BACKGROUND: Osteonecrosis of the humeral head is a frequent complication in adults with sickle cell disease. However, little is known about the rate of, and the factors influencing, progression of symptomatic shoulder osteonecrosis in patients with this disease.
METHODS: Eighty-two adult patients with sickle cell disease and symptomatic osteonecrosis of the humeral head (104 shoulders) were identified with magnetic resonance imaging (MRI) between 1985 and 1993. Nineteen of the eighty-two patients were homozygous for hemoglobin S (S/S genotype), thirty-seven had hemoglobin S/hemoglobin C (S/C)

This anatomical and biomechanical study focuses on the specific influence of the long head of biceps brachii muscle in controlling axial humeral rotation. The tendon of the long head of the biceps brachii (biceps tendon) is shown to either facilitate or restrict axial humeral rotation. Its effect on axial humeral rotation is strongly related to firstly the amount of biceps tendon load, secondly glenohumeral scapular plane elevation, and thirdly the rotatory position of the humerus. At 0° glenohumeral elevation, biceps tendon load caused an increase of internal humeral rotation, from 0° at 2.25

OBJECTIVE: To establish whether osteochondral autograft transfer (OAT) procedures for osteochondritis dissecans (OCD) of the canine caudocentral humeral head would restore articular contour, resurface osteochondral defects, and resolve lameness in dogs with OCD.
STUDY DESIGN: Case series.
ANIMALS: Dogs (n=14) with caudocentral humeral head OCD (16 shoulders).
METHODS: After arthroscopic assessment, the humeral head was exposed by arthrotomy. The OCD lesion was debrided and OATS(™) instrumentation used for resurfacing the defect with osteochondral core grafts collected from the stifle. Recip

Posterior fracture-dislocation of the humeral head is an uncommon injury, usually associated with seizures, electrocution injury and high-impact trauma. Prompt diagnosis and treatment are important to prevent avascular necrosis of the humeral head. Various methods to treat such injury have been described. We report a 34-year-old man who had a four-part humeral head fracture with concomitant posterior dislocation that was treated with open reduction and internal fixation with non-absorbable polyester sutures. The diagnosis and operative techniques are described and factors influencing our choic

Posterior dislocation of the shoulder is a rare and commonly missed injury. While prompt diagnosis and treatment is important to prevent untoward sequelae, it is often diagnosed and treated too late. The first reported case of a posterior fracture-dislocation of the humeral head following a domestic electrocution accident in Singapore is described in a 52-year-old man. The injury was missed by several doctors before a humeral head replacement was done. The diagnostic pitfalls and management of this injury and ways to avoid a missed or delayed diagnosis are discussed.

One hundred fifty-one patients with 200 shoulders affected with osteonecrosis of the humeral head were evaluated for associated factors, the need for prosthetic replacement surgery, the state of the unoperated shoulder, and the existence of prognostic factors. Associated factors included corticosteroid use in 112 shoulders, trauma in 37, Gaucher's disease in 3, sickle cell disease in 3, and radiation necrosis in 1. No cause was evident in 44 shoulders. Ninety-seven shoulders had replacement surgery. The need for replacement surgery was found to be related to extent and stage of humeral head in

Simultaneous bilateral fractures of either the femoral necks or the humeral heads are rare injuries. Four simultaneous fractures of both femoral necks and humeral heads have not been previously described. The case reported occurred after an epileptic convulsion in a woman with multiple metastases from breast cancer.

Fractures of the proximal end of the humerus are treated by early functional movement. Dislocated fractures of the humeral head should be treated by means of osteosynthesis. In younger patients osteosynthesis with a T- or L-shaped plate is the method of choice; in older patients, open reduction and K-wire fixations can be used. Indications for screw osteosynthesis are rare, and alloarthroplasty is a procedure reserved for exceptions. The functional results are demonstrated by 24 cases with dislocated fractures who were operated on, thus showing the principles of operative treatment.

Dislocated fractures of the humeral head are problematic injuries. The result of an operative therapy of 30 adult patients are given. Intra- and postoperative complications, advantages and disadvantages are pointed out. Traumatic and posttraumatic damages of the cartilage, partial necrosis of humeral head and the combination with lesions of the soft tissues and nerves are the limits to a good functional result.

BACKGROUND: The underlying shoulder pathology in radiographic superior escape of the humeral head and association between acromiohumeral interval (AHI) on radiographs and magnetic resonance imaging (MRI) are poorly understood.
METHODS: A retrospective review of shoulder radiographs and MRI scans was undertaken. AHI was measured using both modalities. Deltoid bulk, long head of biceps brachii subluxation/tear, and rotator cuff disease (tendon involvement, severity, fatty infiltration) were noted using MRI.
RESULTS: A total of 167 patients were included. MRI measurements of AHI were significan

INTRODUCTION: Rheumatoid Arthritis (RA) is a chronic inflammatory disease depicted by peripheral bone erosive damage leading to joint destruction, deformity and functional impairment. Shoulder involvement is less frequent than hands, wrists and feet, and relevant joint damage may be underdiagnosed if a lower threshold for careful analysis of this joint is not settled, especially in uncontrolled disease.
CASE REPORT: A 70-year-old male with a difficult-to-manage RA since 2010, presenting severe shoulder arthritis with MRI showing a striking giant geode in the left humeral head.
CONCLUSION: An

BACKGROUND: Much of the literature on osteonecrosis has traditionally focused on the hip, which remains the most common site for this disease. The shoulder and the knee are the second most frequently affected sites (both approximately 10% of the incidence). There are a variety of techniques for managing this disease, and it is important to optimize this for our patients. This review aimed to compare the results of core decompression (CD) vs. nonoperative modalities for the treatment of osteonecrosis of the humeral head, including (1) success rate defined as lack of progression to further proce

BACKGROUND: Osteonecrosis of the humeral head is an uncommon subchondral bone disease with many etiologies, and there is currently no definite evidence to support an optimal surgical treatment plan. We report a case of surgical treatment of left humeral head necrosis. To the best of our knowledge, this is the youngest patient with non-drug-induced humeral head necrosis and the largest collapsed area.
CASE PRESENTATION: The case involved a 16-year-old male who injured his left shoulder 1 year ago. The patient was admitted to the hospital because of shoulder pain after activity in the year foll

BACKGROUND: The underlying cause of glenohumeral arthritis is poorly understood. Glenohumeral arthrosis patterns have been classified and described, and differential contact stresses within the joint have been implicated as a cause of joint degeneration, but the intrinsic cause of degeneration patterns in the glenohumeral joint (GHJ) remains largely unknown.
PURPOSE/HYPOTHESIS: The purpose of this study was to assess morphological and mechanical differences in articular cartilage (AC) and subchondral bone (SCB) of the glenoid and humeral head in matched cadaveric specimens. We hypothesized th

The bilateral shoulder pain of an 81-year-old Japanese woman due to falls persisted despite celecoxib treatment, and plain X-rays later showed bilateral collapsed humeral heads. After ruling out osteoarthritis, infectious arthritis, crystal-induced arthritis, neuropathic arthropathy, and osteonecrosis, we diagnosed bilateral shoulder joint rapidly destructive arthrosis (RDA). Lumbar bone mineral density showed very low T-score (-4.1). Primary osteoporosis was observed. Histology of biopsied humeral head indicated the features of fracture healing process: callus formation and osteoclasts withou

BACKGROUND AND PURPOSE: Long term outcome of the treatment of displaced complex fractures of humeral head is rare in the literature especially in greater cohorts. Main purpose of our study was the assessment of long term results of intramedullary nailing of 3-4 part fractures.
PATIENTS AND METHODS: 137 patients with 137 three or four-part fractures of the humeral head treated by intramedullary nailing were reassessed after an average follow-up time of 57 (27-93) months. The whole cohort consisted of 85 three-part fractures, 38 four-part fractures and 14 fracture-dislocations. Radiographic and

A 50-year-old male traffic accident victim sustained high-energy trauma leading to intra-thoracic fracture-dislocation of the humeral head. Dislocation of the humeral head into the thoracic cavity is a very rare injury. Only twelve cases have been reported in the literature. Treatment modalities vary and no guidelines have been proposed. The mechanism of the injury and the treatment options are discussed together with a review of the literature.

Superior excursion of the humeral head on the face of the glenoid was investigated to determine whether a relationship exists between the degree of humeral head excursion and the identity of the injured tendon in a shoulder with a torn rotator cuff. Twenty-seven patients with unilateral full-thickness rotator cuff tears diagnosed by either positive arthrogram or magnetic resonance imaging were included in this study. Three groups were formed based on intraoperative findings: 10 patients had isolated supraspinatus tendon tears (group 1), 11 patients had supraspinatus and infraspinatus tears (gr
