BACKGROUND: One specific difficulty in total hip replacement for developmental dysplasia of the hip is a shallow acetabulum with a short anteroposterior diameter. In a prospective cohort study we investigated the short-term results of the Artek cup, designed with a shallow outline and a height-reduced metal inlay for a large size 38 mm metal head, in a group of female patients with mild dysplasia of the hip.

Although several studies of various treatment modalities have been reported during the past decade, osteonecrosis of the femoral head remains a difficult therapeutic problem. The work is devoted to operative treatment of children with repeated dislocations and ankyloses of the hip joint after operative treatment of congenital dislocation of the hip. The dura mater, demineralized allocups and polymer self-resolving film were used as interlayers. The author reports on hip-joint examinations in 16 000 infants, using sonography (age: 1 day up to 12-15 months). Particular attention is given to the classification of disturbances of maturation of the hip joint described. The use of angles and the possibility of making mistakes during measurement are demonstrated. The ultrasonographic traces are compared with X-rays to demonstrate the accuracy of ultrasonic investigation of infant hip joints. Determination of hip-joint maturity using the "sonometer" is described.

Long-term observations proved the necessity of careful control of the development of femur head epiphysis in case of dislocated hip joint. The occurrence of femur head necroses can be greatly reduced by a sparing functional treatment. Endogenously conditioned constructive disturbances, which can be found in 5 to 7 p.c. of all cases, have to be clearly distinguished from head necroses.

The influence of total hip replacement (THR) on pain and use of analgesics was evaluated in 511 patients with McKee-Farrar and Brunswik prostheses. The mean age of the patients was 63.8 years and the mean follow-up time 4.2 years. The average grade of pain decreased from 1.8 to 4.9 as evaluated according to Charnley. Thirty-eight children suffering of a transient synovitis of the hip joint have been reviewed with a follow up of 2 to 20 years (mean 7 years).
