Disparities in outcomes and access to therapy options in hepatocellular carcinoma.

BACKGROUND: Hepatocellular carcinoma (HCC) disproportionately impacts racial and ethnic minorities and patients with lower socioeconomic status. These social determinants of health (SDH) lead to disparities in access to care and outcomes. We aim to understand the relationship between SDH and survival and locoregional treatment options in HCC. METHODS: Using the National Cancer Database, we evaluated survival and access locoregional treatments including non-transplant surgery, liver transplant (LT), and liver-directed radiation therapy (LDRT) in patients with HCC diagnosed between 2004 and 201

The Effect of Insurance Type on Patient Access to Carpal Tunnel Release Under the Affordable Care Act.

PURPOSE: To assess the effect of insurance type (Medicaid, Medicare, and private insurance) on access to hand surgeons for carpal tunnel syndrome (CTS). METHODS: The research team called 240 hand surgeons in 8 states (California, Massachusetts, Ohio, New York, Florida, Georgia, Texas, and North Carolina). The caller requested an appointment for her fictitious mother to be evaluated for CTS and possible surgical management through carpal tunnel release (CTR). Each office was called 3 times to assess the responses for Medicaid, Medicare, or Blue Cross Blue Shield. From each call, we recorded wh

The American College of Surgeons' closed claims study: new insights for improving care.

BACKGROUND: All physicians must be vigilant in the pursuit of safe care for patients. While problems in care are identified, education that provides an understanding of these problems and guidelines for improvement can enhance patient safety. Our objective was to determine problematic aspects of surgical care, including care provided by surgeons before, during, after, and instead of surgery, that negatively affect patient safety. STUDY DESIGN: Four hundred sixty malpractice claims against general surgeons were reviewed by surgeons (FACS). All claims were closed in 2003 or 2004. The data colle

Practice-Level Severity Case Mix and Treatment Patterns for Premenopausal Noncancerous Hysterectomy.

Background: Hysterectomy for noncancerous conditions is a patient-preference-sensitive procedure. Therefore, gynecological practices may provide hysterectomy at varying levels of symptom severity. We assess whether practice-level severity case mix associates with segregation of patients by race and ethnicity or insurance status. Methods: In this case series, we analyzed electronic health records of 1,590 noncancerous hysterectomy patients across 20 clinical practices within a large health care system in the U.S. South (2014-2017). By abstracting 12-month presurgical medical notes, we developed

Providers' and Survivors' Perspectives on Affordability Challenges for Gastrointestinal Cancer Treatment in Two Low Socioeconomic Status States of the Southern United States.

INTRODUCTION: Affordability is an access to care domain contributing to disparities in gastrointestinal (GI) cancer outcomes and care, including surgical care. Affordability challenges for GI cancer care in socioeconomically disadvantaged and diverse states of the southern United States are unknown: this paper addresses this knowledge gap. METHODS: We conducted semi-structured interviews with 32 providers (13 surgeons and 19 other) and 36 survivors (53% colorectal, 19% pancreatic, and 28% esophageal cancer) in Alabama and Mississippi. Questions were about barriers and facilitators to surgical

Racial and Socioeconomic Disparities in Prompt Craniosynostosis Workup and Treatment.

The purpose of this study was to identify racial and socioeconomic disparities in craniosynostosis evaluation and treatment, from referral to surgery. Patients diagnosed with craniosynostosis between 2012 and 2020 at a single center were identified. Chart review was used to collect demographic variables, age at referral to craniofacial care, age at diagnosis, age at surgery, and surgical technique (open versus limited incision). Multivariable linear and logistic regression models with lasso regularization assessed the independent effect of each variable. A total of 298 patients were included.

Socioeconomic and regional differences in the treatment of cervical spondylotic myelopathy.

BACKGROUND: Cervical spondylotic myelopathy (CSM) is the leading cause of spinal cord dysfunction in the world. Surgical treatment is both medically and economically advantageous, and can be achieved through multiple approaches, with or without fusion. We used the Nationwide Inpatient Sample (NIS) database to better elucidate regional and socioeconomic variances in the treatment of CSM. METHODS: The NIS database was queried for elective admissions with a primary diagnosis of CSM (ICD-9 721.1). This was evaluated for patients who also carried a diagnosis of anterior (ICD-9 81.02) or posterior

Influence of Health Insurance Expansion on Disparities in the Treatment of Acute Cholecystitis.

OBJECTIVE: To evaluate the impact of the 2006 Massachusetts (MA) health reform on disparities in the management of acute cholecystitis (AC). BACKGROUND: Immediate cholecystectomy has been shown to be the optimal treatment for AC, yet variation in care persists depending upon insurance status and patient race. How increased insurance coverage impacts these disparities in surgical care is not known. METHODS: A cohort study of patients admitted with AC in MA and 3 control states from 2001 through 2009 was performed using the Hospital Cost and Utilization Project State Inpatient Databases. We ex

The impact of social determinants of health on access to partial nephrectomy, minimally invasive surgery, and robotic-assisted approaches for renal cell carcinoma.

PURPOSE: Social determinants of health (SDOH), including race/ethnicity, socioeconomic status (SES), insurance coverage, and geographic location, are established drivers of disparities in surgical access and outcomes. Robotic-assisted partial nephrectomy (RAPN) has become the preferred minimally invasive approach for localized renal cell carcinoma (RCC), yet the extent to which SDOH influence access to partial nephrectomy, minimally invasive surgery, and RAPN specifically, as well as postoperative outcomes has not been comprehensively characterized. METHODS: A systematic review was conducted

Inpatient surgical site infection after shoulder arthroplasty.

BACKGROUND: Surgical site infection (SSI) after joint arthroplasty is associated with prolonged hospitalization, reoperation, inferior outcomes, and substantial resource utilization. As the number of shoulder replacements performed in the United States continues to rise, measuring the incidence of inpatient SSI after hemiarthroplasty (HSA) and total shoulder arthroplasty (TSA), and associated risk factors for infection is worthwhile. METHODS: Using the Nationwide Inpatient Sample (NIS), we reviewed 241,193 patients undergoing TSA or reverse TSA and 159,795 undergoing HSA between 2002 and 2011

Missed Opportunities for Multimodality Therapy in Resectable Pancreatic Cancer: National Disparities and Consequences for Survival.

BACKGROUND: Multimodal therapy (MMT) combining surgery and chemotherapy remains the optimal management strategy irrespective of sequence for resectable pancreatic ductal adenocarcinoma (PDAC). Determinants of MMT receipt and sequencing remain incompletely understood, particularly regarding neoadjuvant strategies. We aimed to identify modifiable characteristics contributing to differences in MMT receipt and sequencing in the surgical PDAC population. METHODS: Using the 2023 National Cancer Database, we analyzed 11 202 adults with non-metastatic resectable PDAC (2010-2020). Patients were groupe
