A 63-year-old man developed hypotony due to a cyclodialysis cleft after suture fixation of a posterior chamber intraocular lens. Since medical management and argon laser photocoagulation were unsuccessful, a direct cyclopexy was performed. The intraocular pressure returned to normal postoperatively, and gonioscopy confirmed closure of the cleft. To our knowledge, this is the first case report of cyclodialysis complicating ciliary sulcus fixation of a posterior chamber lens.

The tonographic results in the prospective study carried out by the authors showed that there is a two-phase reaction of the aqueous dynamics after cyclocryotherapy. In the first period, 1 to 3 weeks post-operatively, the aqueous flow decreases. Subsequently it recovers and almost reaches preoperative values. In this stage the outflow facility increases remarkably and is thus the main cause of the reduction in intraocular pressure. There is a close correlation between the changes in outflow and the widening of the chamber angle which can be seen by gonioscopy.

A 66-year-old African American man presented with episodic transient visual loss triggered by bending forward. The initial examination did not suggest intraocular pathology and the patient was nearly sent for vascular evaluation given his cardiovascular risk factors. Fundus photographs taken during an episode of visual loss suggested an intraocular process, however. Gonioscopy revealed a microhyphema causing a "snow globe" effect in the anterior chamber, most likely related to recent bleb manipulation in the affected eye.

PURPOSE: To report a case of angle-closure glaucoma in a 10-year-old girl with ocular albinism.
METHODS: Case report. A 10-year-old girl with autosomal recessive ocular albinism was examined and treated for subacute angle-closure glaucoma of the right eye.
RESULT: The patient was successfully treated with Nd:YAG laser iridotomy of the right eye.
CONCLUSIONS: Angle-closure glaucoma may rarely occur in young or adolescent children. Gonioscopy of children with shallow anterior chambers is recommended.

CASE REPORT: A middle-aged adult male was referred to our institution due to unilateral corneal edema for a possible corneal transplant. At first, the patient denied a history of trauma. A small foreign body, which had been overlooked by the primary ophthalmologist, was detected by gonioscopy, embedded in the anterior chamber angle. It was successfully surgically removed and visual results were good.
DISCUSSION: In any patient with unilateral unexplained corneal edema, it is necessary to rule out the presence of a foreign body in the anterior chamber.

The incidence of chronic angle closure glaucoma is considerably greater than the incidence of the acute type. This type of glaucoma may mimic primary open angle glaucoma with visual field deterioration, optic nerve alteration and intraocular pressure elevation with a quiet painless eye. Its diagnosis is based on indentation gonioscopy showing peripheral anterior synechiae. The mechanisms of angle closure are the pupillary block, the plateau iris configuration and the creeping form. The treatment of chronic angle closure glaucoma is based on laser peripheral iridotomy.
