Gastroesophageal reflux. Don't forget to ask about heartburn.

At least 13% of the US adult population have chronic heartburn, and unknown percentages of others have related symptoms of gastroesophageal reflux disease. Studies show that most people with symptoms of reflux self-medicate and do not seek medical advice. Also, they fail to mention these symptoms when visiting their physician for other conditions. Therefore, the importance of thorough history taking cannot be overstated, and it may be prudent for primary care physicians to question all patients routinely about reflux symptoms.

Managing heartburn at the 'base' of the GERD 'iceberg': effervescent ranitidine 150 mg b.d. provides faster and better heartburn relief than antacids.

BACKGROUND: Many individuals with heartburn self-medicate with antacids for relief of their symptoms. AIM: To compare efficacy of effervescent ranitidine to as-needed calcium carbonate antacids in subjects who self-treat heartburn. METHODS: A total of 155 subjects with frequent antacid-responsive heartburn were randomized to receive effervescent ranitidine 150 mg tablets b.d., or as-needed calcium carbonate 750 mg for 12 weeks. Endoscopic oesophagitis severity and mucosal histology were assessed at baseline, and at weeks 6 and 12. Heartburn frequency, severity, and antacid consumption were r…

Functional heartburn: An underrecognized cause of PPI-refractory symptoms.

Functional heartburn--persistent symptoms of esophageal reflux with no objective evidence of gastroesophageal reflux disease (GERD)--is the most common cause of failure of proton pump inhibitor (PPI) therapy, but it is often overlooked by internists and gastroenterologists.

Population-based assessment of heartburn in urban Black Americans.

Prevalence data for heartburn in the urban Black American community is lacking. In order to estimate prevalence for this community, we analyzed data from an ongoing cohort study in progress at our hospital. Comprehensive interviews allowed for exploration of factors associated with heartburn. Complex, stratified sampling design was the method used. Survey invitations are hand-delivered to random blocks in a single zip code tabulation area. One member per eligible household is invited to complete a computer-based survey. Heartburn was defined as ≥ 3 days/week of symptoms as defined by the Montr…

Heterogeneity of endoscopy negative heartburn: epidemiology and natural history.

It has now become clear that only about 40% or less of patients with heartburn and/or regurgitation have esophagitis, and that the majority of them lack visible distal esophageal mucosa breaks. These subjects are referred to as non-erosive gastroesophageal reflux disease (NERD) patients. It has been estimated that in the Western world at least one tenth of the general population has at least weekly heartburn. This proportion seems to be lower in Asia, while prevalence is rapidly increasing. Although it would be extremely useful to have prospective information regarding the fate of such patient…
