BACKGROUND: Keloids are benign fibroproliferative tumors that often recur after surgical excision. Combining surgery with postoperative radiotherapy has emerged as a potential treatment strategy, though optimal radiotherapy protocols remain debated.
OBJECTIVE: This study aims to preliminarily evaluate the efficacy and safety of a single high-dose brachytherapy session administered within 8 h after surgical excision in keloid-prone patients in a retrospective setting.
METHODS: A retrospective case series analysis was conducted on 32 patients who underwent surgical excision of keloids followed…

The current understanding and a standardized assessment or treatment guidelines for keloids are not fully established, highlighting the need for an objective method to gauge keloid severity and treatment outcomes. This study investigated the clinical utility of multimodal ultrasound, integrating Shear Wave Elastography (SWE) and Angio planewave ultrasensitive imaging (AP), to assess keloid severity and treatment responses in 58 keloids across 31 patients. Keloids were categorized into mild, moderate, and severe based on Vancouver Scar Scale (VSS) scores. The results revealed significant differ…

Keloid formation following caesarean section can be a distressing complication. Women of African, Hispanic or Asian descent are more predisposed to abnormal scar healing. During pregnancy, hormonal and mechanical factors may further aggravate keloid growth, leading to pain, cosmetic disfigurement, and emotional distress. Managing such cases requires careful coordination between obstetric, surgical, and dermatological teams to achieve safe maternal and cosmetic outcomes. A 29-year-old gravida 2 para 1 woman of African origin, with a previous history of emergency caesarean section, was seen at 2…

Keloids are pathological scars that form due to abnormal wound healing, characterized by an overgrowth of collagen. They develop beyond the sites of the original lesion and have a high recurrence rate, are cosmetically deforming, can cause irritation, physical discomfort, and psychological stress, and frequently impact the quality of life. Despite an array of therapeutic approaches, both surgical and non-surgical, no solutions have been able to offer long-term relief. Given the limited understanding of keloid etiology, our study employed a novel data-driven approach using the k-modes clusterin…

BACKGROUND: Keloids are dermal fibroproliferative tumours characterised by excessive deposition of extra cellular matrix components. The active form of vitamin D is known to inhibit the proliferation of keloid fibroblasts by inhibiting extracellular matrix production induced by transforming growth factor ß (TGF-β) and increasing matrix metalloproteinase (MMP) activity. Vitamin D derivatives are thought to be an early preventive treatment strategy for keloid.
AIM: To determine the correlation between serum 25-hydroxyvitamin D level with keloid severity.
METHODS: This is a cross-sectional anal…

Keloid following circumcision has been described in the literature despite the rarity of its occurrence in penile skin. In this paper, we review the literature and report the successful management of post-circumcision keloid scarring in a 2-year-old boy. After circumcision a 2-year-old boy of African origin developed keloid scarring at the circumcision site. This was treated with three intralesional injections of triamcinolone acetate over 3 months, followed by surgical excision. There was no recurrence at 6 months after excision. To our knowledge this is the 12th case of keloid following paed…

Mortalin (Mot) is a mitochondrial chaperone of the heat shock protein 70 family and it's pro-proliferative and anti-apoptosis functions could be associated with keloid pathogenesis, and blocking of mortalin and its interaction with p53 might be a potential novel target for the treatment of keloid. Therefore, we generated mortalin-specific small hairpin (sh) RNAs (dE1-RGD/GFP/shMot) and introduced into keloid spheroids for examination of its apoptotic and anti-fibrotic effect. On keloid tissues, mortalin expression was higher than adjacent normal tissues and it's protein expressions were activa…

BACKGROUND: Intralesional cryotherapy is a novel treatment for keloid scars in which the scar is frozen from inside. Published results are promising, but the treatment has only been tested in a Caucasian patient population. Therefore, the authors evaluated intralesional cryotherapy in a patient population including different Fitzpatrick skin types (I through VI).
METHODS: This prospective multicenter study with a 1-year follow-up included 27 patients with 29 keloid scars. Intralesional cryotherapy was administered with a disposable liquid nitrogen-based device called CryoShape. Scar assessmen…

BACKGROUND: Keloid is a fibrotic skin disease for which immune cell infiltration is a primary pathological hallmark. Meanwhile, in autoimmune diseases, triggering of the inflammation response can lead to tissue injury and subsequent organ fibrosis. When the skin is involved in autoimmune disease, skin fibrosis such as that seen in scleroderma can occur. In this study, we propose that keloid possesses features of autoimmune disease.
METHODS: To verify whether keloid possesses features of autoimmune disease, immune cell infiltration and immune complex deposits were detected with immunohistochem…

Although combination therapy for keloid including postoperative radiation therapy (RT) is common, the radiation toxicity of RT in a patient with a history of collagen vascular disease has not been fully recognized. We experienced a case of an acute radiodermatitis in a patient with keloid. This patient had a chest keloid because of the bypass surgery for Takayasu's arteritis. After we performed an excision and postoperative RT, severe radiodermatitis occurred. We speculate that the higher single dose and the use of electron beams may be related to the onset of severe acute radiodermatitis in t…

Hypertrophic scar (HS) and keloid (KL) are two forms of an abnormal cutaneous scarring process, mainly characterized by excessive extracellular matrix deposition and fibroblast proliferation. Despite the increased understanding of the molecular and cellular events leading to HS and KL, the pathogenesis of these lesions remains poorly understood. A pivotal role in the formation of abnormal scars has been ascribed to transforming growth factor-beta, whose activity appears to be mediated through a link with pathways acting via cyclooxygenases (COX-1 and COX-2). To date, there is no report on the …

BACKGROUND: The pathogenesis of keloid and hypertrophic scar is less well understood because of the lack of animal models. The cell-surface Fas receptor, which is widely distributed in skin components, has been shown to be an important factor that induces apoptosis in human dermal fibroblasts. In this report, the authors performed a comparative study on apoptotic signal transduction in fibroblasts derived from keloids and hypertrophic scars.
METHODS: Fibroblasts harvested from 10 patients with keloids and hypertrophic scars were used for this study. At first, apoptosis induction using Fas ant…

BACKGROUND: Keloid is a hypertrophic scar that may arise within 6 months after injury in susceptible individuals. Different therapies like surgical excision, intralesional steroid injections, local application of pressure, or postoperative irradiation with x-rays or electrons are reported. Although an immediate starting of therapy after surgery is usually recommended, delayed radiotherapy may also be effective.
CASE REPORT: We report on a 48 year old women with a history of an invasive ductal carcinoma in the upper lateral quadrant of the left breast. A breast conserving tumor resection with …

PURPOSE: To study the integration of keloid heterograft in hamster (Mesocricetus auratus) cheek pouch.
METHODS: The sample is formed by 18 male hamsters, heterogenic ones, aged between 10 and 14 weeks. Keloid fragments were obtained from keloid scars of the breast region of adult female mulatto patient. Each hamster received keloid fragments into both of its pouches, in a total of 36 grafted fragments. Animals were distributed into 6 groups for having their grafts assessed in the days 5, 12, 21,42, 84, and 168. A macroscopic assessment is performed by comparing the pouch containing the grafte…

Hypertrophic scars and keloids continue to plague the head and neck surgeon as an unpredictable consequence of trauma or incision. The myriad suggested methods of revising such lesions indicate that no one method is foolproof. The application of conventional cryotherapy to the lesion, followed by the dilute intralesional triamcinalone injection, offers several advantages over other methods.
