Advances in left bundle branch area pacing: current evidence, clinical applications, and future perspectives.

INTRODUCTION: Left bundle branch area pacing (LBBAP) resynchronizes the left ventricle through the conduction system rather than through a coronary sinus lead, and is increasingly used as an alternative to biventricular pacing (BVP) for cardiac resynchronization therapy (CRT) in heart failure with reduced ejection fraction. AREAS COVERED: Drawing on a search of PubMed, Embase, the Cochrane Library and ClinicalTrials.gov from January 2015 through September 2026, this review covers the anatomical basis of LBBAP, implantation and confirmation of capture, complications, observational and randomiz

T-type channels in the sino-atrial and atrioventricular pacemaker mechanism.

Cardiac automaticity is a fundamental physiological function in vertebrates. Heart rate is under the control of several neurotransmitters and hormones and is permanently adjusted by the autonomic nervous system to match the physiological demand of the organism. Several classes of ion channels and proteins involved in intracellular Ca(2+) handling contribute to pacemaker activity. Voltage-dependent T-type Ca(2+) channels are an integral part of the complex mechanism underlying pacemaking. T-type channels also contribute to impulse conduction in mice and humans. Strikingly, T-type channel isofor

Physiologic Variants of Cardiac Conduction (Aberration, Gap, Supernormal Conduction).

Wide QRS complexes during supraventricular rhythms can be caused by fixed bundle branch block, functional (intermittent) bundle branch block, preexcitation, or toxic/metabolic causes. Functional bundle branch block can be caused by long-short aberrancy (usually physiologic), or acceleration/deceleration dependent aberrancy (usually pathologic). Electrocardiogram criteria have been proposed to differentiate aberration from ventricular tachycardia; they are not always accurate. The gap phenomenon "paradox" is that with increasingly premature extrastimuli, progressive proximal conduction delay al

An uncommon case of complete AV block.

BACKGROUND: Granulomatosis with polyangiitis (GPA) is a rare systemic inflammatory disorder characterized by vasculitis of the small vessels, as well as necrotizing granulomatous lesions, affecting mainly upper and lower respiratory tracts, lungs and kidneys. Cardiac involvement has traditionally been a rare manifestation of GPA, with misleading clinical presentation until late stages. Cardiac conducting tissue involvement is a rare and potentially life-threatening complication. CASE PRESENTATION: We report the case of a 45-year-old man diagnosed with GPA with typical symptoms, but also compl

The cardiac conduction system: History, development, and disease.

The heart is the first organ to form during embryonic development, establishing the circulatory infrastructure necessary to sustain life and enable downstream organogenesis. Critical to the heart's function is its ability to initiate and propagate electrical impulses that allow for the coordinated contraction and relaxation of its chambers, and thus, the movement of blood and nutrients. Several specialized structures within the heart, collectively known as the cardiac conduction system (CCS), are responsible for this phenomenon. In this review, we discuss the discovery and scientific history o

Cardiac changes implicated in chronic heart block.

Fifteen cases of chronic heart block were studied. Eight of them could be designated as idiopathic or primary heart block; the others were associated with hypertension, diabetes and ischaemic heart disease, either singly or in various combinations. In six cases, the whole heart was available for histopathological study of the conduction system. In the other 9 cases, only a portion of the heart muscle was available for examination. A V nodal fibrosis extending upto the proximal bundle of His was seen in all the six whole heart autopsy materials. Fibrosis of the adjacent myocardium was seen in f

Frequency and clinical outcome in conduction defects in acute myocardial infarction.

BACKGROUND: Conduction defects complicating acute myocardial infarction (MI) are frequent and associated with increased mortality and complications. Common conduction defects after acute MI are atrioventricular nodal blocks (1st, 2nd and 3rd degree) and intraventricular conduction defects (right or left bundle branch blocks and hemiblocks). In myocardial infarction occlusion of coronary arteries at different levels affects the conduction system of heart leading to various types of blocks. Conduction defects usually reflect extensive damage to the myocardium. METHODS: In this descriptive case

Blood Pressure Control and Incident Left-Ventricular Conduction Disease.

PURPOSE OF REVIEW: Cardiac conduction disease, a harbinger of pacemaker implantation, heart failure, and death, is commonly regarded as immutable. However, emerging research suggests it may be a target for upstream prevention strategies such as blood pressure management. This review summarizes recent evidence regarding blood pressure control and the development of conduction disease. RECENT FINDINGS: Recent observational studies link hypertension to both prevalent and incident conduction disease. In randomized trials, intensive blood pressure control among hypertensive individuals reduced the

Autoantibodies in Atrial Fibrillation-State of the Art.

Atrial fibrillation (AF) is the most common type of cardiac arrhythmia. To date, a lot of research has been conducted to investigate the underlying mechanisms of this disease at both molecular and cellular levels. There is increasing evidence suggesting that autoimmunity is an important factor in the initiation and perpetuation of AF. Autoantibodies are thought to play a pivotal role in the regulation of heart rhythm and the conduction system and, therefore, are associated with AF development. In this review, we have summarized current knowledge concerning the role of autoantibodies in AF deve

Heart ventricular activation in VAT difference maps from children with chronic kidney disease.

Children with chronic kidney disease (CKD) are affected by cardiovascular complications, including disturbances in the intraventricular conduction system. Body surface potential mapping (BSPM) is a non-invasive method of assessing the cardioelectrical field. Our aim was to investigate conduction disturbances in young CKD patients using ventricular activation time (VAT) maps. Our study comprised 22 CKD children (mean age: 13.1 ± 2.5 years) treated conservatively and 29 control patients. For each child 12-lead electrocardiogram (ECG) readings were taken, and blood pressure and serum concentratio

Sudden cardiac death in young people with apparently normal heart.

OBJECTIVE: The aim of the present study was to assess the prevalence of subtle morphologic substrates, clinically unrecognizable, underlying sudden cardiac death (SCD) in young people with apparently normal heart. METHODS: In the time interval 1979--1998, 273 consecutive cases of SCD in young people (< or =35 years) which occurred in the Veneto Region of Italy were prospectively studied. Following exclusion of extracardiac causes of sudden death, the heart was examined according to a detailed morphologic protocol consisting of macroscopic and histologic examination, including study of the spe

The cardiac conduction system: development, function and therapeutic targets.

The cardiac conduction system (CCS) has a vital role in initiating and coordinating nearly 3 billion heartbeats throughout a person's lifetime. The CCS comprises two primary tissue types: the impulse-generating, slow-conducting nodes and the fast-conducting components of the ventricular conduction system. Dysfunction in this system can give rise to a spectrum of clinical symptoms, including palpitations, syncope, heart failure and even sudden cardiac death. Owing to the limited therapeutic options other than electronic pacemakers, substantial research efforts have been aimed at uncovering the

Left bundle branch pacing in patients with structural heart disease: personalizing cardiac resynchronization therapy.

Biventricular pacing remains the cornerstone of cardiac resynchronization therapy (CRT) in patients with heart failure, with well-established benefits. Left bundle branch pacing (LBBP) offers a physiologic alternative by engaging the native conduction system to restore synchrony and has generated significant enthusiasm. However, the growing adoption of LBBP should be tempered by recognition that a one-size-fits-all approach may not address the underlying substrate, particularly in those with intraventricular conduction delay. While a less-than-optimal LBBP implant may be sufficient in bradycar

[Pathophysiology of atrioventricular conduction disturbances in sports practitioners].

In athletes practicing endurance sports, atrioventricular conduction disturbances may be physiological. This holds true for first-degree atrioventricular blocks, type 1 second-degree atrioventricular blocks, and slight ventricular activation delays (slurred S wave in V1). On the contrary, right (RBBB) and left bundle branch blocks (LBBB) and fascicular blocks (left anterior [LAH] and left posterior hemiblocks [LPH]), either isolated or associated with RBBB, are rare in asymptomatic athletes. The latter can be the consequence of structural heart disease or primary degeneration of the intraventr
