Mechanical analysis of the landing phase in heel-toe running.

Results of mechanical analyses of running may be helpful in the search for the etiology of running injuries. In this study a mechanical analysis was made of the landing phase of three trained heel-toe runners, running at their preferred speed and style. The body was modeled as a system of seven linked rigid segments, and the positions of markers defining these segments were monitored using 200 Hz video analysis. Information about the ground reaction force vector was collected using a force plate. Segment kinematics were combined with ground reaction force data for calculation of the net inters…

Percussion Massage Therapy Reduced EMG Activity During Standing Heel Raise.

OBJECTIVES: This study examined the acute impact of percussion massage therapy (PMT) on calf muscle activation during heel raises in individuals with and without chronic ankle instability (CAI). METHODS: Thirty-nine university students, 20 with CAI and 19 controls, were randomized to 30 seconds of PMT or no intervention (NOPMT). Surface EMG measured medial gastrocnemius activity during heel raise before and after. RESULTS: Pre-intervention, CAI limbs displayed significantly lower peak muscle activation than limbs without CAI (26%, p = 0.012). Post-intervention, both PMT groups showed signifi…

Fat transfer for plantar heel pain: A retrospective case series.

BACKGROUND: Plantar heel pain (PHP) affects 4-7% of the population. It encompasses a range of different pathologies. Heel fat pad atrophy is thought to be the second leading cause of PHP. Atrophy of the heel fat leads to decreased cushioning over the calcaneus. The associated pain can be disabling and lead to reduced mobility and social isolation. This case series describes how fat transfer can alleviate symptoms of PHP. METHODS: Twenty-one procedures of fat transfer were performed using the Coleman technique for PHP between 2013 and 2023. All surgical candidates had clinical and/or radiologi…

Plasmodium telomere maintenance: uncovering the Achilles' heel for novel antimalarials.

This review examines the potential of disrupting telomere maintenance in Plasmodium as a novel antimalarial strategy. Telomeres are repetiti … s, and roles in pathogenesis. We further explore how selective targeting could exploit an Achilles' heel in parasite survival, offering fresh possibilities for next-generation, parasite-specific malaria therapies.

Flavivirus proteases: The viral Achilles heel to prevent future pandemics.

Flaviviruses are important human pathogens and include dengue (DENV), West Nile (WNV), Yellow fever virus (YFV), Japanese encephalitis (JEV) and Zika virus (ZIKV). DENV, transmitted by mosquitoes, causes diseases ranging in severity from mild dengue fever with non-specific flu-like symptoms to fatal dengue hemorrhagic fever and dengue shock syndrome. DENV infections are caused by four serotypes, DENV1-4, which interact differently with antibodies in blood serum. The incidence of DENV infection has increased dramatically in recent decades and the CDC estimates 400 million dengue infections occu

Heel Wounds Predict Mortality but Not Amputation after Infrapopliteal Revascularization.

BACKGROUND: Ischemic heel ulcerations are generally thought to carry a poor prognosis for limb salvage. We hypothesized that patients undergoing infrapopliteal revascularization for heel wounds, either bypass or endovascular intervention, would have lower wound healing rates and amputation-free survival (AFS) than patients with forefoot wounds. METHODS: A retrospective chart review was performed on patients who presented between 2006 and 2013 to our institution with ischemic foot wounds and infrapopliteal arterial disease and underwent either pedal bypass or endovascular tibial artery interve…

EFFECT OF HEEL LIFTS ON PATELLOFEMORAL JOINT STRESS DURING RUNNING.

BACKGROUND: Patellofemoral pain is a debilitating injury for many recreational runners. Excessive patellofemoral joint stress may be the underlying source of pain and interventions often focus on ways to reduce patellofemoral joint stress. PURPOSE: Heel lifts have been used as an intervention within Achilles tendon rehabilitation programs and to address leg length discrepancies. The purpose of this study was to examine the effect of running with heel lifts on patellofemoral joint stress, patellofemoral stress impulse, quadriceps force, step length, cadence, and other related kinematic and spa…

Novel Surgical Treatment for Refractory Heel Ulcers in Werner's Syndrome.

Patients with Werner's syndrome frequently develop chronic leg ulcers that heal poorly. We present a patient who suffered from this rare syndrome and developed typical heel ulcers. Treatment of the ulcer is challenging, as flap options are limited over the lower third of the leg and skin grafting is not easy as there is a lack of healthy granulations. We successfully treated the ulcer with osteomyelitis by drilling the bone and applying an ultrathin split thickness skin graft with the thigh skin as the donor site.

Venepuncture versus heel lance for blood sampling in term neonates.

BACKGROUND: Heel lance has been the conventional method of blood sampling in neonates for screening tests. Neonates undergoing heel lance experience pain which cannot be completely alleviated. OBJECTIVES: To determine whether venepuncture or heel lance is less painful and more effective for blood sampling in term neonates. SEARCH STRATEGY: Randomized or quasi-randomised controlled trials comparing pain response to venepuncture versus heel lance were identified by searching the Cochrane Central Regsiter of Controlled Trials (CENTRAL, The Cochrane Library), MEDLINE, EMBASE, CINAHL, and clinica…

Targeted therapies in solid tumours: pinpointing the tumour's Achilles heel.

It is now exactly 100 years ago (1908) that Paul Ehrlich, who is regarded as the inventor of the concept of targeted therapy, received the Nobel Prize for Medicine. His initial perception leading to this theory was derived from observations that certain substances are capable of selectively staining either tissues or microorganisms. These observations culminated in the discovery of the inorganic mercury compound arsphenamine (Salvarsan) by Sahachiro Hata in the laboratory of Paul Ehrlich. Salvarsan might be regarded as the first effective "targeted" treatment for syphilis at that time. Tamoxif

Factors associated with chronic plantar heel pain: a systematic review.

Chronic plantar heel pain (CPHP) is one of the most common soft tissue disorders of the foot, yet its aetiology is poorly understood. The purpose of this systematic review was to examine the association between CPHP and the various aetiological factors reported in the literature. Seven electronic databases and the reference lists of key articles were searched in August 2005. The resulting list of articles was assessed by two independent reviewers according to pre-determined selection criteria and a final list of articles for review was created. The methodological quality of the included articl…

Posterior heel pain (retrocalcaneal bursitis, insertional and noninsertional Achilles tendinopathy).

The majority of patients with Achilles tendinopathy respond to nonoperative treatment. In patients with refractory symptoms, surgery can be considered. If paratenonitis is present, the paratenon is partially excised, and adhesions are released. Areas of symptomatic tendinosis are excised with repair of the residual defect in the Achilles tendon. An alternative for patients with tendinosis who are at increased risk for wound problems or who do not want a large open incision is percutaneous or endoscopic tenotomy. A symptomatic Haglund's deformity or inflamed retrocalcaneal bursa is excised. Aug

Newborn infants' cry after heel-prick: analysis with sound spectrogram.

The aim of the study was to test the hypothesis that a newborn infant's cry can be used in conjunction with an instrument to measure pain. Crying due to pain was analysed after a heel-prick stimulus. In a prospective, descriptive study, 50 healthy newborn infants were subjected to a heel-prick for phenylketonuria screening. Their cries of pain were recorded and analysed. Duration of the crying sound was analysed and, using a sound spectrogram, the fundamental frequency and the cry melody of the first five cry sounds were analysed. The analysis showed that the crying sound after the painful sti…

Entrapment neuropathy: the etiology of intractable chronic heel pain syndrome.

Chronic heel pain syndrome (CHPS) is a common clinical entity. The etiology of CHPS has never been completely defined and there are no clear treatment regimens in the literature. Most authors agree that nonoperative treatment is effective in most patients. However, in 5%-10% of patients, operative intervention is required. Outcomes for these patients have been inconsistent. A series of 51 patients with intractable CHPS who were diagnosed with an entrapment of the posterior tibial nerve and its terminal branches is presented. Descriptive statistics were obtained for the demographic data and pre…

Efficacy of first-time steroid injection for painful heel syndrome.

The purpose of this study was to evaluate the results of a single injection of corticosteroids in patients with painful heel syndrome. Twenty-seven heels in 24 patients were injected with a combination of 1 ml of lidocaine and 1 ml of betamethasone (6 mg). These patients had never previously received an injection to their heels and had continued symptoms of pain after a trial of other nonoperative treatment modalities. After the injection, patients were seen and surveyed periodically for a period of 5 months to 8 months. The amount of pain relief that they obtained, the length of time this las…
