugs which the patient is receiving but which have been prescribed by other specialists. This is pertinent particularly in pathological changes of the oral mucosa, impairment of salivary secretion, hearing, smell and taste as well as disturbances of tubal function and voice changes. Nephrotoxicity is a well-known adverse effect of cyclosporine A (CyA) treatment in children with steroid-dependent (SD) and steroid-resistant (SR) nephrotic syndrome (NS). We analyzed nine children (age: 3.3-15.7 years, two girls) with SD or SR NS who experienced a significant decrease in their GFR under CyA treatment as measured by inulin clearance (C(IN)). Mycophenolate mofetil (MMF) was introduced progressively until doses of 1 g/1.73 m(2) twice daily were reached. Clinical observations suggested that the dose requirements for some immunosuppressive drugs might be lower in some Eastern populations. We performed a study of 50 among 162 patients who were converted from a cyclosporine-based immunosuppression regimen to one based on tacrolimus. Hurler syndrome, the most severe form of mucopolysaccharidosis type I (MPS I), is a rare lysosomal storage disease. The overall incidence of MPS I is 0.99-1.99/100,000 live births.
