Cryptococcal meningitis in immunocompetent patients.

Five cases of cryptococcal meningitis (CM) in immunocompetent patients who were initially diagnosed as either tuberculous meningitis or partially treated pyogenic meningitis are presented. There were 3 male and 2 female patients in the age range of 21 to 42 years. These patients presented as subacute meningitis. Additional features were visual impairment (4 cases), papilloedema (3 cases), sixth cranial nerve palsy (3 cases) and ataxic hemiparesis (one case). There was a mediastinal mass of fungal aetiology in one patient. While routine analysis of CSF showed a variable picture, it is the India

Lymphomatous meningitis in immunocompetent patients.

A prospective study of combined modality therapy of non-AIDS related lymphomatous meningitis was carried out. Lymphomatous meningitis is diagnosed increasingly as anti-lymphoma therapies become more effective and result in prolonged patient survival. Twenty-two patients (range 38-69 years; median 60) with lymphomatous meningitis due to metastatic non-AIDS related non-Hodgkins lymphoma were treated. Neurologic presentation included: headache (n=13); cranial neuropathies (n=9); ataxia (n=5); cauda equina syndrome (n=3); myelopathy (n=1); and meningismus (n=1). All patients underwent radiographic

Bacillary angiomatosis in an immunocompetent child.

Bacillary angiomatosis (BA) is a pathological process characterized by prominent vascular proliferation secondary to organisms of the genus Rochalimaea. BA has been most commonly associated with HIV-1+ patients, but has also been reported rarely in other immune-suppressed patients and in a small group of patients with no demonstrated immune suppression. Even in immune-suppressed children, BA is extremely rare. We report a 5-year-old girl with no apparent immune suppression and no risk factors for HIV-1+ disease, who presented with a skin lesion that histopathologically was diagnostic of BA.

Treatment of varicella in the immunocompetent adult.

Varicella in the immunocompetent adult is an infrequent but potentially serious infection. Previous studies in immunocompetent hosts and normal adults have demonstrated the value of intravenous acyclovir in the treatment of varicella-zoster virus infections. Oral acyclovir has also shown efficacy in both normal adults with zoster (shingles) and immunocompetent children with varicella. A recently completed double-blind placebo-controlled study of oral acyclovir in immunocompetent adults with uncomplicated varicella also demonstrated efficacy. Therapy within the first day reduced the time to 100

Cryptosporidia in immunocompetent patients with gastroenteritis.

In a prospective study Cryptosporidium spp. were detected in nine immunocompetent patients (0.6%). The frequency was remarkably higher in children in the 0 to 14 year age group (1.6%), surpassed only by Salmonella spp. and Campylobacter spp. The predominant clinical features were watery diarrhea, abdominal pain, and vomiting. Comparison of the epidemiological data of the patients with those of a control group showed that contact persons with enteritis and drinking of unpasteurized milk were significantly associated with cryptosporidiosis.

Listeria meningitis in an immunocompetent child.

We report a case of Listeria monocytogenes meningitis in an apparently healthy 3-year-old boy. This is the first case reported in English literature where adequate evaluation of immune status did not reveal an immunologic defect. The patient is in the age group least affected by an opportunistic organism and has been living in a district known for a high incidence of Listeria in livestock. The epidemiologic implication of this observation is discussed.

Effects of different levels of organic and inorganic chromium on growth performance and immunocompetence of broilers under heat stress.

This experiment was carried out to investigate the effects of different levels of organic and inorganic chromium on the performance, immune function and some serum mineral concentrations of broilers under heat stress condition (23.9-37°C cycling). A total of 150 one-day-old broiler chicks according to a completely randomized design were assigned into five treatment groups. Each treatment consisted of three replicates and each replicate contained ten chicks. Chicks were fed on corn-soybean meal basal diets with added different concentrations of chromium (0, 600 and 1,200 μg kg(-1) chromium chlo

The late effects of selected immunosuppressants on immunocompetence, disease incidence, and mean life-span. II. Cell-mediated immune activity.

The late effects of various immunosuppressive insults on cell-mediated immunity in mice were studied in an attempt to assess the role of immune surveillance in the aging process. Results were obtained using susceptibility to allogeneic tumor cell challenge, graft-versus-host reaction (GVHR), blastogenic response to PHA, a thymus derived T cell-specific plant mitogen, and cytolytic activity against allogeneic tumor cells as measures of immunologic activity. In vivo studies late in life show that resistance to allogeneic tumor cells is significantly decreased in thymectomized mice, whereas those

Influence of Feeding Omega-3 Polyunsaturated Fatty Acids to Broiler Breeders on Indices of Immunocompetence, Gastrointestinal, and Skeletal Development in Broiler Chickens.

Modern broiler chickens are associated with rapid growth rates and superior feed efficiency. However, they are also susceptible to physiological and metabolic disorders (e.g., skin lesions, lameness, sudden death, enteric diseases, myopathies) that exert substantial economic losses to producers. This is further exacerbated by consumer pressure and mandated cessation of production practices such as indiscriminate use of antimicrobial growth promoters. Manipulation of broiler breeder (BB) nutrition and management can influence chick quality, robustness, and resilience to stressors in the product

Isolated hepatic histoplasmosis in an immunocompetent patient.

Isolated involvement of the liver in histoplasmosis is rare. We report a case of an immunocompetent patient in their mid-40s who presented with prolonged fever and elevated liver enzymes. Liver biopsy revealed granulomas, and histoplasmosis was confirmed by Periodic Acid-Schiff staining and positive urinary Histoplasma antigen test. No other organ involvement was found. Treatment with Amphotericin B and Itraconazole resulted in complete recovery, and Urinary Histoplasma antigen was negative 6 weeks after treatment initiation. This report highlights the importance of considering histoplasmosis

Pulmonary Fungal Infections in Immunocompetent Patients.

Fungi are a group of microorganisms encompassing thousands of species, with only a minority causing disease in humans. Fungal pathogens and infections are increasing global public health problems. They are an important cause of opportunistic infections in immunocompromised patients; however, they can also affect immunocompetent individuals. This paper reviews the incidence, clinical features, radiographic manifestations, and pathological findings of major fungal infections in non-immunocompromised patients. Only a restricted subset of fungi, specifically the endemic dimorphic fungi, selected A

A Case of Visceral Leishmaniasis in an Immunocompetent Adult.

Visceral leishmaniasis is a rare parasitic infection in immunocompetent individuals, being more frequent in children and immunocompromised individuals. Despite being endemic in several regions of the world, including Portugal, it continues to be underdiagnosed. The most characteristic clinical manifestations include constitutional symptoms, pancytopenia, polyclonal hypergammaglobulinemia, and splenomegaly. Direct identification of the parasite is the preferred diagnostic method; however, this approach is not always feasible. Consequently, serological testing becomes essential, particularly in

Primary sternal osteomyelitis in an immunocompetent adolescent.

Primary sternal osteomyelitis is an uncommon but challenging condition that frequently poses diagnostic difficulties. Imaging plays a crucial role in both diagnosing the condition and assessing potential complications. Treatment typically involves a combination of surgical drainage and a prolonged course of antibiotics. We report the case of a 17-year-old immunocompetent male who presented with chest pain. Laboratory investigations revealed elevated inflammatory markers, and blood cultures, along with contrast-enhanced computed tomography, confirmed the diagnosis of primary sternal osteomyelit

Pasteurella multocida Spondylodiscitis in an Immunocompetent Patient.

We present the case of an 82-year-old woman, previously independent in activities of daily living, who developed fever, myalgias, and headache over one week. Two weeks earlier, she had been treated with antibiotics for a lower respiratory tract infection. The patient had no history of immunosuppression and was a pet owner. She was admitted to the emergency department (ED) with a fever and multiple perforating wounds on her hands. Laboratory findings revealed elevated inflammatory markers, including C-reactive protein and procalcitonin, without an obvious infectious source. During observation a

A Case of Cryptococcal Meningitis in an Immunocompetent Individual.

Cryptococcal meningitis is classically associated with immunocompromised hosts but can also occur in individuals without identifiable immune dysfunction. Its nonspecific and often indolent presentation may delay diagnosis, particularly when early neuroimaging suggests alternative etiologies. We report a case of a 49-year-old immunocompetent man who presented with progressive headaches, sensory changes such as transient hearing loss, visual changes, and balance issues. Initial imaging showed a hyperdense area on CT, which was interpreted as a possible cerebrovascular accident, delaying recognit
