Does the interpersonal model apply across eating disorder diagnostic groups? A structural equation modeling approach.

BACKGROUND: Interpersonal model has been validated with binge-eating disorder (BED), but it is not yet known if the model applies across a range of eating disorders (ED). PURPOSE: The goal of this study was to investigate the validity of the interpersonal model in anorexia nervosa (restricting type; ANR and binge-eating/purge type; ANBP), bulimia nervosa (BN), BED, and eating disorder not otherwise specified (EDNOS). PROCEDURE: Data from a cross-sectional sample of 1459 treatment-seeking women diagnosed with ANR, ANBP, BN, BED and EDNOS were examined for indirect effects of interpersonal pro

Therapists' perceptions of interpersonally challenging patients in generalized anxiety disorder.

OBJECTIVE: This study explores in cognitive-behavioral therapy (CBT) for generalized anxiety disorder (GAD) to what degree therapists' perceptions of their patients as interpersonally challenging (IC) is explained by the therapist or patient effects, if baseline patients' characteristics predict IC, and if IC is related to outcome. METHOD: Eighty patients diagnosed with GAD and treated by 20 therapists with 16 sessions of CBT were randomized to two different implementation conditions. Patients completed baseline measures of depression, anxiety, interpersonal problems, and interpersonal streng

Bulimia nervosa. Uncovering a secret disorder.

Bulimia nervosa is a common eating disorder among women. Sociocultural and interpersonal pressures are thought to be highly influential in the development of the syndrome. An association with depression has also been suggested. Recognizing bulimia nervosa is usually difficult, because patients are hesitant to reveal their symptoms. If the condition is diagnosed early in its course, the prognosis is more favorable. Use of tricyclic antidepressants and psychotherapy is the accepted treatment regimen.

Mentalization-Based Group Therapy for Personality Disorder in Adolescence: A Single Case Study.

Treating personality disorders in adolescents presents a significant challenge, as intense emotions and unstable relationships often lead to early dropout from therapy. This is particularly concerning, given that psychotherapy is the primary treatment for personality disorders and that such a diagnosis is typically associated with severe difficulties daily functioning. The case report of a 16-year-old girl with borderline personality disorder illustrates how a history of trauma and early unstable relationships can contribute to later difficulties in emotion regulation and impair the ability to

Psychological treatments for eating disorders.

PURPOSE OF REVIEW: This review summarizes recent evidence on psychological treatments for eating disorders. RECENT FINDINGS: Eating disorders are serious psychiatric conditions requiring evidence-based intervention. Treatments have been evaluated within each eating disorder diagnosis and across diagnoses. For adults with anorexia nervosa, no one specialist treatment has been shown to be superior. Cognitive behavioral therapy and interpersonal psychotherapy remain the most established treatments for bulimia nervosa and binge eating disorder, with stepped-care approaches showing promise and new…

Umwelt-A New Strategy for Mentalizing Patient Experience.

There is an inevitable limit to understanding the internal experience of patients with whom therapists work in psychotherapy. The farther their experience is from that of their therapist, the more challenging this endeavor can be. Accepting that therapists cannot exactly know a patient's internal experience invites them to explore novel ways of appreciating another person's way of perceiving relationships, deriving meaning from them, and using their experience to motivate behavior. The concept of umwelt can be used as a metaphor to help therapists imagine the internal world of their patients,

Working through the body in metacognitive interpersonal therapy to change relational patterns in dependent personality disorder: The case of Lia.

Persons with dependent personality disorder (DPD) have difficulties describing their inner world, and in realizing their negative ideas about the self, such as being weak, unworthy or powerless are just ideas. As a consequence, they tend to over-rely on others and may lose control over their emotions. Treating these persons can gain benefits from including body-focused techniques as they can promote a) awareness of internal states, b) better emotion regulation, c) the capacity to consider their negative ideas about themselves as not necessarily true, and d) gain power of and agency. We will de

Dissociation predicts symptom-related treatment outcome in short-term inpatient psychotherapy.

OBJECTIVE: Previous research has indicated that dissociation might be a negative predictor of treatment outcome in cognitive behavioural therapy for patients with obsessive-compulsive and anxiety disorders. Using a naturalistic design it was hypothesized that higher levels of dissociation predict poorer outcome in inpatients with affective, anxiety and somatoform disorders participating in a brief psychodynamic psychotherapy. METHOD: A total of 133 patients completed the Symptom Check List (SCL-90), the German short version of the Dissociative Experiences Scale and the Inventory of Interperso

The association between patient characteristics and the therapeutic alliance in cognitive-behavioral and interpersonal therapy for bulimia nervosa.

The therapeutic alliance is an established predictor of psychotherapy outcome. However, alliance research in the treatment of eating disorders has been scant, with even less attention paid to correlates of alliance development. The goal of this study was to examine the relation between specific patient characteristics and the development of the alliance in 2 different treatments for bulimia nervosa (BN). Data derive from a large, randomized clinical trial comparing cognitive- behavioral therapy (CBT) and interpersonal therapy (IPT) for BN. Across both treatments, patient expectation of improve

Relational sexuality: an understanding of low sexual desire.

Low sexual desire is discussed within the context of the relationship in which it occurs, with emphasis on each partner's intrapsychic issues as they intersect with the interpersonal issues between them. The conceptualization of the problem is based in object relations theory, whereas treatment involves systems theory and gestalt and experiential psychotherapy. Relational sexuality and intimacy in marriage are presented as foundations for treatment procedures. The therapy techniques are illustrated by case examples.

A psychotherapist engages in behaviour therapy.

The article reviews clinical findings that led an experienced psychotherapist to favour behaviour therapy over psychotherapy for patients who have anxiety disorders involving avoidance of specific situations or avoidance of specific activities. It is maintained that anxiety disorders are best treated through psychotherapy when they are demonstrably connected to disturbances in interpersonal relations. Three basic procedures in behaviour therapy are described: desensitization, direct exposure, and cognitive restructuring. Transference patterns in each of these procedures are delineated. A fourt

Patients' attitudes and expectations toward a digital inpatient-like psychotherapy concept: a qualitative interview study.

BACKGROUND: In recent years, digital mental health interventions have become increasingly important due to the rising demand for psychotherapy and the shortage of resources. Digital inpatient-like psychotherapy (DIPT) is a therapeutic concept that allows patients to receive inpatient-like psychotherapy in a digital environment, encompassing multiple dimensions of psychotherapy. It represents a comprehensive and intensive form of psychotherapeutic treatment. OBJECTIVE: This qualitative interview study sheds light on patients' attitudes and expectations toward digital inpatient-like psychothera

Comparing Therapeutic Treatment Goals Across Settings for Those in or At-Risk for Substance Use Treatment in West Alabama.

BACKGROUND: Despite the variety of treatment programs for those with or at risk for substance use disorders (SUD), relatively little is known about treatment goals beyond substance use reduction across treatment settings. Understanding these treatment goals may inform the tailoring of treatment and improve implementation. METHODS: This study compares treatment goals across 4 years at 3 treatment facilities. All sites were situated in West Alabama, a geographic region known for substantial health, economic, and educational disparities as well as behavioral and mental health stigma. Psychosocia

Metacognitive Interpersonal Therapy-Eating Disorders (MIT-ED) Versus CBT-E for Adults: A Proof-of-Concept Randomized Controlled Trial.

OBJECTIVE: There is an urgent need for psychological therapies in Eating Disorders (ED) that target both eating disorder symptoms and broader aspects of psychological functioning, such as personality disorder (PD) features. We integrated Metacognitive Interpersonal Therapy, developed for treating PDs, with existing CBT techniques for eating disorders (MIT-ED). This psychotherapy targets aspects of ED that are not included in the transdiagnostic CBT-E model, specifically poor metacognition and interpersonal schemas. METHOD: A proof-of-concept randomized clinical trial (RCT) recruited 21 indivi

Pharmacologic and psychotherapeutic issues in coexistent paranoid schizophrenia and narcolepsy: case report.

The case of a 32-year-old man with schizophrenia and narcolepsy, two seemingly unrelated disorders, is discussed from the perspective of the diagnostic and therapeutic challenges raised by their coexistence. In addition, the development of tardive dyskinesia and its subsequent amelioration with a depot form of a high-potency neuroleptic are discussed in relation to these disorders. Consistent and supportive psychotherapy for such patients is recommended for maintaining compliance, for pharmacotherapy, and an optimal level of personal, occupational, and interpersonal functioning.
