In typical cases, normal pressure hydrocephalus (NPH) manifests itself with the triad of gait disturbance, which begins first, followed by mental deterioration and urinary incontinence associated with ventriculomegaly (on CT or MRI) and normal cerebrospinal fluid (CSF) pressure. These cases present minor diagnostic difficulties and are the most likely to improve after shunting. Problems arise when NPH shows atypical or incomplete clinical manifestations (25-50% of cases) or is mimicked by other diseases. In this scenario, other complementary tests have to be used, preferentially those that can

Idiopathic normal pressure hydrocephalus (iNPH) is a syndrome of gait disturbance, dementia and urinary incontinence without any preceding disorders in elderly patients. Rapid increase of elderly population in Japan prompted us to make guidelines for diagnosis and management of iNPH. The Japanese guidelines were published in 2004 and their characteristics were summarized as follows; 1) Simple and treatment-oriented, 2) Three diagnostic levels: possible, probable and (definite); Shunt surgery is indicated for probable iNPH only, 3) Clinical significance of gait disturbance as the most frequent

Normal pressure hydrocephalus (NPH) is a clinical syndrome associated with dementia, gait disturbance and urinary incontinence. Gait disturbance is usually the initial sign and most important symptom, but its objective evaluation has not been established. We analyzed the gait of an idiopathic NPH before and after ventricular shunting with the gait analysis system. Before shunting, the stride was short and irregular, and the truncal movement was unsteady. Three-dimensional patterns of angular relationships between 3 joints, namely the ankle, knee and hip were small and irregular. The vector pro

A 74-year-old woman presented with gait impairment, urinary incontinence, and dementia. She showed lymphocytic CSF pleocytosis and pronounced intrathecal Borrelia burgdorferi antibody production, indicating active Lyme neuroborreliosis. The syndrome of normal-pressure hydrocephalus (NPH) fully remitted after ceftriaxone treatment. Lyme neuroborreliosis may cause NPH by interfering with subarachnoid CSF flow.

Although extrapyramidal features in normal pressure hydrocephalus (NPH) are not uncommon, presentations with Parkinson's syndrome as the predominant feature are rare and may give rise to diagnostic difficulties. Failure of patients with parkinsonism to respond to therapy, should alert one to the possibility of NPH.

Nineteen patients with idiopathic normal pressure hydrocephalus (NPH) were treated with intraventricular shunts. Ten of the 14 who improved and 14 of the total group had systemic hypertension. The prevalence of hypertension in both the improved and whole NPH groups was significantly greater than in a control group with dementia (N = 122) and the published prevalence of hypertension in the US population for this age group. Four possible mechanisms for this association are discussed.

A xenon-133 method was used to measure cerebral blood flow (CBF) before and after cerebrospinal fluid (CSF) removal in patients with normal pressure hydrocephalus (NPH). Preliminary results suggested that shunting should be performed on patients whose CBF increased after CSF removal. There was a significant increase in CBF in patients with NPH, which was confirmed by the favorable outcome of 88% of patients shunted. The majority of patients with senile and presenile dementia showed a decrease or no change in CBF after CSF removal. It is suggested that although changes in CBF and clinical sympt

Eight cases with well-documented normal-pressure hydrocephalus were studied prospectively for 6 months by history, neurological examinations, Mini-Mental Status tests, xenon-contrast computed tomography measurements of local cerebral blood flow, and cerebral xenon solubility expressed as partition coefficients. Local cerebral blood flow and local partition coefficients were reduced throughout frontal and temporal lobes, basal ganglia, and thalamus. Cerebrospinal fluid shunting procedures were carried out in seven cases. As a result, local cerebral blood flow and local partition coefficients in

Shunt-responsive normal-pressure hydrocephalus developed in a 67-year-old man and in his 74-year-old sister. To our knowledge, a familial association in this disorder has not been reported previously.

In three patients who fulfilled the clinical criteria of progressive supranuclear palsy, radiologic investigations suggested normal-pressure hydrocephalus. Shunt procedures in all three resulted in temporary improvement of gait, mentation, and bladder control, but gaze paralysis and extrapyramidal findings did not change.

This report compares the clinical and post mortem findings in a case of normal pressure hydrocephalus (NPH) syndrome occurring in association with hypertensive cerebrovascular disease, with the only two such previously reported autopsied cases. The presence of hypertension and focal neurological findings in a patient with otherwise classic clinical and radiologic features of NPH syndrome should alert one to the possibility of hypertensive cerebrovascular disease as the cause of the syndrome. Transient improvement occurred following shunting in two, and none in the third case. All died within t

Normal-pressure hydrocephalus and the associated triad of dementia, apraxic gait and urinary incontinence may be casued by various, sometimes unsuspected, lesions, usually those that block the flow of cerebrospinal fluid (CSF) around the tentorium. A 58-year-old woman with insidious onset of behaviour and gait problems had occult, normal-pressure hydrocephalus and a tentorial meningioma, resection of which produced complete recovery. This case demonstrates that a distinction must be made between the syndrome of normal-pressure hydrocephalus resulting from disturbance in CSF dynamics and the pa
