The natural history of influenza infection in the severely immunocompromised vs nonimmunocompromised hosts.

INTRODUCTION: Medical advances have led to an increase in the world's population of immunosuppressed individuals. The most severely immunocompromised patients are those who have been diagnosed with a hematologic malignancy, solid organ tumor, or who have other conditions that require immunosuppressive therapies and/or solid organ or stem cell transplants. MATERIALS AND METHODS: Medically attended patients with a positive clinical diagnosis of influenza were recruited prospectively and clinically evaluated. Nasal washes and serum were collected. Evaluation of viral shedding, nasal and serum cy

Multidetector computed tomography of the acute abdomen in the immunocompromised host: a pictorial review.

Immunocompromised patients include those receiving chemotherapy for malignant disease, post-transplant patients, patients with acquired immunodeficiency syndrome (AIDS), and those receiving steroids for autoimmune diseases. Advances in transplantation, oncology, and the treatment of AIDS have extended these patients' life expectancies and thereby increased the immunocompromised population. Classical clinical signs of abdominal sepsis may be absent in the immunocompromised host. Consequently, the radiologist is increasingly called on to diagnose specific acute intra-abdominal complications asso

Antigen detection, serology, and molecular diagnosis of invasive mycoses in the immunocompromised host.

Understanding the uses and limitations of methods for rapid diagnosis of fungal disease is essential in order to diagnose and treat these infections early in their course. Antigen detection methods are useful for diagnosis of aspergillosis, cryptococcosis, histoplasmosis, blastomycosis, paracoccidioidomycosis, and penicilliosis marneffei. The accuracy of the beta-glucan assay for diagnosis of aspergillosis and candidiasis and its role in fungal diagnosis remains unclear, in part because the few published studies report widely varying specificity. Serologic tests for antibodies are also useful

Cytomegalovirus involving gastric antrum in immunocompromised hosts: a report of 5 cases.

Cytomegalovirus infection, which is common in immunosuppressed patients, only rarely affects the stomach, especially the gastric antrum. We report five patients with cytomegalovirus infection of the stomach with antral involvement. Of these, four had undergone renal transplant and one had HIV infection. All patients presented with upper gastrointestinal symptoms that did not respond to proton pump inhibitors and prokinetic drugs. In addition, all had systemic symptoms. Diagnosis was made at upper GI endoscopy and biopsy, and ganciclovir treatment led to improvement.

Clinical spectrum of hepatic tuberculosis: comparison between immunocompetent and immunocompromised hosts.

BACKGROUND: Hepatic tuberculosis has been reported in normal and immunocompromised hosts. However, no published comparisons between these two groups of subjects with hepatic tuberculosis have been found. The aim of this study was to compare the clinical manifestations, biochemical tests, radiologic features and pathological findings of hepatic tuberculosis in immunocompromised and immunocompetent patients. METHOD: The authors reviewed retrospectively 20 patients with hepatic tuberculosis admitted between January 1993 and October 2000 to Chulalongkorn University Hospital, Thailand. There were

Disseminated cutaneous protothecosis in an immunocompromised host: a case report and literature review.

Protothecosis is an infection caused by achloric algae of the genus Prototheca. These organisms have been isolated from water, sewage, soil, and the slime flux of trees, and are a known cause of disease in other mammals. Infection in humans occurs after traumatic inoculation, producing localized olecranon bursal or, rarely, systemic disease. Only two previous cases of disseminated cutaneous disease have been reported in patients with defective neutrophil function. We describe a rare case of widespread cutaneous dissemination occurring after an arthropod bite in an immunocompromised patient.

US experience with itraconazole in Aspergillus, Cryptococcus and Histoplasma infections in the immunocompromised host.

Itraconazole has emerged as an important new oral agent in the treatment of systemic fungal infections. This paper summarizes the data available on its use in aspergillosis, cryptococcosis and histoplasmosis, compiled in the United States with particular attention to the immunocompromised host. Data have been accrued in open-label studies including 57 patients with cryptococcal disease where the overall response rate among patients with meningitis was 86%, and in 28 patients (8 with acquired immune deficiency syndrome (AIDS) or human immunodeficiency virus (HIV) infection) with invasive asperg

Ganciclovir treatment of cytomegalovirus disease in transplant recipients and other immunocompromised hosts.

Thirty-one immunocompromised patients with severe cytomegalovirus (CMV) disease were treated with intravenous ganciclovir. Twenty-one patients had received transplants--15 bone marrow recipients, five renal allograft recipients, and one liver transplant recipient--while the other ten were immunocompromised due to acquired immunodeficiency syndrome (six), hematologic malignancies (three), and systemic lupus erythematosus (one). They presented with one or more of the following syndromes: CMV pneumonitis (19), CMV of the gastrointestinal tract (six), CMV retinitis (seven), and CMV hepatitis (thre

Within-host SARS-CoV-2 diversity in immunocompromised patients during acute infection.

UNLABELLED: Within-host SARS-CoV-2 diversity during acute infection is largely characterized by low-frequency intra-host single-nucleotide variants (iSNVs), but whether early iSNV patterns differ by immune status remains unclear. We analyzed biobanked diagnostic specimens from adults with RT-qPCR-confirmed SARS-CoV-2 infection collected in Cali, Colombia, between 2020 and 2022. Immunocompromised cases sampled during the acute phase were classified into four clinical categories, and immunocompetent cases from the same catchment area were used for comparison. Whole-genome sequencing (Illumina) y

Disseminated Mycobacterium marinum infection: diagnostic and therapeutic challenges in an immunocompromised host.

Mycobacterium marinum causes a rare, slow-progressing zoonotic infection without distinctive clinical features. History of aquatic exposure increases clinical suspicion. We present a case of disseminated M. marinum infection which progressed from a localised cutaneous infection over 1.5 years. A delayed diagnosis, due in part to a low index of suspicion and underlying CD4 cytopenia, contributed to clinical deterioration. Despite numerous courses of antimicrobial therapy, the patient experienced treatment failure and ultimately succumbed to the disease. Notably, the patient had a longstanding h

Intramedullary spinal cord lesions in an immunocompromised host due to Mycobacterium haemophilum.

Mycobacterium haemophilum is a slow growing acid-fast bacillus (AFB) in the nontuberculous mycobacteria (NTM) group. M. haemophilum typically causes cervicofacial lymphadenitis in children, cutaneous diseases, septic arthritis and osteomyelitis. However, it rarely causes isolated spinal cord disease. We report the first case, to our knowledge, of isolated intramedullary spinal lesions secondary to M. haemophilum. This case involved a patient with newly diagnosed human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS). He developed significant immune reconstitution inflamma

Vaccination in immunocompromised host: Recommendations of Italian Primary Immunodeficiency Network Centers (IPINET).

Infectious complications are a major cause of morbidity and mortality in patients with primary or secondary immunodeficiency. Prevention of infectious diseases by vaccines is among the most effective healthcare measures mainly for these subjects. However immunocompromised people vary in their degree of immunosuppression and susceptibility to infection and, therefore, represent a heterogeneous population with regard to immunization. To date there is no well- established evidence for use of vaccines in immunodeficient patients, and indications are not clearly defined even in high-quality reviews

Fatal haemorrhagic duodenal mucormycosis in a non-immunocompromised host: A case report.

Mucormycosis is an opportunistic infection caused by the fungi of the Mucorales order of the class Zygomycetes. Gastrointestinal mucormycosis is an uncommon, fatal condition accounting for only 7% of the cases. We present the case of a gastroduodenal mucormycosis presenting as recurrent massive hematemesis. We report this case to alert clinicians of this rare but fatal condition and to encourage further research into its pathogenesis and management.

Community acquired respiratory and gastrointestinal viral infections: challenges in the immunocompromised host.

PURPOSE OF REVIEW: Newer molecular diagnostic techniques have advanced the field of clinical microbiology and infectious diseases, particularly with respect to characterizing the role that community acquired viruses play in the clinical course and outcomes of the immunocompromised host. This review will examine recent studies describing the impact of adenovirus, rhinovirus, hepatitis E and norovirus in the course of solid organ and stem cell transplant recipients, as well as their epidemiology and implications for infection prevention and control. RECENT FINDINGS: Adenovirus transmission is p

Paecilomyces lilacinus olecranon bursitis in an immunocompromised host: case report and review.

Paecilomyces lilacinus is a little-known mold that causes rare cases of invasive infections in humans regardless of their immune status. We present a unique case in an immunocompromised host with olecranon bursitis because of multidrug-resistant P. lilacinus treated with systemic ketoconazole therapy and surgical debridement. Recognition of this fungus is difficult initially because of its appearance, which can be confused with that of other fungi. Once this organism has been identified, it is recommended that antifungal susceptibility testing be obtained to guide appropriate therapy. Combinat

Disseminated Histoplasmosis Masquerading as Metastatic Malignancy: A Diagnostic Odyssey in an Immunocompromised Host.

Disseminated histoplasmosis is a rare systemic fungal infection caused by Histoplasma capsulatum, typically reported in immunocompromised individuals. Bilateral adrenal involvement, though a recognized but uncommon manifestation, frequently presents a formidable diagnostic challenge owing to its striking radiologic resemblance to disseminated malignancy. We report a case of a 46-year-old male farmer from India presenting with abdominal pain, a progressive paraumbilical lump, low-grade fever, and weight loss. Contrast-enhanced CT revealed large bilateral adrenal masses, widespread abdominal sof
