Issues, goals, and guidelines in selecting first-line drug therapy for hypertension.

Modern antihypertensive therapy is enriched by an explosion in drug development that makes available increasingly specific agents whose effects have advanced our understanding of pressor mechanisms. This and other research into hypertensive mechanisms has defined the clinical, pharmacological, and endocrinologic heterogeneity of human hypertension. The sum of these developments is a greatly enhanced ability to identify curable and definable causes of hypertension and to pathophysiologically stratify the remaining cases of essential hypertension. Modern treatment can be much more specific than

Protocol for the Development of The Australian Motor Neurone Disease (MND) Guideline.

INTRODUCTION: Motor neurone disease (MND) is a rare but devastating neurodegenerative condition that results in progressive muscle weakness, disability, and eventual respiratory failure. In Australia, the impact of MND is significant, and there is a pressing need for a high-quality, locally relevant guideline to support equitable, person-centred care. QUESTIONS: This guideline aims to address key clinical and care-related questions that arise across the MND trajectory. It will identify the most pressing topics and uncertainties in MND management through interest holder consultation and struct

Navigating barriers: two-year follow up on recommendations to improve the use of maternal health guidelines in Kosovo.

BACKGROUND: Although there are a growing number of initiatives aimed at supporting guideline implementation in resource-constrained settings, few studies assess progress on achieving next steps and goals after the initial activities are completed and the initial funding period has ended. The aim of the current study was to conduct a qualitative process evaluation of progress, barriers, facilitators, and proposed solutions to operationalize nine recommendations to prepare Kosovo to implement the 2012 World Health Organization (WHO) prevention and treatment of postpartum haemorrhage guideline.

Advice on the response from public and environmental health to the detection of cryptosporidial oocysts in treated drinking water. PHLS Advisory Committee on Water and the Environment.

New water quality regulations in the United Kingdom (UK) will lead to the regular sampling of water supplies for the presence of cryptosporidial oocysts. It is likely therefore that consultants in communicable disease control (CCDCs) and environmental health officers (EHOs) will be informed of the presence of oocysts in water supplies rather more often. These guidelines were developed, by an ad hoc working group of the PHLS Advisory Committee on Water and the Environment, to help CCDCs and EHOs respond to such reports. The implications for public health of oocysts in treated water supplies is

Removing Unnecessary Medical Barriers to Contraception: Celebrating a Decade of the U.S. Medical Eligibility Criteria for Contraceptive Use.

In 2010, the Centers for Disease Control and Prevention (CDC) released the U.S. Medical Eligibility Criteria for Contraceptive Use, providing recommendations for health care providers on safe use of contraception for people with certain characteristics or medical conditions. Adapted from World Health Organization guidance, the goal of the recommendations is to remove unnecessary medical barriers to contraception. Over the past decade, CDC has updated recommendations based on new evidence, collaborated with national partners to disseminate and implement the guidelines, and conducted provider su

[Development of the practical implementation of quality indicators on palliative care wards in Germany - Results of a Delphi survey].

INTRODUCTION: The S3 Guideline on Palliative Care for Patients with Incurable Cancer and the quality indicators (QIs) formulated therein are intended to contribute to improving palliative care for patients with incurable cancer. There are reservations and barriers to the practical implementation of the guideline content and QIs. To date, there have been no specific ideas for the further development of the implementation and feasibility of QIs from the S3 guideline on palliative care. The aim of this sub-project is to merge the results from the previous project phases and to agree on recommenda

Healthy children ready to learn: an essential collaboration between health and education.

The "Healthy Children Ready to Learn" initiative starts with the underlying concept that health is a critical partner to optimum education. All children have a right to be healthy. At a minimum, this right assumes promoting optimum use of available and effective preventive measures, such as ensuring compliance with immunization recommendations; promoting measures to prevent injuries; ensuring opportunities to identify disease and disabilities early; and providing prompt treatment when needed. Families must receive the support and assistance they need to raise healthy and educated children. Act

Variation in adherence to the treatment guidelines for Neisseria gonorrhoeae by clinical practice setting, California, 2009 to 2011.

BACKGROUND: Declining susceptibility of Neisseria gonorrhoeae to available antimicrobial agents has prompted repeated updates of the Centers for Disease Control and Prevention (CDC) treatment guidelines. The only regimen currently recommended as first-line treatment is dual therapy consisting of an intramuscular dose of ceftriaxone together with azithromycin or doxycycline. The objective of this analysis is to identify how adherence to the CDC guidelines varies by clinical practice setting. METHODS: A geographically representative random sample of N. gonorrhoeae cases reported from 2009 to 20

Moving sexual health into primary care.

The rise in sexually transmitted infections (STIs) and the high rate of teenage pregnancies mean that sexual health services need to be increasingly provided in primary care settings by primary care professionals. Guidance from the National Institute of Clinical Excellence (NICE) in 2007 recommends proactive intervention sessions with individuals whose sexual history identifies them as at risk. Midwives and health visitors should make regular visits to under-18s who are pregnant or already mothers to provide advice on sexual health and contraception. Primary care professionals should be proact

Providers' perceptions of implementing standardized postpartum family planning: a qualitative study of midwives and nurses in Ghana.

BACKGROUND: The World Health Organization publishes and maintains evidence-based guidance for the delivery of essential family planning services in the postpartum period. Despite significant time and effort in the development of these guidelines in the past decades, a significant unmet need for postpartum family planning persists, exposing an implementation gap. Implementation strategies involving education and infrastructure support can improve family planning uptake and mobile health interventions are a feasible adjunct in improvement of health outcomes. OBJECTIVE: We sought to understand p

The application of disability data from epidemiological surveys to the development of indicators of service needs for dependent elderly people.

Increases in the prevalence of disability due to the ageing of the population have given rise to the need for simple indicators which may be used to estimate service needs and to assess whether resources have been fairly distributed. The present study involved the validation of an indicator based on confinement level by reference to epidemiological studies of disability, social support, and living conditions in three regions of France. Using factorial correspondence analysis and regression methods it was found that such an indicator could adequately summarize more complex empirical observation

Improved Compliance With Anesthesia Quality Measures After Implementation of Automated Monthly Feedback.

PURPOSE: Minimization of postoperative complications is important in patients with cancer. We wished to improve compliance with anesthesiology quality measures through staff education reinforced with automated monthly feedback. METHODS: The anesthesiology department implemented a program to capture and report quality metrics. After staff education, monthly e-mail reports were sent to each anesthesiology physician and nurse anesthetist to detail individual compliance rates for a set of quality measures. For each measure, the proportion of patient cases that passed the measure before and after

Eligibility and outcomes reporting guidelines for clinical trials for patients in the state of a rising prostate-specific antigen: recommendations from the Prostate-Specific Antigen Working Group.

PURPOSE: To define methodology to show clinical benefit for patients in the state of a rising prostate-specific antigen (PSA). HYPOTHESIS: A clinical states framework was used to address the hypothesis that definitive phase III trials could not be conducted in this patient population. PATIENT POPULATION: The Group focused on men with systemic (nonlocalized) recurrence and a defined risk of developing clinically detectable metastases. Models to define systemic versus local recurrence, and risk of metastatic progression were discussed. INTERVENTION: Therapies that have shown favorable effects
