In the treatment of Helicobacter pylori-associated peptic ulcer disease, the complexity (ie, frequency and duration) of drug administration, the presence or development of bacterial antibiotic resistance, and the occurrence of side effects influence patient compliance and eradication rates, which consequently affect the costs of treatment regimens. The National Institutes of Health Consensus Conference and 1997 Digestive Health Initiative Update Conference have recommended that all patients with gastric or duodenal ulcer and H. pylori infection, whether on first presentation or recurrence, be …

Chronic urticaria can result from multiple causes. A number of factors have been identified that can appear to be important in the pathogenesis of individual cases, including intolerance to food, drugs, some internal diseases and some infections. Recently a possible relationship between chronic urticaria and Helicobacter pylori has been suggested. One hundred and twenty-five patients were investigated for Helicobacter pylori infection by means of ELISA assay and 13C urea-breath tests. When the two tests were positive, gastric biopsy was performed after informed consent. Patients with Helicobac…

BACKGROUND: CagA antigen of Helicobacter pylori is highly immunogenic in humans. There is an increasing evidence that infection with CagA-positive strains is related to the development of peptic ulcer disease, atrophic gastritis, or gastric cancer. The aim of our study was to assess seropositivity to CagA in a group of 95 clinically symptomatic adults who underwent gastroduodenoscopy and to correlate results to their disease characteristics.
METHODS AND RESULTS: Serum immunoglobulin G antibodies to CagA detected by ELISA kit (Helicobacter p120, Viva Diagnostika, Germany) were compared to stan…

For many decades the dictum 'no acid, no ulcer' dominated thinking on the pathogenesis of peptic ulcer. When I was a medical student, not su … e first H2-receptor antagonist, cimetidine, two major developments occurred namely the discovery of Helicobacter pylori and the characterisation of the proton pump with the development of drugs to inhibit its action. The discovery of H. pylori not only turned the aetiopathogenesis of peptic ulceration on its head but soon emerged as a major factor in the causa …

Helicobacter pylori infection is now recognised as the major cause of chronic active gastritis and peptic ulcer disease and eradication of the infection will prevent recurrence of the majority of such ulcers. A large number of different treatment combinations have been tried, but 100% H pylori eradication has not been achieved due to the use of wrong drug combinations or dosages, non-compliance and development of primary or acquired bacterial resistant strains. However, consistent 95-96% H pylori eradication can now be achieved with triple therapy employing a high-dose proton pump inhibitor tw…

OBJECTIVES: We investigated the relationships between gastric metaplasia occurring during the healing and scarring stages of duodenal ulcers and Helicobacter pylori by examining the course of gastric metaplasia in H. pylori-eradicated and non-eradicated patients.
METHODS: One hundred and six H. pylori-positive patients with active duodenal ulcers were assigned to either a non-eradication group or an eradication group. The non-eradication group received lansoprazole for 6 wk, followed by an H2-receptor antagonist. The eradication group also received amoxicillin and metronidazole for 1 wk, in a…

In this study, we tested the effectiveness of Rifaximin, a surface antibiotic which is not absorbed when given orally, in the eradication of Helicobacter pylori (HP). The drug was combined in triple therapy either with Amoxicillin and Omeprazole or with Erythromycin-ethylsuccinate and Omeprazole. Twenty-three patients complaining of dyspeptic symptoms and gastric infection due to HP were evaluated. The patients were randomly given one of the following therapeutic protocols: Rifaximin susp. 600 mg/day x 3/day (at least two hours after meals: 10:00 am, 2:00 pm, 9:00 pm), Amoxicillin tab. 1 g x 2…

The clinical and experimental work suggests the following scheme for the pathogenesis of gastric lymphoma. The first step is accumulation of lymphoid tissue (MALT) in response to infection of the stomach by H. pylori. In rare instances, this lymphoid infiltrate contains cells with a growth advantage, possibly because of a genetic change (trisomy 3?). The result is a monoclonal lymphoproliferative lesion that is responsive to H. pylori-driven T-cell help. Further genetic changes [t(1;14)?] may lead to escape from T-cell dependency and, ultimately, high-grade transformation. Low-grade B-cell gas

Gastric carcinoma of the intestinal type is assumed to develop from precancerous gastric lesions. It is now widely accepted that Helicobacter pylori (HP) infection causes chronic gastritis and, after a period of time, intestinal metaplasia (IM). It was suggested that these gastric lesions may evolve into gastric carcinoma after a lengthy latency period. HP seropositivity is high in Turkey at early ages. This may explain the high incidence of gastric carcinoma in this geographic region. In this study, we examine the relationship between HP and IM in endoscopic gastric biopsy specimens. We exami…

Helicobacter pylori is a recently recognized bacterial pathogen associated with diverse pathologies of varying severity, such as chronic gastritis, peptic ulceration, mucosa-associated lymphoid tissue (MALT) lymphoma, and gastric carcinoma. We here present a review of our current knowledge on the properties of H. Pylori that adapt it to its particular niche by allowing it to survive in the stomach and to colonize the gastric mucosa, as well as those that underlie its persistence and pathogenicity. While the bacterial determinants that preclude the persistent colonization of the gastric mucosa …

Helicobacter pylori is probably the most common bacterial infection worldwide and the accepted cause of chronic active gastritis. It has a critical role in duodenal ulcer, where the prevalence of infection is 90-95%. There is a dramatic reduction in the rate of ulcer recurrence after successful eradication of the organism to about 4% per annum compared with up to 80% when the infection persists. What is true for duodenal ulcers is also true for patients with gastric ulcer who are infected with H. pylori. The risk of recurrent ulcer complications with bleeding is virtually abolished following s…

Helicobacter pylori (H. pylori) is the most common human gastrointestinal pathogen, infecting almost 50% of human population. By our present knowledge H. pylori is the cause of chronic active B type gastritis and the bacterium is accepted as a major pathogenetic factor of peptic ulcer disease. The prevalence of H. pylori positivity is about 95% in duodenal ulcer patients, in gastric ulcer patients 70-90% H. pylori positivity can be detected. The gastric mucosal barrier is weakened by the bacterial urease enzyme and vacoulating cytotoxin production. Special inflammatory and immunological proces…

Helicobacter pylori is a curvilinear motile gram-negative bacillus which is now recognized as the principal cause of type B gastritis, duodenal ulcer, gastric ulcer and MALT lymphoma. There is also a high degree of association between H. pylori gastritis and gastric carcinoma and non-Hodgkin's lymphoma of the stomach. H. pylori infection of the stomach is the most frequent chronic infection in the world. H. pylori is an extraordinary pathogen with virtually unrivaled versatility. The organism possesses the capacity to colonize and prosper in the hostile acidic gastric lumen, an environment in …

Helicobacter pylori is now recognized as the major causative agent of chronic superficial gastritis in humans. The virulence factors of H. pylori are still poorly understood. Vacuolating cytotoxin (VacA) is one of the factors that has been identified so far. VacA induces cytoplasmic vacuolation in eukaryotic cells. The vacA gene encodes a precursor protein of 140 kDa which consists of a 33-amino acid signal sequence, the 87 kDa cytotoxin and a 50 kDa C-terminal domain. The 50 kDa domain is involved in translocation of VacA across outer membrane. Sequence analysis of vacA gene derived from diff…
