Causes of Death in HIV Patients and the Evolution of an AIDS Hospice: 1988-2008.

This paper reports on the transformation that has occurred in the care of people living with HIV/AIDS in a Toronto Hospice. Casey House opened in the pre-HAART era to care exclusively for people with HIV/AIDS, an incurable disease. At the time, all patients were admitted for palliative care and all deaths were due to AIDS-defining conditions. AIDS-defining malignancies accounted for 22 percent of deaths, mainly, Kaposi sarcoma and lymphoma. In the post-HAART era, AIDS-defining malignancies dropped dramatically and non-AIDS-defining malignancies became a significant cause of death, including li

Adverse effects in hospice patients with chronic kidney disease receiving hydromorphone.

BACKGROUND: Pain is one the most common symptoms experienced by palliative care patients. The treatment of pain involves the use of strong opioids such as hydromorphone, morphine, methadone, fentanyl, oxycodone, oxymorphone, or levorphanol for moderate to severe pain. Hydromorphone is metabolized by the liver to hydromorphone-3-glucuronide (H3G), a compound that can potentially cause neuroexcitatory phenomena with accumulation. Pharmacokinetic studies have shown that H3G levels in patients with renal insufficiency are 4 times as high as those with normal renal function; however, reports have b

Symptomatic treatment of infections in patients with advanced cancer receiving hospice care.

Symptom control is one of the primary goals of hospice care. We prospectively followed patients with advanced cancer receiving outpatient hospice care to determine if the use of antimicrobials for a clinically suspected infection improved infection-related symptoms. During a 24-month period, 1,731 patients with a cancer diagnosis were admitted to a community-based outpatient hospice program. Over 89% of the patients had a Karnofsky performance of < or =60%. Six hundred twenty-three of 1,598 study patients were diagnosed with a total of 685 infections. Six hundred thirty-three of the infections

Evaluation of the Bereavement Risk Index (BRI): a community hospice care protocol.

AIM: to test the validity, reliability and feasibility of using a modified version of Parkes' (1993) Bereavement Risk Index (BRI) and bereavement support protocol in an Australian home hospice care setting. In addition, the study aimed to explore what types of family members are most likely to experience a more difficult grief reaction. DESIGN: a prospective, descriptive study was used. One hundred and fifty bereaved family members participated. Bereaved family members were classified as high, medium or low risk and received a structured bereavement support protocol based on their level of ri

The evolution of volunteerism and professional staff within hospice care in North Carolina.

Although the pursuit of Medicare certification has been one of the most controversial events in hospice history no study has examined its impact using data from a defined population of hospices before, during, and after federal legislation was enacted. This paper revisits the debate over the changing role of volunteers using such a longitudinal data source. Over time, as the patient population grew, aggregate levels of volunteers and professional staff increased. However, the ratios of professional staff and volunteers to patients reveal that regardless of certification status, hospices retain…

[The possibilities and problems of a home setting hospice service for a patient living alone].

We provided home care service for a patient in the terminal stage of lung cancer. This patient had some difficulties, such as living by himself, having little financial means, and unable to use welfare programs because of the age restrictions of certain institutions. We, however, arranged the home care the patient wanted by carefully making contact with the health and welfare institutions involved. Through this case, we reported the possibilities and problems of a home setting hospice service for a patient living alone.

A retrospective study of patient care needs on admission to an inpatient hospice facility.

BACKGROUND: The care and support of dying patients and their families are among the most important skills of a family physician. In this century, an increasing proportion of deaths have occurred in hospitals with resulting medicalization of the dying process. Hospice care has emerged to focus on the relief of suffering rather than the cure of illness. This descriptive study reports information about the diagnoses, care needs, and attending physicians of a cohort of patients admitted to a free-standing, inpatient hospice program. METHODS: We undertook a retrospective chart review of 335 patien

Caregiving Relationships, Advance Care Planning, and Hospice Use in Later Life.

BACKGROUND: Little is known about whether primary caregiver relationship is associated with hospice use or advance care planning (ACP) in later life. This study examined variation in hospice use and ACP across primary caregiver relationships. METHODS: Gateway Harmonized Health and Retirement Study End-of-Life data were restricted to Wave 12 and later. The sample included 3952 decedents aged 60 years or older. Caregiving category identified the primary caregiver as spouse/partner, child/grandchild, extended family, or non-kin informal, with formal care only and no identified caregiver as addit

Medicare Plan Switching and Hospice Care Among Decedents With Advanced Cancer.

IMPORTANCE: Hospice is central to end-of-life (EOL) care for patients with advanced cancers and is an excluded benefit under Medicare Advantage (MA), with coverage instead provided by traditional Medicare (TM). With growing MA penetration, more beneficiaries also switch between MA and TM for financial protection and physician access considerations, although less is known about how different Medicare programs and plan switching behaviors affect EOL care for patients with advanced cancers. OBJECTIVE: To evaluate hospice utilization and places of hospice care by Medicare plan switching patterns.

Parental experiences of perinatal loss, with a focus on hospice provision: A thematic analysis.

OBJECTIVE: Perinatal palliative care (PPC) offers holistic support to families of babies with life-limiting conditions, addressing emotional, psychological, and practical needs alongside ensuring dignity for the baby. While there is growing evidence to support its benefits, there remains inconsistent service provision, limited integration with maternity care, and regional disparities. This study explores parental experiences with perinatal hospice services to inform future care models. METHODS: The study was undertaken in the Northwest of England. Fourteen semi-structured interviews were unde

Optimizing hospice care: A scoping review of staffing strategies to enhance practice.

BACKGROUND: Strategic hospice staffing is essential to optimize patient care and strengthen workforce performance and retention, particularly as aging populations and rising rates of life-limiting illnesses increase the demand for end-of-life care. Despite growing recognition of staffing challenges in hospice care, there is limited synthesis of evidence on effective models and practices that support high-quality, sustainable care delivery. OBJECTIVE: This scoping review aimed to synthesize evidence on workforce characteristics and strategies that support sustainable, high-quality hospice care

Elderly at a Crossroads: Rehabilitation and Hospice Outcomes in Small Traumatic Brain Injuries.

Background: Traumatic brain injury (TBI) in older adults is a growing public health concern, yet outcome research has primarily focused on moderate-to-severe injuries or aggregated severity groups. Small-volume intracranial lesions, subdural hematomas (SDH ≤ 8mm), epidural hematomas (EDH ≤ 8mm), and cerebral contusions (≤ 2cm), are often classified as mild, but this may underestimate the functional burden in aging populations. Evidence is lacking on discharge outcomes in patients with isolated small-volume TBIs, particularly across age strata. Objective: To evaluate age-related differences in h
