We report the case of a women, 40 years old, suffering from a severe mitral insufficiency caused by the simultaneous presence of three anatomical anomalies: mitral valve prolapse, mitral anulus dilation and calcification. Using the techniques of decision analysis, several possible mitral valve prolapse screening strategies for U.S. Air Force aircrew candidates are evaluated. Available clinical and epidemiological data are organized so that the relative merits of various strategies can be assessed by decision makers. The obtained experience of echocardiography use in wide population examinations demonstrate that it is of practical value for diagnosis of lesions of heart valves or other heart structures (about 10% of pathology in grown-up population). The efficacy of the method could be highly increased (from 37% to 81%) if the investigation was performed in patients with murmurs above the heart area, with cardiovascular diseases in anamnesis and with marked ECG alterations. Children and adolescents exhibited mainly various types of mitral valve prolapse.

Sonocardiography, especially bi-dimensional, currently represents an examination of choice in the detection of valvular prolapses. In a retrospective ultrasonographic study, 32 cases of acquired disease of the tricuspid valve were detected amongst 7000 consecutive patients. Forty patients with simple atrial septal defect had left ventriculograms using cineangiography and/or Elema or Sircam still films.
