Heart failure is considered a garbage code when assigned as the underlying cause of death. Reassigning garbage codes to plausible causes reduces bias and increases comparability of mortality data. Two redistribution methods were applied to Brazilian data, from 2008 to 2012, for decedents aged 55 years and older. Prescribing data downloaded from the MUMPS-based hospital information system of the Department of Veterans Affairs has been used to target specific interventions aimed at changing physician prescribing patterns. Focused interventions aimed at specific groups of physicians, patients, clinics, and drugs have proven effective in reducing pharmaceutical costs by rationalizing prescribing patterns. The results indicate the potential usefulness of a microcomputer with a commercial database program in manipulating hospital information system data.

BACKGROUND: Nursing home residents are prone to acute illness due to their high age, underlying illnesses and immobility. We examined the incidence of acute hospital admissions among nursing home residents versus the age-matched community dwelling population in a geographically defined area during a two years period.
