BACKGROUND: Primary and incisional hernia can be repaired by multitrocar laparoscopy. Single-access laparoscopy (SAL) recently gained interest to decrease the invasiveness and to reduce the abdominal trauma, besides improved cosmetic results. The authors report first 50 patients who consulted for primary and incisional hernia and treated by SAL prosthetic repair.
PATIENTS AND METHODS: Between December 2009 and March 2012, 50 patients (24 females, 26 males) were submitted to SAL for primary (23) and incisional hernia (27). Mean age was 49.1 ± 15.1 years (17-75), and mean body mass index 29.7 ±…

We compared fascial wounds repaired with non-cross-linked intact porcine-derived acellular dermal matrix versus primary closure in a large-animal hernia model. Incisional hernias were created in Yucatan pigs and repaired after 3 weeks via open technique with suture-only primary closure or intraperitoneally placed porcine-derived acellular dermal matrix. Progressive changes in mechanical and biological properties of porcine-derived acellular dermal matrix and repair sites were assessed. Porcine-derived acellular dermal matrix-repaired hernias of additional animals were evaluated 2 and 4 weeks p…

Acute appendicitis is the commonest cause of acute abdomen. Early indication for surgery -- appendectomy, plays the key role in its therapy. The rate of incisional hernias (of all operated hernias) is high and they are, to a certain extent, caused by technical, mechanical factors and the patient himself. The authors present a case review of a female patient, presenting with atypical urgent abdomen, who was hospitalized with a diagnosis of advanced absces of the abdominal wall, resp. strangulated incisional hernia. Surgical revision confirmed that the condition was caused by perforated gangreno…

BACKGROUND: Hernias commonly coexist with pregnancy; however, an incarcerated hernia with bowel obstruction is rare at advanced gestation and requires urgent intervention.
CASE: A multiparous woman with a known large incisional hernia presented at 33 weeks and 5 days gestational age with acute-onset, upper abdominal pain and nausea. The patient was diagnosed with small bowel obstruction secondary to an incarcerated hernia. She was managed with serial abdominal exams until her repeat cesarean section and simultaneous hernia repair were performed 24 h after admission and betamethasone administr…

BACKGROUND: Abdominal wall defects, such as incisional hernias, are a common source of pain and discomfort and often require repeated surgical interventions. Traditional mesh repair techniques typically rely on fixed overlap based on defect size, without considering important biomechanical factors like muscle activity, internal pressure, and tissue elasticity. This study aims to introduce a biomechanical approach to incisional hernia repair that accounts for abdominal wall instability and to evaluate a visualisation tool designed to support surgical planning.
METHODS: We developed HEDI, a too…

Pregnancy complicated by incisional hernia is rare but can become an obstetric challenge if the gravid uterus becomes displaced or incarcerated into the hernial sac or if there is ulceration of the overlying dermis as a result of increased intra-abdominal pressure being transmitted to the skin. We report a case of a pregnant woman presenting with a large incisional hernia at 19 weeks of gestation and discuss how problems encountered with progressing pregnancy were managed conservatively by adopting a multidisciplinary team approach (which included surgeons and radiologists). She underwent a ca…

UNLABELLED: The use of the anterior technique for abdominal wall components separation combined with "onlay" alloplasty (ACST + onlay) in giant incisional hernias (IH) may pose a surgical challenge as it does not exclude increased intra-abdominal pressure (IAP) and the occurrence of abdominal compartment syndrome (ACS). There remains a high incidence of surgical site complications. In our view, the use of intra-abdominal alloplasty combined with anterior separation of the anterior abdominal wall components (ACST + IPOM) will contribute to the improvement of surgical outcomes in giant IH.

OBJECTIVE: Post-laparotomy wound dehiscence, evantration and evisceration are important complications leading to an increase in both morbidity and mortality. Incisional hernias are frequently observed following abdominal surgeries and their occurrence is related to various local and systemic factors. This study aims to analyze the factors affecting wound healing by investigating the parameters that may cause wound dehiscence, incisional hernia, sinus formation and chronic incisional pain.
MATERIAL AND METHODS: The records of 265 patients who underwent major abdominal surgery were analyzed. Th…

BACKGROUND: During laparoscopic incisional hernia repair, conversion to open surgery is sometimes needed, especially in cases of large complicated incisional hernias. No guidelines exist for determining when conversions should be considered. This study aimed to investigate the safety of a combined technique as an alternative to conversion in the laparoscopic repair of large complicated incisional hernias and to evaluate the impact of early conversion to the combined technique on patient outcome.
METHODS: Beginning in November 2008, early conversion was initiated for patients with large compli…

American journal of transplantation : official journal of the American Society of Transplantation and the American Society of Transplant Surgeons2011eng
Despite the wide popularity of laparoscopic incisional hernia repair (LIHR) in the nontransplant population, there are very few reports of LIHR available in abdominal organ transplant patients and none exclusively on kidney and/or pancreas (KP) transplant patients. We retrospectively reviewed a consecutive series of LIHR in KP transplant recipients performed over a period of 4 years and compared the results with LIHR in non-transplant patients during the same period. A total of 36 transplant patients were compared with 62 nontransplant patients. There were five patients converted to the open p…

BACKGROUND: Abdominal wall hernias are a common complication following open gastric bypass (RYGBP) surgery. In an effort to reduce the incidence of incisional hernias with our form of open RYGBP, progressively smaller, upper midline epigastric incisions have been utilized along with permanent sutures. The purpose of this study is to analyze whether incision location, size and choice of suture material affect the incidence of incisional hernias following open RYGBP.
METHODS: Clinically evident incisional hernias and other wound complications and non-wound related complications were analyzed fo…

BACKGROUND: The computerized noninvasive measurement of respiratory mechanics enables new prospects in the study of respiratory physiopathology in surgical repair of large incisional hernias.
STUDY DESIGN: We studied 10 patients with COPD ventilated with a Servo Ventilator 900C. We measured inspiratory flow by means ofa pneumotacograph, the volume by integrating the flow signal, and esophageal and airway opening pressure by means of two differential pressure transducers (an esophageal balloon measures, separately, chest wall and lung mechanical properties). The signals were sent by an analogi…

BACKGROUND: Adhesion formation between viscera and mesh is almost inevitable following incisional hernia repair with prosthetic mesh. Such adhesions may lead to intestinal obstruction and enterocutaneous fistulae formation and make further laparotomies extremely difficult. Sodium carboxymethylcellulose (SCMC) and Interceed TC7 (oxidized regenerated cellulose) as physical barriers have been shown to be effective in reducing postoperative adhesions.
MATERIALS AND METHODS: To evaluate the effects of SCMC and Interceed TC7, we used an incisional hernia model in rats. A ventral abdominal defect (1…

OBJECTIVE: To estimate the risk factors for and incidence of post-laparoscopy incisional hernia.
METHODS: A questionnaire was sent to all individuals on a mailing list from the American Association of Gynecologic Laparoscopists. Adequately completed responses were entered into a computer-based data bank for analysis. Of the 11,500 surveys mailed, 3293 were returned; of these, 3217 were evaluable.
RESULTS: A total of 933 hernias was reported from an estimated 4,385,000 laparoscopic procedures (an incidence of 21 per 100,000); 167 (17.9%) were reported to have occurred despite fascial closure.…

AIM: This study evaluates trends in incisional hernia repair techniques and their associated outcomes over 10 years. It hypothesizes that advancements in surgical techniques have led to an increase in minimally invasive procedures, resulting in a subsequent decrease in 30-day postoperative complications. MATERIAL & METHODS: A retrospective review of prospectively collected data from the Abdominal Core Health Quality Collaborative (ACHQC) database was conducted from January 2013 to December 2023. Adult patients who underwent incisional ventral hernia repair via open, laparoscopic, or robotic ap…

BACKGROUND: Minimally invasive donor nephrectomy has been shown to be procedurally safe; however, rates of minor complications vary significantly, and long-term surgical complications remain underreported in an era of changing surgical practice. The incidence and predictive factors of incisional hernia (IH) after donor nephrectomy remain underinvestigated, especially with regard to robotic-assisted donor nephrectomy (RADN).
METHODS: A single-institution retrospective analysis of consecutive kidney donors between January 2017 and December 2025 was conducted. Donors were categorized by surgical…
