Selective IgA deficiency (SIgAD) is the most common type of primary immunoglobulin deficiency. Most individuals with SIgAD are asymptomatic. However, some patients are associated with allergic and autoimmune disease. SIgAD is included in the list of differential diagnoses of eosinophilia. We experienced a patient who initially presented with abdominal pain and eosinophilia. A >1-year follow-up revealed SIgAD, and we had difficulty differentiating it from Churg-Strauss syndrome (CSS) or hypereosinophilic syndrome (HES). A 66-year-old Japanese male presented with a history of recurrent abdominal

Selective IgA deficiency (IGAD) is the most common primary immunodeficiency, yet its pathogenesis is elusive. The IG (heavy) H chain human 3' Regulatory Region harbors three enhancers and has an important role in Ig synthesis. HS1.2 is the only polymorphic enhancer of the 3' RRs. We therefore evaluated HS1.2 allelic frequencies in 88 IGAD patients and 101 controls. Our data show that IGAD patients have a highly significant increase of homozygousity of the allele *1 (39% in the IGAD patients and 15% in controls), with an increase of 2.6-fold. Allele *4 has a similar trend of allele *2, both sho

BACKGROUND AND OBJECTIVE: In order to build panels of IgA deficient blood donors, an assay is described that is sensitive, inexpensive and easily adaptable to the automated sample processors and turnaround times of blood banks.
DESIGN AND METHODS: We developed a two-step enzyme-linked immunosorbent assay (ELISA) carried out in microwell plates coated with rabbit anti-human IgA antibody. Captured IgA was revealed with the same antibody conjugated to horseradish-peroxidase. The assay was adapted to the automatic pipetting system and ELISA processors used in routine blood donor screening.

OBJECTIVES: While physicians are often confronted with immunoglobulin A (IgA) deficiency in children with recurrent infections, the clinical relevance of this finding is unclear. Large-scale studies examining the significance of IgA deficiency in children are hampered by differences in techniques for measuring IgA and the physiological increase of IgA with age. Both result in a variety of reference values used for diagnosing IgA deficiency. We propose a new laboratory-independent method to accurately compare IgA measurements in children of varying ages.
METHODS: We present a method to standar

A female patient, currently aged 40, has attended our Lupus Clinic for 8 years with a clinical picture of urticarial vasculitis, joint pains, severe myalgias and abdominal pain which has occurred in bouts at irregular intervals. She has been found to have consistently reduced C4 levels, C1-esterase inhibitor deficiency and a persistent monoclonal band in the gamma region on electrophoresis. Her serum IgA level has been continuously low. Her symptoms have failed to respond to antihistamines and a variety of other measures but have been partially alleviated by immunosuppressive therapy ('pulsed'

IgA nephropathy (IgAN) has long been viewed as a disease driven almost exclusively by IgA1, in particular galactose-deficient IgA1. However, emerging evidence challenges this IgA1-centric paradigm. In this issue of Kidney International, Li et al. demonstrate that IgA2 is consistently deposited in IgAN glomeruli and actively promotes complement activation, macrophage infiltration, and tubulointerstitial injury. These findings could redefine IgA2 from a passive bystander to a pathogenic amplifier, linking mucosal immunity, complement biology, and progression of kidney disease in IgAN.

BACKGROUND: The most prevalent primary immunodeficiency is selective immunoglobulin A deficiency (SIgAD), which is characterized by serum IgA levels equal to or less than 7 mg/dL with normal levels of IgG and IgM in a patient who is at least 4 years old. Most patients are asymptomatic; however, SIgAD increases the risk of respiratory and gastrointestinal infections, autoimmune disorders, transfusion reactions, and atopy. CASE PRESENTATION: Our 4-year-old female patient had a history of recurrent severe infections requiring multiple hospital admissions, raising our suspicion of an immunodeficie

BACKGROUND: This study investigated the significance of galactose-deficient immunoglobulin A1 staining in kidney diseases with IgA deposition.
METHODS: A total of 120 patients with IgA-dominant deposition in kidney tissues were enrolled and divided into four groups: primary IgA nephropathy (PIgAN), secondary IgA nephropathy (SIgAN), monotypic IgA nephropathy (MIgAN), and IgA variant monoclonal gammopathy of renal significance (IgA-MGRS). KM55 (the antibody of galactose-deficient immunoglobulin A1), IgA subtypes, and complement pathway factors (properdin, C4d, and C1q) were detected through im

We present the case of a 35-year-old female patient, pregnant in her third trimester, with no ophthalmologic history of interest and a medical history of IgA deficiency syndrome with bronchiectasis as the only symptomatology, who came to another center with clinical symptoms of ocular discomfort. She was initially diagnosed with anterior uveitis and treated with topical and periocular corticosteroids. Edema and palpebral erythema appeared a few days later and she was diagnosed with idiopathic orbital inflammation and was treated with intravenous (I.V.) corticosteroids, which led to the appeara

Understanding the migration of lymphocytes to nonintestinal mucosal sites is fundamental to developing mucosal vaccination strategies. Studies have shown that nasal and oral immunization with cholera toxin (CT) stimulates, in addition to α4β7+ , the induction of αE (CD103)β7+ B cells. To determine the extent to which αE-associated β7 contributes to antigen (Ag)-specific immunoglobulin (Ig)A responses in the upper respiratory tract, nasal CT vaccination was performed in wild-type (wt) and β7-/- mice. At 16 days postprimary immunization, upper respiratory tract IgA responses were greater in β7-/

We report a case of ABO incompatible living kidney transplantation in a 45-year-old man with selective IgA deficiency. He has been on hemodialysis for 1 year due to chronic renal failure. Although the serum anti-IgA antibody was negative, he has experienced anaphylactic reaction to red blood cell product in the past. Before the transplantation, we performed double filtration plasmapheresis for two times, but didn't performed plasma exchange, considering the possibility of producing anti-IgA antibody and anaphylactic reaction. He underwent kidney transplantation from his mother without anaphyla

Immunoglobulin A (IgA) nephropathy (IgAN), the leading cause of primary glomerulonephritis, is characterized by IgA1-containing immunodeposits in the glomeruli. IgAN is a chronic disease, with up to 40% of patients progressing to end-stage renal disease, with no disease-specific treatment. Multiple studies of the origin of the glomerular immunodeposits have linked elevated circulating levels of aberrantly glycosylated IgA1 (galactose-deficient in some O-glycans; Gd-IgA1) with formation of nephritogenic Gd-IgA1-containing immune complexes. Gd-IgA1 is recognized as an autoantigen in susceptible

17 years ago, selective IgA deficiency was diagnosed in a 45-year-old woman suffering from dermatitis ulcerosa. In 1984, this patient developed oral cancer with metastases in the regional lymph nodes. Her long-term history of nicotine and alcohol abuse in combination with IgA deficiency seemed to be a risk factor for oral malignancy.

BACKGROUND Selective immunoglobulin A deficiency (SIgAD) is the most common primary immunodeficiency. However, most people with SIgAD are asymptomatic, and it is typically diagnosed during screening for blood donations or unexpected transfusion reactions. Screening programs for IgA deficiency have not yet been established, and the exact etiology of IgA deficiency remains unclear. CASE REPORT We report a case of SIgAD in a man who was diagnosed after a febrile non-hemolytic transfusion reaction (FNHTR) during erythrocyte transfusion for severe anemia. A 70-year-old man with a history of surgery

Immunoglobulins, C3 and C4 levels and circulating immune complexes have been measured on admission sera from 34 children admitted to a Naples hospital during the winter of 1978/79 with severe, usually fatal, acute respiratory infections and in age-matched controls from Rome. The Naples sera were characterised by low IgA, C3, and C4 levels, increased IgM and IgG concentrations, and high titres of circulating immune complexes. These immunological abnormalities could explain the high concentration of fatal cases.
