Rapid diagnosis of gram negative pneumonia by assay of endotoxin in bronchoalveolar lavage fluid.

BACKGROUND: Diagnosis of ventilator associated pneumonia can be made by quantitative cultures of bronchoalveolar lavage fluid or of protected specimen brushings, though cultures require 24-48 hours to provide results. In 80% of cases aerobic Gram negative bacteria are the cause. METHODS: A rapid diagnostic method of assessing the endotoxin content of lavage fluid by Limulus assay is described. Forty samples of lavage fluid were obtained from patients with multiple trauma requiring mechanical ventilation for a prolonged period. Pneumonia was diagnosed on the basis of clinical, radiological, an

Meteorological factors and ambient bacterial levels in a subtropical urban environment.

We conducted a study to investigate the characteristics and determinants of ambient bacteria in Taipei, Taiwan from August 2004 to March 2005. We monitored ambient culturable bacteria in Shin-Jhuang City, an urban area in the Taipei metropolitan areas, using duplicate Burkard Portable Air Samplers with R2A agar. The average concentration of total bacteria was 1,986 colony-forming units per cubic meter of air (CFU/m(3)) (median = 780 CFU/m(3)) over the study period, with the highest level in autumn. Most bacterial taxa had similar seasonal variation, with higher concentrations in autumn and win

Community-acquired Pseudomonas aeruginosa sacro-iliitis in a previously healthy patient.

Pyogenic sacro-iliitis is an uncommon osteo-articular infection that occurs usually in immunocompromised patients and is associated with gram-positive cocci. It is very rarely linked with a gram-negative aetiology. The first case of Pseudomonas aeruginosa sacro-iliitis is described, which occurred in a previously healthy young man, without history of prior traumatic events, hospitalisation or chronic underlying disease.

Listeria monocytogenes peritonitis in cirrhotic patients. Value of ascitic fluid gram stain and a review of literature.

Listeria monocytogenes has been increasingly implicated in spontaneous bacterial peritonitis in patients with cirrhosis. This bacterium can be mistaken for diphtheroids and gram-positive cocci if special attention is not paid to the motility pattern and specific biochemical tests. Although the sensitivity of ascitic fluid Gram stain is low, we describe a case in which the Gram stain of the ascites fluid was positive. This issue is now pertinent given recent recommendations of third-generation cephalosporin antibiotics as empiric therapy for spontaneous bacterial peritonitis; Listeria is resist…

Acute fibrinopurulent blepharitis and conjunctivitis associated with Staphylococcus hyicus, Escherichia coli, and Streptococcus sp. in chickens and turkeys.

Multiple outbreaks of acute severe fibrinopurulent lesions of the eyelids occurred in chickens and turkeys. Lesions began as tiny foci of epidermal necrosis and ulceration and spread to involve the entire eyelid. Scabs overlying the epidermis contained large gram-positive cocci; lesser numbers of small cocci and gram-negative bacilli were in more superficial areas. Staphylococcus hyicus was isolated from birds in all stages of the disease. Escherichia coli and Streptococcus sp. were isolated only during severe stages; no anaerobic bacterial pathogens were isolated. Vasculitis and perivascular …

Empirical antimicrobial therapy with Timentin plus amikacin in febrile granulocytopenic cancer patients.

Timentin (ticarcillin plus clavulanic acid) plus amikacin was administered as an empirical regimen to 52 febrile granulocytopenic patients with cancer and appeared as effective as the other commonly recommended combinations of antimicrobial agents. A favourable response was observed in 61% of episodes with bacteraemia and in 83% of the episodes without bacteraemia. However, the efficacy in Gram-positive bacteraemia was suboptimal and the emergence of superinfections caused by Gram-positive cocci may represent a clinical challenge.

Blood culture results during the period 1995-2002 in a Greek tertiary care hospital.

Data on BacT/Alert blood cultures in a Greek hospital from 1995 to 2002 were analysed retrospectively. There was a gradual increase in the number of blood cultures (4981 in 1995 to 9054 in 2002), the true positive rate (14.4% to 16.5%) and the number of bloodstream infections/1000 hospital admissions (22.1 to 30.7). The five most common pathogens were Eschericia coli, Staphylococcus aureus, coagulase-negative staphylococci, enterococci and Klebsiella spp. The relative rates of Gram-negative and Gram-positive isolates inverted during the study period because of an increasing frequency of coagul

A rare case of ecthyma gangrenosum associated with methicillin-resistant Staphylococcus aureus infection.

Ecthyma gangrenosum (EG) is a well-recognized dermatological condition caused by gram-negative bacillary infection, particularly Pseudomonas aeruginosa. Association with gram-positive cocci is very rarely reported in the literature. To the best of our knowledge, we describe the third case of EG caused by methicillin-resistant Staphylococcus aureus in a patient with AIDS who presented with multiple typical necrotic lesions.

Antibiotic-Impregnated Catheters for Ventriculoperitoneal Shunt in Neonates and Infants: A Systematic Review and Meta-Analysis.

BACKGROUND: Ventriculoperitoneal shunt (VPS) is the standard treatment for hydrocephalus in neonates and infants, but is commonly complicated by shunt infections. Antibiotic-impregnated catheters (AICs) have been introduced to mitigate infection risk; however, their effectiveness remains uncertain. METHODS: We conducted a systematic review and meta-analysis comparing outcomes in neonates and infants who underwent VPS with and without AICs. Primary outcomes included shunt infection risk, time to first infection, and infections caused by gram-positive cocci and gram-negative rods. Pooled odds r…

[Clinical evaluation of high dose intravenous injection of fosfomycin on the severe infections associated with the treatment of haematological disorders].

Fosfomycin (FOM) was administered intravenously to 65 cases of severe infections complicated with 62 cases of several haematological disorders. Out of 65 cases, 45 were treated with high doses FOM, i.e., 8 g per day or more. Another 20 cases were treated in usual doses of 4--6 g per day. Causative organisms were isolated from 52 cases of which 32 cases were Gram-negative bacilli and 19 cases were Gram-positive cocci. The effective rate of FOM was 57.8% in the high-dose treatment group (26/45) and 45.0% in the usual-dose treatment group (9/20), but the significant difference was not defined. Am…

Evaluation of a rapid direct assay for identification of bacteria and the mec A and van genes from positive-testing blood cultures.

We performed the first evaluation of a DNA strip assay (GenoType blood culture; Hain Lifescience, Nehren, Germany) for the detection of the most relevant bacterial sepsis pathogens directly from positive BACTEC blood culture bottles (Becton Dickinson, Heidelberg, Germany). The test comprises two panels, one for the direct species identification of important gram-positive cocci and the other for gram-negative rods. Additionally, detection of the mec A and the van genes are implemented. The GenoType assay was validated regarding its analytical sensitivity with blood cultures spiked with referenc…

[Prevention of bacterial infections using ciprofloxacin in granulocytopenic patients with cancer].

The optimal approach to reduce bacterial infections in granulocytopenic patients is still controversial. Recently, fluoroquinolones have been developed and real progress has been achieved in the prevention of Gram negative bacilli septicemia. This study reports our experience with ciprofloxacin and shows the excellent tolerance of ciprofloxacin by our patients as well as promising data for the reduction of Gram negative bacilli infection. However, practical modalities to prevent infection caused by Gram positive cocci remain to be defined.
