A case of septic arthritis caused by Capnocytophaga canimorsus in an HIV patient.

Invasive infections caused by Capnocytophaga canimorsus , a Gram-negative rod found in the oral cavity of healthy dogs and cats, are rare but they are increasing worldwide. We report a case of septic arthritis in a native knee joint due to this micro-organism. A 57-year-old man, with a well-controlled chronic HIV infection, attended the Emergency Department because of left knee pain and shivering without measured fever. A knee arthrocentesis and a computed tomography scan were performed, revealing septic arthritis with collections in the left leg posterior musculature. He was admitted to the I

Septicaemia due to Actinobaculum schalii.

We report a case of septicaemia caused by Actinobaculum schalii, an Actinomyces-like bacterium, a gram-positive, non-sporulating rod. The search for the bacterium is relevant in cases with unexplained pyuri and septicaemia, because it may cause severe infection in predisposed humans. The bacterium is susceptible to a wide range of antibiotics.

Identification of cultivable microorganisms from root canals with apical periodontitis following two-visit endodontic treatment with antibiotics/steroid or calcium hydroxide dressings.

The study was aimed at comparing the efficacy of disinfection of root canals with periapical radiolucencies when treated with either antibiotics/steroid medicaments (Ledermix or Septomixine) or a calcium hydroxide paste (Calasept). Microbiological samples were taken before and after two-visit endodontic treatment from 88 canals with apical periodontitis. All of the canals but one (87 of 88) had cultivable growth before treatment. After dressing with Ledermix, Septomixine, or Calasept, the percentages of canals remained with positive growth were 48% (13 of 27), 31% (8 of 26), and 31% (11 of 35)

Cross-sectional analysis of Toll-like receptor variants and bacterial vaginosis in African-American women with pelvic inflammatory disease.

OBJECTIVE: Bacterial vaginosis (BV) is a common condition associated with serious complications including pelvic inflammatory disease (PID). However, the pathogenesis of BV is poorly understood. Toll-like receptors (TLR) are responsible for microbial recognition and elimination through inflammatory responses. TLR variants have been implicated in infectious and inflammatory diseases and may be involved in BV pathogenesis. We conducted a cross-sectional study to determine if TLR variants are associated with BV. METHODS: Logistic regression was used to test associations between 14 variants assay

Antibody response to Arcanobacterium haemolyticum infection in humans.

Arcanobacterium haemolyticum causes pharyngitis, exanthema, and other infections. The evidence of the pathogenicity of A. haemolyticum depends on clinical descriptions of culture-positive patients and a comparison of carrier rates of patients with pharyngitis and healthy, matched controls. In this investigation, the antibody response of the host was studied for the first time, using SDS-PAGE and Western blot analyses. Paired acute and convalescent sera showed development of antibodies to A. haemolyticum in 7 of 8 patients. The antibodies reacted primarily with four distinct cell wall-associate

A Case of Severe Pneumonia with Extensive Lung Abscesses Due to a Klebsiella Isolate Presumptively Identified as Klebsiella Pneumoniae Subsp. Rhinoscleromatis.

A 67-year-old woman was admitted with the chief complaints of sputum, fever, dyspnea, and malaise. Chest CT revealed a large mass shadow occupying the right middle and lower lobes with multiple nodular shadows in the left lung, and complete atelectasis of the right lung developed the following day. Gram-negative rods in the blood, sputum, and lung tissue were presumptively identified as the rare subspecies Klebsiella pneumoniae subsp. Rhinoscleromatis. The isolate exhibited a hypermucoviscous phenotype based on a positive string test, and PCR assays targeting rmpA and magA were positive. She r

Antibiotic sensitivity of recent clinical isolates from patients with ocular infections.

We examined the status of bacterial infection and drug sensitivity in patients who consulted the Ocular Infection Clinic of Niigata University between 1992 and 1995. Overall, 850 strains were detected. Gram-positive cocci were the most frequent (380 strains, 44.7%). We detected 63 strains of gram-negative bacilli (7.4%), 207 strains of anaerobic bacteria (24.4%) and 18 strains of fungi (2.1%). Among gram-positive cocci, coagulase-negative staphylococci (CNS) were the most prevalent (52.6%). Of gram-negative bacilli, glucose-nonfermenting gram-negative rods (GNF-GNR) were the most prevalent (27

Erythrasma - A Red Herring in the Ongoing Epidemic of Tinea.

We describe the presentation of five adults with erythrasma, misdiagnosed as tinea and prescribed antifungal treatment which resulted in incomplete clearance. The lesions were restricted to axillary and/or inguinal folds. They were dry, brown macules with fine scaling. Except for one with moderate pruritus, they were asymptomatic. No fungal elements could be demonstrated in any of the patients in 10% KOH preparation. Gram stain revealed short gram-positive rods in varying proportions. All the patients showed a pink fluorescence on Wood's lamp examination. After treatment with topical clindamyc…

Clinical evaluation of a direct fluorescent monoclonal antibody test for detection of Pseudomonas aeruginosa in blood cultures.

A direct fluorescent monoclonal antibody test (DFA; Genetic Systems Corp., Seattle, Wash.) was evaluated for the detection of Pseudomonas aeruginosa in 178 blood culture broths obtained from 128 patients. The DFA identified 44 (98%) of 45 blood cultures positive for P. aeruginosa and was negative in 131 (98%) of 133 blood cultures which grew gram-negative rods other than P. aeruginosa. Upon further investigation, saline suspensions of the organism from the false-negative blood culture were strongly (4+) DFA positive. The false-positive reactions were not due to cross-reactivity, as shown by la

Peptide nucleic acid fluorescence in situ hybridization for rapid detection of Klebsiella pneumoniae from positive blood cultures.

This study evaluated a novel peptide nucleic acid (PNA) probe targeting a region of the 23S rRNA gene of Klebsiella pneumoniae by fluorescence in situ hybridization (FISH). Analytical performance was determined using 39 reference strains and other well-characterized strains of Klebsiella spp. and Enterobacter aerogenes. The probe was found to be specific for the K. pneumoniae complex (K. pneumoniae including Klebsiella ozaenae and Klebsiella variicola). The diagnostic accuracy was evaluated with 264 blood cultures containing Gram-negative rods. Using conventional identification as the referenc

Early neonatal bacteraemia. Comparison of group B streptococcal, other Gram-positive and Gram-negative infections.

All cases of neonatal bacteraemia associated with clinical illness occurring at Hammersmith Hospital, over a 9-year period, 1967-1975 inclusive, have been reviewed. The infants studied were those born in the hospital's maternity unit and those admitted from other hospitals from a wide area round London who were ill or of low birthweight. Positive blood cultures occurred in 91 infants, 47 of them in the first 48 hours of life. These 47 infants were analysed separately and divided into three groups, 13 with group B streptococcal infections, 11 with other Gram-positive infections, and 23 with Gra

Outbreak of canine parvovirus 2b and Clostridium difficile infection in Asian small-clawed otters.

A concurrent outbreak of infection by canine parvovirus 2b (CPV-2b) and Clostridium difficile producing A and/or B toxins occurred in Asian small-clawed otters (Amblonyx cinereus). The 5 clinically affected otters were 6- to 24-mo-old intact females that had severe diarrhea, dehydration, were acutely comatose, and died 1-4 d after the onset of clinical signs. Postmortem examination was performed in 3 of 7 otters. Macroscopically, the small intestine was diffusely reddened and contained red-to-brown, malodorous, watery digesta without formed feces (3 of 3). Histologic examination identified los

Actinomycosis of the Forearm: An Unusual Presentation of an Early Event.

Actinomycosis is a rare chronic infectious disease, most commonly affecting the cervicofacial, pulmonary, or genitourinary areas. It is caused by the Actinomyces spp. bacteria, which are facultative anaerobes and gram-positive, nonsporing rods. The disease is characterized by the formation of cold abscesses or fistulas, as well as granulomatous tissue, reactive fibrosis, and necrosis, which can resemble local malignancy. Cutaneous actinomycosis in the trunk or extremities is extremely rare and is usually related to previous surgical procedures or trauma. However, we report one such rare case o
