[Treatment of neonatal jaundice].

BACKGROUND: Examination for and treatment of jaundice is one of the most common occurrences in neonatal medicine. Neonatal jaundice has a foundation in normal physiology, but may be exacerbated by a number of factors. Therapy is given because pronounced jaundice carries a risk for brain damage (kernicterus). Intervention limits for therapy continue to be debated, as the scientific basis suffers from obvious weaknesses. MATERIAL AND METHODS: The American Academy of Pediatrics recently published new guidelines for the management of hyperbilirubinaemia in newborns. The new guidelines differ in i…

Phenobarbital following phototherapy for Crigler-Najjar syndrome type II with good fetal outcome: a case report.

In the presence of severe hyperbilirubinemia in Crigler-Najjar syndrome Type II, a fetus is at risk for kernicterus. A 34-year-old woman, gravida 4, para 1, with Crigler-Najjar syndrome Type II was treated with phenobarbital administration following phototherapy during each of 2 pregnancies. Both infants were healthy and developed normally.

Vigintiphobia revisited.

In this review the historical tenets and evidence-based clinical research in support of a bilirubin exchange threshold of >20 mg/dL for the healthy term neonate are revisited. In addition, a hypothesis is ventured that recent cases of kernicterus are related in part to changes in population factors coupled with genetic predispositions that have unmasked an unappreciated potential for marked neonatal hyperbilirubinemia.

Neonatal management and outcome in alloimmune hemolytic disease.

Hemolytic disease of the fetus and newborn (HDFN) occurs when fetal and neonatal erythroid cells are destroyed by maternal erythrocyte alloantibodies, it leads to anemia and hydrops in the fetus, and hyperbilirubinemia and kernicterus in the newborn. Postnatal care consists of intensive phototherapy and exchange transfusions to treat severe hyperbilirubinemia and top-up transfusions to treat early and late anemia. Other postnatal complications have been reported such as thrombocytopenia, iron overload and cholestasis requiring specific management. Areas covered: This review focusses on the cur…

Treatment of Crigler-Najjar Syndrome type 1 by hepatic progenitor cell transplantation: a simple procedure for management of hyperbilirubinemia.

Crigler-Najjar Syndrome (CNS) is characterized by mild, chronic unconjugated hyperbilirubinemia resulting from an autosomal-recessive inherited deficiency of hepatic uridine/diphosphoglucuronate-glucuronosyl transferase 1Al since birth. Herein we have reported a confirmed case of CNS type 1 in a 2-year-old girl with an unconjugated hyperbilirubinemia (>30 mg/dL) treated by hepatic progenitor cell infusion through the hepatic artery. No procedure-related complications were encountered. No kernicterus was observed. The total bilirubin started falling at 10 days after cell infusion. Two months af…

The management of hemolytic disease in the fetus and newborn.

Rh hemolytic disease (HDN) is the prototype of maternal alloimmunization and fetal hemolytic disease. There are other antigens capable of causing alloimmunization and hemolytic disease such as c, Kell, and Fya. Rh immunization is usually caused by a prior Rh positive fetal maternal transplacental hemorrhage, which occurs in at least 75% of pregnancies. Unless treated, hemolytic disease will result in kernicterus or fetal hydrops in 25% of cases, respectively. Neonatal exchange transfusion has eradicated kernicterus. Measures available to predict severity of fetal hemolytic disease are maternal…

Berberine-induced haemolysis revisited: safety of Rhizoma coptidis and Cortex phellodendri in chronic haematological diseases.

Two commonly used berberine-containing Chinese herbs, Rhizoma coptidis (RC) and Cortex phellodendri (CP), have been banned in Singapore for the past three decades due to implication of berberine in aggravating jaundice and kernicterus in neonates with glucose-6-phosphate dehydrogenase deficiency. Here we conducted a single arm, phase I/II clinical study on Chinese herbal medicine for patients with chronic cytopenic haematological conditions and we analysed a subset of 20 patients who also had RC, CP or both in their herbal concoction. We found no organ toxicity or electrolyte imbalance in thes…

Shrill cry, opisthotonus, and sun setting sign in a yellow neonate.

We report a case of severe neonatal hyperbilirubinaemia (NNH) in a term neonate due to rhesus isoimmunisation resulting in classical clinical features of acute bilirubin encephalopathy and its sequel, kernicterus. Though NNH is common, its complications are less often encountered today with the widespread availability of effective phototherapy. It is important not to miss the clinical signs of this preventable cause of neuro-disability.

Hyperbilirubinemia in otherwise healthy infants.

Treatment of hyperbilirubinemia in otherwise healthy infants is controversial, since hyperbilirubinemia in the absence of hemolytic disease, hepatobiliary disease or prematurity is not clearly associated with kernicterus or other long-term morbidity. Phototherapy, exchange transfusion and the withholding of breast milk are the only treatments that have proved effective. Home phototherapy may be useful in treating infants for whom hospitalization is otherwise unnecessary.

The "bronze baby" syndrome: postmortem data.

The case history and autopsy findings of an infant with the "bronze baby" syndrome are presented. These findings substantiate that kernicterus occurs in term infants receiving phototherapy for concentrations of serum indirect bilirubin below 20 mg/dl. The findings at autopsy suggest that the photodecomposed pigmented products of bilirubin are unable to pass the blood-brain barrier. The need for establishing the cause of jaundice prior to initiation of phototherapy is stressed.

Decreased neonatal jaundice readmission rate after implementing hyperbilirubinemia guidelines and universal screening for bilirubin.

Readmission rate for neonatal jaundice approximate 10 per 1000 live births. After applying hyperbilirubinemia guidelines and universal screening for bilirubin in term and near-term newborns, the readmission rate declined significantly from 24 to 3.7 per 1000 live births. Decreased readmission rate for neonatal jaundice may reduce kernicterus rate and health care costs. Further studies are necessary to explore these potential benefits.

Guidelines for detection and management of hyperbilirubinemia in term and late preterm newborns (≥35 weeks gestational age).

Hyperbilirubinemia is a common condition and is usually benign in term and late preterm newborns. Developing a standardized approach for all newborns to prevent, identify, and manage those with clinically significant hyperbilirubinemia will minimize the potential risks of long-term neurological sequelae from acute bilirubin encephalopathy and kernicterus. This statement provides recommendations on universal screening, risk factors assessment, and approaches to hyperbilirubinemia treatment, investigations, and monitoring.

Clostridium difficile infection precipitating hemolysis in glucose-6-phosphate dehydrogenase-deficient preterm twins causing severe neonatal jaundice.

The incidence of glucose-6-phosphate dehydrogenase deficiency in neonates of immigrant mothers in Canada is increasing. Newborn screening programs in Canada do not screen for this disorder. Infants with G-6-PD deficiency may develop jaundice resulting in kernicterus with devastating sequelae. In this case report, we speculate that Clostridium difficile infection may have triggered severe jaundice in G-6-PD-deficient neonates.

Bilirubin encephalopathy in a term infant after planned home delivery.

A term infant was delivered at home by his father after a pregnancy which involved no organized medical care. By the third day of life the infant developed fever, poor feeding and severe jaundice. The infant was found to have a serum indirect bilirubin of 49 mg/dl secondary to isoimmune hemolytic anemia due to anti-c antibody. The infant survived but suffers from clinical manifestations of kernicterus.
