Therapeutic value and price of the new pharmaceuticals commercialized in Argentina: Are they worth what they cost?

In Argentina, new drugs can be authorized by presenting the drug's certificate of approval in at least one of 15 countries considered to have rigorous health surveillance, without needing to carry out a local evaluation of the efficacy, safety or added therapeutic value of the new product. In this article, we evaluate the new drugs commercialized in Argentina in 2016 using different approaches: their approval by other regulatory agencies, the demonstration of their efficacy in randomized clinical trials, types of outcomes studied, rating of their added therapeutic value using two widely recogn

Marijuana and medicine: assessing the science base: a summary of the 1999 Institute of Medicine report.

In response to public pressure to allow the medical use of marijuana, the Office of National Drug Control Policy, Washington, DC, funded a study by the Institute of Medicine evaluating the scientific evidence for benefits and risks of using marijuana as a medicine. The report used scientific reviews, public hearings, and reports from other agencies, and was evaluated by knowledgeable advisors and reviewers. It called for heavier investment in research on the biology of cannabinoid systems, careful clinical studies of cannabinoids in clinical syndromes, analysis of cannabinoids' psychological e

An assessment of potential injury surveillance data sources in Alaska using an emerging problem: all-terrain vehicle-associated injuries.

Using injuries associated with three-wheeled all-terrain vehicles in Alaska as an example, the existing injury data bases were assessed for usefulness, cost, simplicity, acceptability, flexibility, sensitivity, specificity, representativeness, and timeliness. In this study strengths and weaknesses of existing data for all-terrain vehicles were identified and ways to improve data collection and linkages across data systems are suggested. Based on this evaluation, linked death certificates and medical examiner data provide an excellent mechanism for monitoring vehicle-related fatalities. Informa

Towards unified global action on ultra-processed foods: understanding commercial determinants, countering corporate power, and mobilising a public health response.

The rise of ultra-processed foods (UPFs) in human diets is harming global public health. However, policy responses are still emerging-much like tobacco control efforts decades ago-indicating the need to understand root causes and accelerate global action. This paper, the third in a three-part Lancet Series, takes several steps to advance knowledge of these causes, and to inform a global public health response. First, we show that the UPF industry is a key driver of the problem, as its leading corporations and co-dependent actors have expanded and restructured food systems almost everywhere, in

The Mitigating Toxic Stress study design: approaches to developmental evaluation of pediatric health care innovations addressing social determinants of health and toxic stress.

BACKGROUND: Health care administrators must establish and promote effective partnerships with community agencies to address social determinants of health, including reducing exposure of infants and young children to chronic stress. Because infants' experiences are inextricably tied to their caregivers, an important target for mitigating "toxic" stress exposure in early childhood is through reducing parents' experiences of chronic stress in addition to protecting children from direct experiences of harm such as physical or sexual abuse. Conducting screening to identify when children are exposed

Improving Implementation of NCD Care in Low- and Middle-Income Countries: The Case of Fixed Dose Combinations for Hypertension in Kenya.

Health systems in low- and middle-income countries face the challenge of addressing the growing burden of non-communicable diseases (NCDs) with scarce resources to do so. There are cost-effective interventions that can improve management of the most common NCDs, but many remain poorly implemented. One example is fixed dose combinations (FDCs) of medications for hypertension. Included in WHO's Essential Medicines List, FDCs combine two or more blood pressure lowering agents into one pill and can reduce burden on patients and the health system. However, implementation of FDCs globally is poor. W

Responding to epidemics in large-scale humanitarian crises: a case study of the cholera response in Yemen, 2016-2018.

BACKGROUND: Large epidemics frequently emerge in conflict-affected states. We examined the cholera response during the humanitarian crisis in Yemen to inform control strategies. METHODS: We conducted interviews with practitioners and advisors on preparedness; surveillance; laboratory; case management; malnutrition; water, sanitation and hygiene (WASH); vaccination; coordination and insecurity. We undertook a literature review of global and Yemen-specific cholera guidance, examined surveillance data from the first and second waves (28 September 2016-12 March 2018) and reviewed reports on airst

Driving a public health culture of quality: how far down the highway have local health departments traveled?

CONTEXT: There has been an extensive investment in building public health organizational capacity to improve performance and prepare for accreditation. An evolving perspective has focused not only on the practice of quality improvement (QI) within the health department but also upon the extent the culture of QI is embraced within the agency. OBJECTIVE: No studies have examined the current national baseline of QI culture implementation, nor estimated the degree of QI sophistication local health departments (LHDs) have attained. We attempt to fill this void by aligning the findings from the QI

COVID-19 - Enacting a 'new normal' for people who use drugs.

The COVID-19 crisis has magnified existing social, economic and political inequities. People who use drugs are particularly vulnerable due to criminalisation and stigma and often experience underlying health conditions, higher rates of poverty, unemployment and homelessness, as well as a lack of access to vital resources - putting them at greater risk of infection. On the other hand, COVID-19 presents an opportunity to confront the mistakes of the past and re-negotiate a new social contract. The International Network of People who use Drugs (INPUD) believe that this crisis must be an occasion

Medicaid Expansion Increased Medications For Opioid Use Disorder Among Adults Referred By Criminal Justice Agencies.

Individuals involved with the US criminal justice system have high rates of opioid use disorder (OUD) but face significant barriers to evidence-based treatment. Using 2008-17 data from the Treatment Episode Data Set-Admissions, we examined trends in receipt of medications for OUD among individuals referred by criminal justice agencies and other sources both before and after Medicaid expansion. Individuals referred by criminal justice agencies were less likely to receive medications for OUD than were those referred by other sources during our study period, although this disparity narrowed sligh

Public health consequences from hazardous substances acutely released during rail transit--South Carolina, 2005; selected States, 1999-2004.

On January 6, 2005, two freight trains collided in Graniteville, South Carolina (approximately 10 miles northeast of Augusta, Georgia), releasing an estimated 11,500 gallons of chlorine gas, which caused nine deaths and sent at least 529 persons seeking medical treatment for possible chlorine exposure. The incident prompted the Agency for Toxic Substances and Disease Registry (ATSDR) to review data from its Hazardous Substances Emergency Events Surveillance (HSEES) system and update an analysis of 1993-1998 railroad events. The HSEES system is used to collect and analyze data concerning the pu

Perspectives on Turnover and Workforce Shortages in Community Behavioral Health: Implications for Policy and Research.

There is a high rate of worker turnover in public mental health settings, contributing to a critical provider shortage with serious implications for mental healthcare access, particularly for low-income and underserved populations. These challenges unfold within strained public mental health systems characterized by chronic underfunding, high caseloads, and administrative demands, all of which exacerbate workforce instability. The primary objective of this study was to qualitatively examine public mental health workers' perspectives on factors influencing turnover and experiences of ongoing wo

The relation of FIMR programs and other perinatal systems initiatives with maternal and child health activities in the community.

OBJECTIVES: To evaluate the association of the presence of a fetal and infant mortality review (FIMR) program, other perinatal systems initiative (PSI), or both in a community with the performance of essential maternal and child health (MCH) services by local health departments (LHDs). METHODS: Data were obtained from telephone interviews with professionals from LHDs across the United States. Logistic regression was used to estimate the odds of a LHD conducting each essential MCH service in communities with and without FIMR programs or with and without PSIs, adjusted for geographic area. Lessons learned from the COVID-19 pandemic: opportunities and challenges to leverage investments in HIV, tuberculosis and malaria for pandemic preparedness and response.

INTRODUCTION: The COVID-19 pandemic strained public health infrastructure in low-income and middle-income countries (LMICs), leading them to leverage resources of existing public health programmes including HIV, tuberculosis (TB) and malaria. This manuscript systematically documents how these programmes were used in response to understand both the barriers and facilitators encountered in the process. METHODS: From March through August 2023, we conducted 72 structured interviews with staff in 13 countries who played central roles in the response. Countries were purposively selected from the po

Crimean-Congo hemorrhagic fever virus in Central, Eastern, and South-eastern Asia.

Crimean-Congo hemorrhagic fever (CCHF), caused by Crimean-Congo hemorrhagic fever virus (CCHFV), is endemic in Africa, Asia, and Europe, but CCHF epidemiology and epizootiology is only rudimentarily defined for most regions. Here we summarize what is known about CCHF in Central, Eastern, and South-eastern Asia. Searching multiple international and country-specific databases using a One Health approach, we defined disease risk and burden through identification of CCHF cases, anti-CCHFV antibody prevalence, and CCHFV isolation from vector ticks. We identified 2313 CCHF cases that occurred in 194

Telehealth Policy, Practice, and Education: a Position Statement of the Society of General Internal Medicine.

Telehealth services, specifically telemedicine audio-video and audio-only patient encounters, expanded dramatically during the COVID-19 pandemic through temporary waivers and flexibilities tied to the public health emergency. Early studies demonstrate significant potential to advance the quintuple aim (patient experience, health outcomes, cost, clinician well-being, and equity). Supported well, telemedicine can particularly improve patient satisfaction, health outcomes, and equity. Implemented poorly, telemedicine can facilitate unsafe care, worsen disparities, and waste resources. Without fur

Decolonizing global health in an age of fragmentation: reimagining equity for universal health coverage.

The global health landscape is undergoing a significant transformation as traditional frameworks of cooperation face fragmentation amid geopolitical tensions. Declining support from Western nations, exemplified by US withdrawal from the World Health Organization and cuts to programs like the President's Emergency Plan for AIDS Relief, has exposed the profound instability of an aid architecture built on colonial dependencies. The COVID-19 pandemic, marked by vaccine nationalism, was a stark litmus test of this systemic failure for low- and middle-income countries (LMICs). This commentary argues

Resurgence of Ebola in the Democratic Republic of Congo: Lessons Learned and Future Directions.

INTRODUCTION: The Democratic Republic of the Congo (DRC) remains the global epicenter of Ebola virus disease (EVD), having experienced more than 15 outbreaks since the disease was first identified in 1976. The 2025 resurgence in Kasai Province, caused by Orthoebolavirus zairense, resulted in over 60 confirmed cases and a case fatality rate exceeding 65%, highlighting persistent vulnerabilities in outbreak preparedness and response despite previous control successes. This article examines the drivers, response strategies, and public health implications of the 2025 Ebola outbreak. METHODS: This
