BACKGROUND: Traumatic brain injury (TBI) often leads to neurobehavioral disorders (NBDs) that hinder functional recovery. Although demographic (gender, age, years of education) and clinical factors (post-traumatic amnesia duration [PTA], Glasgow Coma Scale severity) have been studied as potential predictors of NBDs, the impact of the levels of cognitive functioning (LCF) and traumatic axonal injury (TAI) has received less attention.
AIM: This study investigates the relationship between the variables and the onset of NBDs following TBI. It also examines the correlation between NBDs and patient…

Traumatic brain injury (TBI) is associated with greater 'brain age' that may be caused by atrophy in grey and white matter. Here, we investi … ain age was associated with greater injury severity measured by post traumatic amnesia duration and Glasgow Coma Scale. 'Brain age' was significantly and inversely associated with verbal memory, but unrelated to visual memory/ability and cognitive flexibility and processing speed. A longitudinal study is required to determine whether TBI leads to a 'one-off' change …

AIM: To examine the parameters influencing prognosis, and the effectiveness of surgical techniques in patients aged over 65 years with acute subdural hematoma (ASH).
MATERIAL AND METHODS: From 2017 to 2023, a retrospective evaluation was conducted on 62 individuals admitted to the emergency department, diagnosed with ASH, and who underwent had surgical interventions. Demographic data, Glasgow Coma Scale (GCS) score upon admission, radiological findings including hematoma volume, midline shift, and surgical techniques used (craniotomy, craniectomy), were examined. The impact of each predictive…

Background and Objective: The study's aim is to identify a subgroup of patients who would benefit from primary decompressive craniectomy (pDC) after acute subdural hematoma (aSDH) evacuation. Materials and Methods: A retrospective analysis of 290 patients undergoing aSDH evacuation between 2016 and 2021 was conducted. Osteoplastic craniotomy (OC) was performed in 213 cases (73.4%), whereas 77 individuals underwent pDC. Preoperative characteristics, such as age, initial GCS score, hematoma thickness, midline shift, and cisternal effacement score (CES), were used to predict outcome at discharge

BACKGROUND AND OBJECTIVES: With improving life expectancy, the proportion of geriatric patients is steadily increasing in the population and … with the overall mortality at the end of 1 month being 30.9%. On multivariate regression analysis, Glasgow Coma Scale (GCS), RS, and coagulation abnormality were found to be independent prognostic factors; these three variables were then used to design a nomogram.
CONCLUSION: In the absence of specific geriatric TBI prognostic models, a nomogram using GCS, RS, and …

OBJECT: This report summarizes the treatments and outcomes of a large series of patients with pediatric head injuries (PHIs), who were admitted to a tertiary pediatric trauma center at the University of Mississippi Medical Center from January 1, 2003 through December 31, 2006.
METHODS: Data were retrieved from the Department of Neurosurgery's Brain Trauma Registry (BTR) on patients who are ≤16 years old. Data include Glasgow Coma Scale (GCS) and injury severity scores (ISS) on admission and Glasgow Outcome Scale (GOS) scores at 6 months follow-up.
RESULTS: The BTR registered 554 patients wit…

Our purpose was to improve the technique of measuring optic nerve sheath diameter (ONSD) for intracranial pressure (ICP) monitoring in cases of traumatic head injury with hemorrhage. In a retrospective study, computed tomography (CT) data of 312 adult patients were collected and analyzed. ONSDs were measured at 3 mm and 8-10 mm distance from the globe together with the eyeball transverse diameter (ETD). The ONSD/ETD ratio was calculated. The correlation analysis was performed with gender, age, Glasgow Coma Scale score, and Glasgow Outcome Score. ONSD was enlarged in all cases when CT scans ind…

A 41-year-old man with a previous kidney transplant was referred for arterial hypertension and acute renal failure. Initial neurological examination was normal. Laboratory data showed a high serum cyclosporine A concentration. A few hours later, he developed generalised tonic-clonic seizures. The brain computed tomogram was not remarkable, but Glasgow Coma Scale score remained at 8. Mechanical ventilation was required for rapidly progressive hypoxaemia related to Staphylococcus aureus pneumonia and septicaemia. Noradrenaline infusion was needed for only nine hours, with no major drop in mean a…

A retrospective chart review of 62 children aged 2 to 18.5 years admitted over a 4-year period to a brain injury rehabilitation unit was performed. Glasgow Coma Scales were not always recorded at the referring institutions, but all but three of the children in the series experienced a period of unresponsiveness to the environment for a period of at least one hour. Available Glasgow Coma scores at the time of injury ranged from 3 to 13. Maximum coma duration was 3 months. All children received an audiological evaluation. Sixteen per cent of the children had conductive hearing losses, 13% had se…

PURPOSE: To determine the neurologic outcome of patients with intracranial hypertension treated with barbiturate-induced coma.
MATERIALS AND METHODS: The records of 49 patients who were admitted to a 31-bed medicosurgical intensive care unit over a 5-year period in whom a barbiturate coma was induced to control intracranial hypertension were analyzed retrospectively. Analysis included assessment of the response to barbiturate coma and evaluation of the long-term neurologic outcome according to the Glasgow Outcome Scale (GOS).
RESULTS: Intracranial hypertension was caused by head trauma in 28

Brain-stem auditory evoked potentials (BAEP's) were recorded in 19 patients with spontaneous intracerebral hemorrhage. More than half of the patients were deeply comatose. There was no correlation between BAEP changes and different types of spontaneous intracerebral hemorrhage or between BAEP's and coma grading by the Glasgow Coma Scale. However, BAEP's were a significant prognostic aid in these cases and useful in indicating the level of the brain-stem lesion.

The association between admission coma score and eventual outcome was assessed using a coma scale developed for children with a variety of central nervous system injuries. As opposed to the Glasgow coma scale, this scale does not demand assessment of verbalization, and thus can be applied to the preverbal or previously intubated child. Cortical function is graded from 6 (purposeful, spontaneous movements) to 0 (flaccid), and brainstem function is graded from 3 (intact) to 0 (absent and apneic). Maximum total score is 9. In 91 children treated for intracranial hypertension, the association was …
