Quality of life of patients with atrial septal defect following percutaneous closure.

INTRODUCTION: Quality of life has become an important outcome measure in addition to mortality and morbidity in patients with congenital heart disease. Atrial septal defect is a common congenital heart disease, and transcatheter atrial septal defect closure has become an accepted treatment modality. The aim of this study is to assess the quality of life of patients with atrial septal defect who underwent percutaneous closure. MATERIALS AND METHODS: We examined the quality of life of 69 patients with atrial septal defect and 69 healthy controls matched according to age, sex, educational level,

The Congenital Malformation of the Interatrial Septum-A Review of Its Development and Embryology with Clinical Implications.

The development process of the heart and cardiovascular system is fundamental in human development and highly regulated by genetic factors. This process needs to be highly regulated to prevent malformations. Nevertheless, some heart defects may be identified, especially with modern imaging methodology. Atrial septal defects (ASDs) are particularly common. Understanding the mechanisms involved in ASD formation is fundamental for developing new treatment strategies. In this article, we explore cardiac development and embryology, with a focus on atrial septal defects and their clinical implicatio

[Temporary occlusion of atrial septal defect in the Lutembacher syndrome].

We report the case of an 82 year-old woman with symptoms of advanced heart failure and pulmonary arterial hypertension. An echocardiogram showed an ostium secundum type atrial septal defect and concomitant mitral valve stenosis (Lutembacher syndrome). Echocardiographic assessment of mitral pathology was hampered by the interatrial septal defect. Transient percutaneous occlusion test of the atrial septal defect was performed and severe mitral valve stenosis was detected. Atrial septal defect size modified the clinical manifestations and the transient occlusion test helped to decide the therapeu

Primum atrial septal defect.

UNLABELLED: The purpose of this report is to review our surgical experience with primum atrial septal defect. Since 1982, infants with primum atrial septal defect have undergone complete repair consisting of closure of the cleft of the left atrioventricular valve and atrial septal defect with a pericardial patch. Ages at operation ranged from early neonatal period until 5 years. In most patients, echocardiography was diagnostic and cardiac catheterization was performed in children with associated defects. Severe congestive heart failure and left atrioventricular valve regurgitation necessitate

Pathophysiology and natural history of atrial septal defect.

Atrial septal defects are among the third most common types of congenital heart disease. This group of malformations includes several types of atrial communications allowing shunting of blood between the systemic and the pulmonary circulations. The nature of shunting across the defect in patients with atrial septal defect is of particular interest. Understanding the underlying mechanisms of interatrial shunts and their natural history will help selecting the best timing for closure, before irreversible cardiac and pulmonary injury occur. This review describes the different pathophysiologic mec

Sinus venosus defect of the pulmonary vein-type: An easily missed diagnosis.

Atrial septal defects are one of the most common forms of congenital heart disease, however sinus venosus communications, particularly pulmonary vein-type defects, are rare and are easily misdiagnosed. Patients with pulmonary vein-type sinus venosus defects often present earlier than those with ostium secundum defects with significant right heart dilation. Correct diagnosis has important implications for management. We discuss the clinical courses and review multimodality imaging of three patients correctly diagnosed with pulmonary vein-type defects after an initial diagnosis of an ostium secu

Morphogenesis and molecular considerations on congenital cardiac septal defects.

The primary unseptated heart tube undergoes extensive remodeling including septation at the atrial, atrioventricular, ventricular, and ventriculo-arterial level. Alignment and fusion of the septal components is required to ensure full septation of the heart. Deficiencies lead to septal defects at various levels. Addition of myocardium and mesenchymal tissues from the second heart field (SHF) to the primary heart tube, as well as a population of neural crest cells, provides the necessary cellular players. Surprisingly, the study of the molecular background of these defects does not show a great

Familial secundum atrial septal defect with dysrhythmia associated with web neck.

Most cases of atrial septal defect occur sporadically, but a few families have the defect as a genetic abnormality. A family having familial type secundum atrial septal defect with dysrhythmia associated with web neck is reported. In this family, two female siblings aged 11 (Case 1) and 4 years (Case 2) and their father had secundum atrial septal defect. Case 1 presented with two year history of syncope attacks and Case 2 with easy fatigability since early childhood. Both sisters also had web neck as a solitary anomaly. Electrocardiograms revealed prolonged PR interval and right bundle-branch

Narrowing the Survival Gap: Trends in Survival of Individuals with Down Syndrome with and without Congenital Heart Defects Born 1979 to 2018.

OBJECTIVE: To evaluate the hypothesis that childhood survival for individuals with Down syndrome (DS) and congenital heart defects (CHDs) has improved in recent years, approaching the survival of those with DS without CHDs. STUDY DESIGN: Individuals with DS born from 1979 to 2018 were identified through the Metropolitan Atlanta Congenital Defects Program, a population-based birth defects surveillance system administered by the Centers for Disease Control and Prevention. Survival analysis was performed to evaluate predictors of mortality for those with DS. RESULTS: The cohort included 1671 in

Iatrogenic atrial septal defect caused by repeated catheter ablation.

Iatrogenic atrial septal defect is an issue after percutaneous interventions for structural heart disease. A 63-year-old man, who had previously received 5 catheter ablations for paroxysmal atrial fibrillation, was found to have an iatrogenic atrial septal defect that persisted after the fourth intervention. Approximately 4 years later, he suffered exertional dyspnea. Pulmonary hypertension was caused by a left-to-right shunt via a large iatrogenic atrial septal defect. We performed surgical closure and the symptom improved. The timing of treatment for persistent iatrogenic atrial septal defec

Severe hypoxia related to uncomplicated atrial septal defect.

A 73-year-old man was admitted to our institution with severe hypoxia. Cardiac catheterization and transesophageal echocardiography revealed an atrial septal defect with an interatrial right-to-left shunt but with no pulmonary hypertension. Direct examination at surgery revealed an elongated thoracic aorta that caused the aortic annulus to reside at the level of the diaphragm and the heart to be positioned transversely in the mediastinum. Surgical closure of the atrial septal defect normalized oxygenation. We attribute the unusual occurrence of an atrial septal defect and a right-to-left shunt

[Linear insertion of the atrioventricular valves without defect].

On a first anatomical series of 52 hearts of trisomic 21 fetuses, published in June 2002, we described a new minor cardiac anomaly, belonging to the atrioventricular septal defect, with a linear insertion of the atrioventricular valves without defect. We want to confirm these data, on a larger series of 213 new hearts of trisomic 21 fetuses by adding a complementary section to the standard examination; 100% of controls have shown a normal insertion with an offsetting of the atrioventricular valves. On 113 out of these 213 hearts of trisomic 21 fetuses, with a so called "normal" heart at the st

[Interatrial communication associated with dilated cardiomyopathy].

A case of atrial septal defect in association with dilated cardiomyopathy is presented. Evolutive features are mainly assessed by echocardiography. On initial presentation the findings on physical examination pointed to the diagnosis of atrial septal defect with severe congestive heart failure. In addition to the demonstration of the congenital heart disease, echocardiography showed very dilated right atrial and ventricular cavities and later appearance of right atrium intracavitary thrombi. Left cavities were initially normal but, after surgical repair of the atrial septal defect, they rapidl

Descriptive epidemiology of selected congenital heart defects, Hawaii, 1986-1999.

Congenital heart defects are the most common type of birth defect and contribute the most to infant mortality due to birth defects. This study examined the relationship between several demographic factors and selected congenital heart defects among the unique multiethnic population in Hawaii during 1986-99, using data from a population-based birth defects registry. Rates were significantly higher in 1993-99 than in 1986-92 for transposition of the great arteries and Ebstein's anomaly, and significantly lower for tetralogy of Fallot. Significantly elevated rates were found with maternal age of

Corrected transposition of the great vessels associated with an obscured ventricular septal defect due to an abnormal systemic atrioventricular valve (report of a case and surgical considerations).

A case of corrected transposition associated with atrial septal defect, obscured ventricular septal defect, mild pulmonary valvar stenosis, severe systemic atrioventricular valve regurgitation and complete heart block is described. Closure of an atrial septal defect and a ventricular septal defect was carried out through a left atriotomy. Systemic atrioventricular valve replacement with a 27 mm. Hancock porcine valve was performed through the same approach. A permanent Demand rechargeable pulse generator unit with myocardial unipolar electrode lead was implanted.
