A radioimmunoassay for porcine parathyroid hormone has been developed and applied to measure immunoreactive parathyroid hormone (PTH) in plasma of pigs with hereditary vitamin D dependency rickets (VDDR) (pseudovitamin D deficiency rickets). Levels of 25-hydroxycholecalciferol (25-(OH)-D3) in plasma were measured by a protein binding assay. Both plasma concentrations of PTH and 25-(OH)-D3 showed an approximately 4-fold increase compared to normal pigs. PTH levels increased with duration of the disease. Three children with azotaemic renal osteodystrophy were treated with 1,25-dihydroxycholecalciferol (1,25(OH)2D3). All showed clinical, biochemical, and radiological improvement within 6 months of starting treatment. There were no complications. 200 Asian 300 African, Chinese, or Scottish children were examined for clinical, biochemical, and radiological evidence of vitamin-D deficiency. Among the Asians there were 10 with florid rickets and 15 with subclinical rickets. There is increasing evidence that D-hormones (active vitamin D metabolites) are effective agents in preventing posttransplantation bone loss of various organs. This effect may relate to several mechanisms including reversal of secondary hyperparathyroidism from cyclosporine, reversal of corticosteroid effects on bone, or immunomodulatory effects with corticosteroid sparing. Further clinical trials are necessary to establish their comparative efficacy to other agents, but D-hormones should be considered as prophylactic therapy in patients undergoing organ transplantation.

Synthesis of the biologically active metabolite(s) from 1 alpha-hydroxyvitamin D3 (1 alpha(OH)D3) was examined in various types of human leukemia cell lines. Untreated monocytoid leukemia cells (U937 and HEL/S) metabolized 1 alpha (OH)D3 to the active metabolite(s), possibly 1 alpha, 24- and/or 1 alpha, 25-dihydroxyvitamin D3, and these cells were efficiently induced to differentiate by treatment with 1 alpha (OH)D3. We have evaluated the serum levels of 25-hydroxyvitamin D (25-OHD) and 1,25-dihydroxyvitamin D (1.25-OHD) in 33 patients with primary hyperparathyroidism and normal renal function, relating them with the clinical expression of the disease and other biochemical parameters. Serum 25-hydroxyvitamin D3 levels were determined in patients with gastric ulcer, duodenal ulcer, and subtotal gastrectomy (Billroth II); 35 healthy persons served as controls. 25-hydroxyvitamin D3 levels were correlated with the signs of osteopenia determined radiologically. 25-hydroxyvitamin D3 serum level was not different from normal in subjects with gastric or duodenal ulcer. Those who underwent gastric surgery had significantly lower values. Within this group those with osteopenia had lower values than those without.
