A case is reported of a huge convexity meningioma associated with galactorrhea and an elevated serum prolactin level of 41 ng/mL. Total removal of the tumor resulted in a return of the prolactin level to normal (9.7 ng/mL). Because of a severe mass effect, the distant tumor might have encroached upon the hypothalamic-pituitary axis and interrupted PRL inhibitory factor regulation.

Eight young women developed clinical manifestations of hyperprolactinemia after the application of bilateral methylpolixolosane mammary prostheses. Elevated serum levels of prolactin were present in all cases. Treatment with bromocriptine or methergoline corrected the excessive prolactin production and associated symptoms. The pathophysiology of this syndrome is unexplained as yet.

The human prolactin molecule has been isolated and its structure characterized. This anterior pituitary hormone plays an important function in the induction and maintenance of lactation in the post-partum nursing mother. Prolactin (PRL) and progesterone were systematically measured in all women presenting with amenorrhea, oligomenorrhea or galactorrhea at the Infertility Clinic of the Department of Obstetrics and Gynecology, State University of Campinas, Brazil, during a period of 34 months. Because animal experiments have suggested that PRL might regulate the serum somatomedin (SM) concentration, the effect of sustained hyperprolactinemia on the serum SM level was studied in patients with proven pituitary microadenomas. We report three patients with craniopharyngiomas who had galactorrhea, oligo/amenorrhea, and abnormal sellar tomograms, clinically suggesting the presence of a prolactinoma. One patient had an intrasellar craniopharyngioma (Rathke's cleft cyst) diagosed during surgical exploration of the pituitary fossa for removal of a suspected prolactinoma, and two had suprasellar caraniopharyngiomas whose presence was suspected on the basis of computed tomography. A 48 year old premenopausal woman presented with galactorrhea and amenorrhea associated with chest wall burns. Basal serum prolactin levels were raised, and were further elevated by the administration of L-dopa, chlorpromazine and TRH. Intercostal nerve block and bromocryptine treatment reduced prolactin levels to normal, but did not noticably reduce milk secretion.
