BACKGROUND AND PURPOSE: Intra-arterial chemotherapy is a very effective treatment option for intraocular retinoblastoma. However, direct catheterization of the OA is not always possible. The purpose of this work was to report our initial results with intra-arterial chemotherapy for intraocular retinoblastoma when delivery of the drug was not via direct catheterization of the OA.
MATERIALS AND METHODS: Retrospective review of 110 eyes (89 patients) undergoing a total of 351 intra-arterial treatments at our institution between 2006 and 2010 identified 18 eyes (14 patients) that received at leas

BACKGROUND: In intra-arterial chemotherapy for retinoblastoma, a backflow from unreachable external carotid artery branches in the ophthalmic artery can be challenging.
OBJECTIVE: To describe a new endovascular technique using Gelfoam pledgets to temporarily occlude distal branches of the external carotid artery to reverse the competitive backflow into the ophthalmic artery in order to perform intra-arterial chemotherapy via the ostium of the ophthalmic artery in selected cases.
METHODS: We queried our prospectively collected database of 327 consecutive patients treated for retinoblastoma by

BACKGROUND: The cardiovascular complications of Marfan syndrome arise due to alterations in the structural and functional properties of fibrillin, a constituent of vascular connective tissues. Fibrillin-containing microfibrils are closely associated with arterial endothelial cells, indicating a possible functional role for fibrillin in the endothelium. Plasma concentrations of endothelial cell products are elevated in Marfan subjects, which indirectly indicates endothelial dysfunction. This study directly assessed flow- and agonist-mediated endothelium-dependent brachial artery reactivity in M

Hepatic artery infusion (HAI) chemotherapy is a regional, intra-arterial treatment of hepatic malignancies, delivering high-dose floxuridine directly to the liver. The rationale for HAI is based on the dominant blood supply of liver tumors from the hepatic arteries and near complete first-pass hepatic extraction of certain chemotherapeutic agents, which limits systemic toxicity. Although HAI chemotherapy is most commonly used among patients with colorectal liver metastases, it is now being more frequently employed for other disease processes such as intrahepatic cholangiocarcinoma and hepatoce

Hepatic arterial infusion (HAI) pumps are used to deliver liver-directed therapy by allowing the administration of selective chemotherapy to the liver via a catheter implanted most commonly into the gastroduodenal artery connected to a subcutaneous pump. This selective administration helps maximize the chemotherapeutic effect within the hepatic tumors while minimizing systemic toxicity. While HAI therapy has primarily been used to treat liver-only metastatic colorectal cancer, the indications have expanded to other malignancies, including intrahepatic cholangiocarcinoma. Radiologists play an i

Standard therapy for frostbite has been unaltered for more than 20 years. Recently there have been reports of improved outcomes with the addition of a vasodilator to intra-arterial thrombolytic therapy. We report the case of severe frostbite of the hands successfully treated with transcatheter intra-arterial tPA after papaverine infusion in a 37-year-old man. Upper extremity and hand angiography within 16 h of the insult showed thrombotic occlusive disease and vasospasm. Intra-arterial catheters were placed in the brachial arteries and a papaverine infusion was initiated, followed by intra-art

Regional intra-arterial infusion of human placental lactogen in anaesthetized pigs has been shown to cause coronary, renal and iliac vasoconstriction by antagonizing the vasodilatory effects of beta2-adrenergic receptors. Since nitric oxide is known to modulate or mediate beta2-adrenergic effects, the present study was planned in the same experimental model to determine the role of nitric oxide in the above vascular responses to human placental lactogen. In eight pigs anaesthetized with sodium pentobarbitone, changes in anterior descending coronary, left renal and left internal iliac blood flo

Adenosine is a potent vasodilator used extensively to study the coronary circulation of animals. Its use in humans, however, has been hampered by lack of knowledge about its effects on the human coronary circulation and by concern about its safety. We investigated in humans the effects of adenosine, administered by intracoronary bolus (2-16 micrograms), intracoronary infusion (10-240 micrograms/min), or intravenous infusion (35-140 micrograms/kg/min) on coronary and systemic hemodynamics and the electrocardiogram. Coronary blood flow velocity (CBFV) was measured with a 3F coronary Doppler cath

BACKGROUND: Intravenous dantrolene has been used to prevent and treat cerebral vasospasm. We report a case of refractory cerebral vasospasm treated with intra-arterial dantrolene after aneurysmal subarachnoid hemorrhage.
METHODS: A 56-year-old woman suffered a diffuse subarachnoid hemorrhage from a ruptured anterior communicating artery aneurysm which was successfully treated with coil embolization. She subsequently developed bilateral severe angiographic vasospasm which was treated with intra-arterial vasodilators. However, owing to the recurrence of vasospasm, intra-arterial dantrolene foll

BACKGROUND: There have been some reports on the use of intra-arterial tirofiban in ruptured intracranial aneurysms, but few studies have reported on the use of 24 h of intravenous tirofiban infusion in patients with subarachnoid hemorrhage.
OBJECTIVE: To present our experience with intravenous tirofiban infusion, in the form of a monotherapy as well as in addition to intra-arterial tirofiban, as a prophylactic, and as a rescue management for thrombus in patients who have undergone embolization with coils for ruptured intracranial aneurysms.
METHODS: Between December 2008 and January 2015, we

1 The cardiovascular responses to dimaprit (S-(3-dimethylaminopropyl) isothiourea) a selective histamine H2-receptor agonist have been studied in anaesthetized cats, dogs, rats and rabbits. 2 Dimaprit lowered systemic arterial blood pressure in all species whether given by intravenous injection or by continous infusion. Intra-arterial injections or infusions caused vasodilatation in the femoral vascular bed. 3 In the cat, continuous intravenous infusions of dimaprit lowered blood pressure without any significant change in heart rate or cardiac output. The fall in blood pressure was thus due to

Pulmonary mean-transit-time volume (PBV) of 12 anesthetized dogs was measured repeatedly by using pairs of dye-dilution curves recorded from the pulmonary artery (PA) and left atrium (LA) after right atrial injections of indocyanine green. In 345 pairs, the areas of the extrapolated LA curves exceeded those of the PA curves by 8% (sd, 14%), indicating that delayed color development of the dye-blood mixture is not a serious source of error in standard dye-dilution techniques. In eight dogs, measurement every 3–5 min before and after 2- or 5-min infusions of morphine (1 mg/kg) revealed no change

Clinical usefulness of arterial redistribution using steel coils to prevent hepatic arterial occlusion and gastric ulcer was compared with the conventional method. Fifty-two patients with liver malignancies were inserted with steel coils into the common hepatic artery after percutaneous hepatic catheterization (CHA-coil) to prevent hepatic arterial occlusion. The tip of the indwelling catheter was fixed to the vascular wall, and the contact area between the catheter and blood into the liver became smaller. Fifty-nine other patients were inserted with steel coils into the right gastric artery (

Continuous hepatic artery floxuridine infusion benefits patients with colorectal liver metastases. Implanted infusion pumps are more expensive but may result in fewer treatment interruptions than when using an external pump connected to a port. We have assessed device-related complications, treatment interruptions and added nurse interventions in 95 patients undergoing a total of 959 treatment cycles via either implanted pump (64 patients) or port (31 patients). Compared with the implanted pump, the port was associated with a significant increase (P < 0.003) in catheter blockage (24/31 vs 2/64

The effect of intra-arterial adenosine on cerebral blood flow was studied in 11 anesthetized dogs. In a first group of 6 dogs, adenosine was infused into a vertebral artery for 40 minutes at a dose of 0.3 to 0.5 mg/kg/min. Blood flow was determined before, during and after the adenosine infusion using the radioactive microsphere technique. In a second group of 5 dogs, adenosine (3 +/- 1 mcg/kg/min) was infused in a similar manner after potentiating its effect with intravenous dipyridamole, and measurements before and after the intravenous dipyridamole and during and after the adenosine infusio

1. Release of VIP from the submaxillary gland, in response to stimulation of the chorda tympani, and its vasodilator action at the site of release have been investigated in anaesthetised cats. 2. Chorda stimulation at 20 Hz produced an abrupt rise in the concentration of VIP in the submaxillary venous effluent plasma, accompanied by a substantial increase in submaxillary blood flow, in the presence or absence of atropine. 3. Intra-arterial infusions of VIP which reproduced the rise in submaxillary venous plasma concentration that occurred during chorda stimulation at 20 Hz, also produced a ris

Aneurysmal subarachnoid hemorrhage (aSAH) is an important cause of death and disability, not just due to the initial event, but also because of the delayed complications. Cerebral vasospasm (CV) stands out as a serious complication, with high prevalence and association with permanent neurologic impairment. The treatment of CV includes non-invasive measures, like oral nimodipine and induced hypertension, but also invasive measures. Endovascular rescue treatment (ERT), with intra-arterial approaches, is linked with improvement of cerebral perfusion and thus associated with a better outcome. Ther

We present the case of a 79-year-old patient with extensive metastatic malignant melanoma (MM) of the scalp. Cutaneous MM of the head and neck often presents a therapeutic challenge. Radical surgical procedures and conventional chemotherapy are often unfeasible and contraindicated because of the difficult anatomy, the extent of the tumour process, and systemic toxicity. In our patient, selective intra-arterial perfusion with pegylated liposomal doxorubicin (PLD) and melphalan was performed after catheterization of both bilateral external carotid arteries with an arterial port system. PLD 4.5 m

Cardioventilatory responses to hypoxia, the O2 chemoreceptor stimulant sodium cyanide (NaCN), and intra-arterial injection of atropine, noradrenaline and DL-propranolol were investigated in the adriatic sturgeon. Hypoxia elicited a bradycardia and hyperventilation. 1 mg NaCN added to water entering the buccal cavity stimulated a transient bradycardia but intra-arterial infusion of 150 micrograms NaCN did not, indicating that hypoxic bradycardia is controlled by chemoreceptors sensitive only to water O2 levels. NaCN stimulated hyperventilation both when added to the water and when infused intra
