[Malignant hyperthermia in children].

INTRODUCTION: Malignant hyperthermia is a rare disease which is mainly an inherited autosomal dominant trait. It is characteristic for muscle rigidity, metabolism or respiratory acidosis, high values of serum creatine kinase. Then appears myoglobinuria which can lead to tubular necrosis and acute renal failure. CASE OUTLINE: The male child, ten years old, hospitalized because of the high temperature, exhaustion and cough. On the second day of hospitalization, he has hyperthermia (39.8 degrees C), dyspnoea, tachypnoea, was somnolent, occasionally raving, exhausted with the pains in the muscles…

[Malignant hyperthermia. The ugly].

The term "malignant hyperthermia" (MH), regarded as the typical anaesthetic disease, refers to a clinical syndrome of varying intensity (from abortive courses to fulminant crises) and develops only under exposure of certain triggering substances or mechanisms. MH is caused by a defect in the ryanodine receptor subtype 1, which can often be proved genetically. Furthermore, it may also be generated by other mechanisms which disturb the membranous integrity of skeletal muscle cells (e.g. some myotonias, muscular dystrophies, malformation syndromes). Hyperthermia is only one of a number of life-th…

Malignant hyperthermia and the otolaryngologist.

Malignant hyperthermia is a rare disorder that can occur in patients who are sensitive to certain agents used in general anesthesia. The treatment of malignant hyperthermia has not changed over the years, but prevention strategies have evolved. These strategies include an increased emphasis on how patients are managed prior to a surgical procedure, on the selection of the particular anesthetic agent, and on postoperative vigilance. Susceptible patients who undergo simple excisions or a low degree of surgical stress can receive treatment safely in the office or ambulatory surgery center and be …

Malignant hyperthermia: a review.

Malignant hyperthermia (MH) was first identified as an anesthetic-related patient syndrome with familial tendencies in 1960. It is defined as a chain reaction of abnormalities triggered in susceptible individuals by commonly used anesthetic agents, and is classified as a hypermetabolic disorder of skeletal muscle. It is believed that susceptible patients possess a genetic predisposition for the development of the disease. In an acute episode of MH, the anesthetic triggering agent appears to interfere with the calcium ion reentry into the sarcoplasmic reticulum after muscle contraction. This pr…

Endothelial cells and hyperthermia.

Most radiation oncologists are aware of the effects of clinical hyperthermia on neoplastic cells. Its effects on blood vessels, however, are not as well recognized. Yet, since the 1960s a number of investigators have described and categorized the effects of hyperthermia on microvessels (in vivo), and on cultured endothelial cells (EC) (in vitro). Both EC and microvessels can be lethally damaged by the hyperthermia doses used as antineoplastic therapy. In vitro data indicate that capillary EC are moderately sensitive to hyperthermia. Proliferating EC are more thermosensitive suggesting that mic…

The genetics of malignant hyperthermia.

Malignant hyperthermia susceptibility remains the commonest cause of death owing to general anaesthesia. This is despite the availability of presymptomatic testing, admittedly by a highly invasive method, and a recognised treatment for implementation immediately a patient shows signs of developing a crisis. Recently the finding of linkage to markers from chromosome 19q13.1-13.2 and the identification of mutations in a candidate gene held out hope of genetic diagnosis being available. However, it is likely that only about 50% of families have a mutation of the skeletal muscle calcium release ch…

Malignant hyperthermia: a review.

Malignant hyperthermia (MH) is a rare genetic myopathy that was first described as a fatal complication of general anesthesia in 1960. It is estimated to affect approximately 1 in 15,000 pediatric patients and 1 in 40,000 adult middle-aged patients. The mode of transmission is genetic: the severest form is autosomal dominant, and the less severe, autosomal recessive. Thus, both men and women can have MH, although there is a slightly higher incidence in the male pediatric population. Malignant hyperthermia is usually triggered by halogenated anesthetic agents with or without depolarizing muscle…

[Hyperthermia in the treatment of cancers].

Encouraging clinical results have renewed the interest in hyperthermia for the treatment of cancer for the past few years. Several experimental studies allowed better knowing the mechanisms of action of hyperthermia at the cellular and tissular level: direct cytotoxic effect of the temperature increase, potentialization through hyperthermia of the effects of ionizing radiation and of anticancerous chemotherapeutic drugs. There are many techniques for tissue heating: local hyperthermia with external applicators (microwaves, radiofrequencies, ultrasound) or with implanted interstitial heaters; r…

A case of malignant hyperthermia?

An acute episode of a malignant hyperthermia-like syndrome is described which occurred after suxamethonium and isoflurane anaesthesia in a 41-year-old healthy male patient undergoing a minor elective hand operation. Dantrolene therapy rapidly reversed the life-threatening signs. Laboratory results appeared to confirm the suspicion of malignant hyperthermia. However, the in vitro contracture test, which was carried out according to the standards of the European Malignant Hyperthermia Group, was equivocal.
