The findings of 69 autopsies of children of the first year of life who died from pneumonia were examined. The children were found to have a significant lowering of IgA and IgG and an increase of IgM which correlated with the immunomorphological appearance of the organs of immunogenesis. The concentration of components C3 and C4 of complement was reduced. The reduction appeared most remarkable in +viral and bacterial pneumonias. Hyper IgM with low IgG and IgA is a rare humoral immunodeficiency. We presently report 12 new observations which have been clinically and immunologically studied. On one occasion the syndrome was found to be associated with congenital rubella. Since 10/12 children were male, X-linked inheritance is suggested which has been confirmed in 2 cases. A young woman developed Hodgkin's disease (nodular sclerosis) in pregnancy and gave birth to a boy who developed common variable immunodeficiency. Initially there was normal IgG with low IgA and IgM, and antibody deficiency. IgG levels fell progressively over 4 years. Cellular immunity was normal. We suggest that this is a further family with immune deficiency presenting with common variable immunodeficiency and lymphoid malignancy.

Serum immunoglobulins and IgG subclasses were measured in 30 children with recurrent infections. Seven patients had low IgG2 concentrations (less than 3SD below the geometric mean for age). Four of these seven patients had normal concentrations of IgG, IgA and IgM, and thus would have been considered immunologically normal by routine criteria.
