[Liver transplantation. Current aspects].

The development of liver transplantation has led to technical refinements such as liver graft reduction or splitting for transplantation in two recipients, the use of living related donors, and the temporary orthotopic transplantation of auxiliary liver grafts. Regarding immuno-suppression, new drugs are currently under evaluation, such as tacrolimus (FK 506) which could be effective on some severe rejections. On the other hand, the indications of liver transplantation have been revisited according to clinical results. In France, the increasing development of organ and tissue transplantation h

Indefinite survival of human skin allografts in patients with long-term immunosuppression.

The transplantation of a sufficient quantity of good quality skin is of great clinical importance. Severe burn patients lack adequate skin donor sites to cover their wounds with autografts. Patients with severe injuries and burns of areas such as the entire face are presently reconstructed with multiple full- or split-thickness skin grafts. The final result is a patchwork appearance of skin with inferior qualities to normal full-thickness facial skin. Before structures such as hands should be transplanted, each individual tissue component should be evaluated to determine whether the tissue wil

Clinical evaluation of two bilateral hand allotransplantations at six and three years follow-up.

We report a six and three years follow-up clinical evaluation of two bilateral hand allotransplantations from brain-dead multi-organs donors performed in two young adult traumatic hand amputees. Lifelong immunosuppressive treatment included tacrolimus, mycophenolate mofetil and prednisolone. In both patients, early complications were observed but were successfully treated. At follow-up, clinical results allow useful hand function in both patients, by far superior to what can be provided by current myoelectric prostheses. Although the long-term risks-benefits ratio of bilateral hand transplanta

Assessment of the Quality of Life in Patients After Liver Transplantation as an Important Part of Treatment Results.

INTRODUCTION: Organ transplantation for many patients with end-stage liver disease is a life-saving operation. Progressing at a time and usually irreversible damage to the organ directly leads to many life-threatening complications as well as to progressive deterioration in health and the quality of life (QOL). On the other hand, the assessment of QOL after liver transplantation, apart from the survival analysis, should take into account general health perception, physical activity, a place in the family and in the environment, as well as the patient's social and professional activity. The future of kidney transplantation. The effect of improvements in survival rate on the shortage of donated kidneys.

New drugs and other medical advances in organ transplantation are expected to increase the rate of retention of transplanted kidneys, or grafts, and therefore also raise patient survival rates. In particular, the immunosuppressive drug cyclosporine has been found to reduce the likelihood of rejection of a transplanted kidney by the immune system of the transplant recipient. A critical issue is the effect changes in the graft and patient survival rates will have on the current shortage of donated kidneys. On one hand, higher survival rates may decrease the demand for transplants, as fewer peopl

[Fresh post-traumatic soft tissue defects of the wrist joint and hand and their closure].

As a universal human tool the hand often suffers injuries of its soft tissue. Different localization of defects (palmar and dorsal) and different injuries of functional structures (tendon, nerve, vessel, bone) also need different soft tissue cover and wound closure. The possibilities in plastic surgery of the hand include free transplantation of skin, local and distant flaps, free flaps and neurovascular flaps.

Reconstruction of penetrating injuries of the hand with dorsalis pedis composite free flaps: a series of 23 patients.

BACKGROUND: Reconstruction of severe hand injuries remains a challenge. We herein report the use of dorsalis pedis composite free flaps (DPCFFs) for the reconstruction of penetrating injuries of the hand. METHODS: From July 2001 to February 2009, 23 patients (15 males and 8 females), mean age of 26 years (range, 17-48 years), with penetrating wounds of the hand were treated with DPCFFs. A second toe composite flap was used in cases of bone, joint, tendon and skin defects, and combined flaps were used for patients with only soft-tissue defects. Total joint transplantation using the proximal in

Bilateral Metacarpal Hands: Reconstruction With 6 Toe Transfers.

Background: Bilateral metacarpal hand injuries are extremely rare, but probably represent the most difficult reconstructive challenge in hand surgery. Methods: We discuss the various options for metacarpal hand reconstruction, including the Krukenberg procedure, bionic prostheses, multiple toe-to-hand transfers, and possibly hand transplantation, and present the long-term functional outcomes, gait analysis, and psychological evaluation after a 4-stage reconstruction of bilateral metacarpal hands in a child using 6 toe-to-hand transfers-bilateral great toe transfers to reconstruct both thumbs a

Clinical Applications of Autologous Fat Grafting in Pathological Hand Conditions.

Background: Autologous fat grafting (AFG) is increasingly explored beyond cosmetic applications, with interest in its regenerative potential for hand conditions such as cutaneous systemic sclerosis, thumb base osteoarthritis, Dupuytren contracture, burn scars, trauma and congenital hand deformities. This systematic review evaluates the therapeutic efficacy, techniques and outcomes associated with AFG across pathological hand conditions. Methods: A systematic literature search was conducted in July 2023 using PubMed, Ovid MEDLINE, Embase and Web of Science. Studies were included if they involve

Impact of donor-specific antibodies in reconstructive transplantation.

For many devastating injuries and tissue defects where conventional reconstruction is not possible, reconstructive transplantation such as hand and face transplantation has become a viable alternative. This novel approach allows for improved restoration of appearance, anatomy and function not feasible by other available treatment options. However, clinical management of these injuries prior to transplantation frequently requires multiple blood transfusion or skin grafts resulting in the formation of alloantibodies (anti-HLA IgG Abs) and a high degree of sensitization. The role of donor-specifi

Chimerism studies as an approach for the induction of tolerance to extremity allografts.

Recent advances in the field of transplant immunology and reconstructive surgery have resulted in an increased interest in extremity allograft. Until now, more than 20 hand transplants have been performed in humans. Rejection is well controlled by currently available immunosuppressive drugs. The hand transplant, however, is not a life-supporting organ transplant and these drugs are unlikely to represent the final solution for hand transplantation due to serious adverse effects. The ultimate goal of extremity allograft is the induction of donor-specific immunotolerance. The major strategies for

Establishing an upper extremity transplantation program: lessons learnt over 12 years.

PURPOSE OF REVIEW: As one of the early hand transplant programs in the world, we are often asked to educate other programs on the lessons we have learned and on how to set up a successful hand transplantation program. RECENT FINDINGS: Two decades of global experience with vascularized composite allotransplantation (VCA) of the upper limb have demonstrated improved functional outcomes and quality of life for carefully screened patients. Despite initial reticence about VCA, over 30 patients in the USA have benefited from hand transplantation, with around 80% long-term successes. We have experie

Autogenic heterotopic vascularized proximal interphalangeal joint transplantation in children.

The proximal interphalangeal joint (PIP) joint is the most crucial joint for the functionality of a finger. For a child with complex injury of the hand every effort should be exercised to maximize function restoration. If the PIP joint is irreparably damaged, its reconstruction is indicated. The technique of autogenic heterotopic vascularized toe joint transplantation provides unique advantage of a composite transfer of skin, tendons, bone and joint alone with growth plate and its efficacy has been affirmed in children. It has been suggested that such transfers require intact flexor tendon to

Combined second toe and partial nail transfer from the big toe by means of an exteriorized pedicle.

The hand of a three-year old girl was reconstructed by means of a bilateral second toe transplantation following traumatic amputation of all five digits. In a second operation, aimed at reconstructing the index finger, the nail apparatus of the big toe was transferred to the thumb as an "island flap" based on a pedicle from the second toe. The thumb was subsequently lengthened enabling a three-fingered hand to be obtained with only minimal functional impairment of the feet. The authors suggest an original technique allowing transfer of the second toe and of the big toe nail in only one operati

[Living donor and kidney transplantation].

The medical, immunological and surgical histories of the transplantation of kidneys from a living donor have been developed differently. Living kidney transplantation involves better organ quality and also better kidney function than postmortem kidney transplantation. In Germany, living kidney transplantation is legally based on the transplantation statute of 1997. Traditionally, retroperitoneoscopic open nephrectomy is the gold standard used by most transplantation centers in Germany. The laparoscopic hand-assisted nephrectomy is a very good alternative to other surgical methods, but must be

Tracheal transplantation: lessons learned that may apply to lung transplantation.

PURPOSE OF REVIEW: The purpose of this review is to explore the lessons learned from experimental and human tracheal transplantation to determine if this information may be applied to lung transplantation. RECENT FINDINGS: Experimental work in animal models and the recent human tracheal transplantation suggests that a robust tracheal vascular supply prevents anastomotic complications. Further, this work demonstrates that tracheal allografts undergo a progressive chimerism as recipient epithelium repopulates the allograft. In contrast to most vascularized composite allografts such as hand and

[Application of antero-lateral femoral free skin flap in the repair of hand injuries].

From July 1988 to Jan 1990, 7 cases of hand injuries with skin defects have been reconstructed using anterolateral femoral free flaps. Reconstruction was successful. The anterolateral femoral flap was suitable for the skin defect in the dorsum and wrist of the hand. In this paper, advantages and disadvantages, management of variations in flap vascular pedicle, delayed primary transplantation of the free flap were discussed.

Before or After Transplantation? A Review of the Cost Effectiveness of Treating Waitlisted Patients With Hepatitis C.

All patients with chronic hepatitis C virus (HCV) infections can and should be treated. Though highly effective direct-acting antiviral therapies are costly, the price of a cure is a 1-time investment that is outweighed by future benefits. For clinicians caring for patients requiring liver transplant, the key question relates to the timing of treatment: before or after liver transplantation? On 1 hand, treating HCV often improves our patients' model for end-stage liver disease (MELD) score, decreasing costs, and potentially improving longevity by reducing our patients' risk of death and transp

Cadaveric Bilateral Proximal Forearm Allotransplantation.

Until, sometime ago, microsurgery meant mainly covering a defect or replanting severed parts back to where they belong. Now, restoration of original function and aesthetic consideration is a must in planning reconstructive procedure. Hand transplant combines hand surgery and microsurgery with complex multidisciplinary care.At the anniversary of our first cadaveric bilateral proximal forearm transplantation done in the country's government institute, we would like to share our experience in performing the surgery, outcomes so far, complications, and lessons learned, to contribute to the growing
