Patient perceptions of ultrasound educational scans in the emergency department.

BACKGROUND: Emergency medicine residents may perform bedside ultrasound (BUS) scans that are carried out solely for educational purposes. This may lead to confusion on the part of patients, as the implications in the context of their medical care may be unclear. STUDY OBJECTIVES: We hypothesized that a scripted introduction would improve understanding of the objectives and limitations of educational BUS. METHODS: A perceptual survey was completed by a prospectively enrolled convenience sample of patients in two emergency departments. In phase 1, fifty patients completed the survey after thei

1995 Emerging Leaders in Healthcare. The new leaders: Gita Budd, Colene Daniel, Elizabeth Gallup, Scott Wordelman.

Fierce pressures for cost containment. Demands for quality improvements. The drive toward patient-centered care. The push for community involvement. Insistent voices of payers, patients, consumers, physicians. Accumulated tensions amid the chaos of change. Balancing all of these demands while inspiring and encouraging the professionals and other workers within the healthcare organization requires a high level of leadership ability. One that insists on the best from everyone involved in a healthcare system--from physicians to staff, nurses to social workers. And then strives for more. The four

A Family Systems Approach to Augmentative and Alternative Communication Service Delivery in the Inpatient Setting: Recommendations for Speech-Language Pathologists.

PURPOSE: Family-centered care is necessary to deliver high-quality health care services. It is especially critical that family members are included in augmentative and alternative communication (AAC) service provision in the inpatient setting. AAC can be overwhelming to families and requires accommodations for it to effectively integrate into daily communicative interactions. Currently, there is minimal guidance for family-centered care by speech-language pathologists (SLPs) working with children with limited functional speech in the inpatient pediatric setting. The purpose of this clinical fo

Making non-emergent abnormal results actionable: a short-term longitudinal qualitative study in two hospital-affiliated health examination centers in China.

BACKGROUND: Abnormal laboratory results in routine health examination reports can initiate prevention, confirmation, monitoring, or further consultation. Disclosure alone, however, does not make a result actionable: recipients must construct meaning, calibrate risk, and identify a feasible next step. This study examined non-emergent abnormal results as public health communication events in a regional hospital-affiliated health examination setting in China. METHODS: A short-term longitudinal qualitative study was conducted with 48 adults who had received at least one non-emergent abnormal labo

The Making of the Mental Health System in South Korea, 1960s-1980s: The Rise of Institutionalization and the Unfulfilled Promise of Community Mental Health.

In recent years, growing attention to the social integration of persons with psychosocial disabilities into their local communities has intensified calls to shift Korea's hospital-centered mental health system toward a community-based model. Historically, the Korean mental health system has often been framed through the narrative of a "delayed transition," which suggests that the country is belatedly undergoing the deinstitutionalization experienced in Western societies during the 1960s and 1970s. However, this linear account fails to explain a crucial divergence: during the very decades when

Obstetric Violence Through the Perspectives of Women and Midwives in Hospital Birth Settings: A Qualitative Study.

Background/Objectives: Obstetric violence is increasingly recognised as a violation of women's autonomy, dignity, and right to respectful maternity care, yet its interpretation may differ between women receiving care and midwives providing care within hospital systems. This study explored obstetric violence from the perspectives of women who perceived their childbirth as traumatic and midwives working in hospital birth settings. Methods: This qualitative study was conducted in one university hospital and two public hospitals in Türkiye. Data were collected through individual semi-structured in

Facilitating the transition from hospital to home after hip fracture surgery: a qualitative study from the HIP HELPER trial.

BACKGROUND: People post-hip fracture have reported experiences of fragmented care and poor discharge planning, therefore improvements in patient flow are required. This study reports the challenges people face during the discharge process and offers potential solutions for improving the transition from hospital to home from the perspectives of patients, carers, and health professionals. METHODS: This was a qualitative study embedded within a multi-centre, feasibility randomised controlled trial (HIP HELPER). We undertook semi-structured interviews with 10 patient-carer dyads (10 people with h

Molecular Properties of Virulence and Antibiotic Resistance of Pseudomonas aeruginosa Causing Clinically Critical Infections.

The increase in the number of hospital strains of hypervirulent and multidrug resistant (MDR) Pseudomonas aeruginosa is a major health problem that reduces medical treatment options and increases mortality. The molecular profiles of virulence and multidrug resistance of P. aeruginosa-associated hospital and community infections in Mexico have been poorly studied. In this study, we analyzed the different molecular profiles associated with the virulence genotypes related to multidrug resistance and the genotypes of multidrug efflux pumps (mex) in P. aeruginosa causing clinically critical infecti

Facility-based maternal deaths: Their prevalence, causes and underlying circumstances. A mixed method study from the national referral hospital of Somaliland.

OBJECTIVE: Somaliland has one of the highest rates of maternal deaths in the world. An estimated 732 women die for every 100,000 live births. This study aims to identify the prevalence of facility-based maternal deaths, the causes and their underlying circumstances by interviewing relatives and health care providers at the main referral hospital. METHOD: A hospital-based mixed method study. The prospective cross-sectional design of the WHO Maternal Near Miss tool was combined with narrative interviews with 28 relatives and 28 health care providers in direct contact with maternal deaths. A service oriented approach for guidelines-based clinical decision support using BPMN.

Evidence-based medical practice requires that clinical guidelines need to be documented in such a way that they represent a clinical workflow in its most accessible form. In order to optimize clinical processes to improve clinical outcomes, we propose a Service Oriented Architecture (SOA) based approach for implementing clinical guidelines that can be accessed from an Electronic Health Record (EHR) application with a Web Services enabled communication mechanism with the Enterprise Service Bus. We have used Business Process Modelling Notation (BPMN) for modelling and presenting the clinical pat

The myth of pandemic preparedness: persistent gaps in global readiness for Disease X.

Recent outbreaks of H5N1 avian influenza have exposed persistent weaknesses in global pandemic preparedness. Despite scientific advances, surveillance remains fragmented, hospital surge capacity is declining, and frontline workers continue to face inadequate protection. In low-income settings, delayed outbreak detection - exemplified by cholera in Sudan and mpox in central Africa - reflects ongoing gaps in diagnostic access and communication infrastructure. These challenges are compounded by geopolitical instability, fragile supply chains, and a lack of transparent, sustained investment in pre

Methicillin Resistant Staphylococcus aureus and public fomites: a review.

Staphylococcus genus is a Gram-positive coccus normally associated with skin and mucous membranes of warm-blooded animals. It is part of the commensal human microflora, or found in animals, or contaminating surfaces in the community and hospital settings. Staphylococcus aureus is the most pathogenic species belonging to this genus, as it possesses a collection of virulence factors that are expressed solely to evade the immune system. The increase in the misuse of antimicrobial agents predisposed S. aureus to develop antibiotic resistance, including the resistance to methicillin which led to th

Interventions for Health: Why and How Health Care Systems Provide Programs to Benefit Unhoused Patients.

Individuals experiencing homelessness have poorer health than housed individuals, while also utilizing more emergency department care and fewer preventive services. Several interventions - including permanent supportive housing, medical respite, and mobile medical clinics - are cost-effective means to improve health outcomes for homeless populations, yet few health systems have invested in such programs. This study aimed to determine the reasons some health systems initiated these interventions, and the early experience of those health systems that did. Quantitative analyses of health systems

Restricted patients and detention in the community: The human rights implications of supervised discharge under the Mental Health Bill 2025.

This article critically analyses provisions in the Mental Health Bill 2025 that, if passed, will amend the Mental Health Act (MHA) 1983 to create a power for tribunals and the Justice Secretary to discharge restricted patients from hospital subject to conditions that deprive them of their liberty in the community. These provisions pose a threat to the human rights of patients who straddle the divide between the mental health and criminal justice systems. Furthermore, the provisions and the cases that preceded them expose the limits of policies of de-institutionalisation and official ambitions
