[Toxic hepatitis caused by ketoconazole: a report of 4 cases].

The authors describe four cases of toxic hepatitis due to ketoconazole. All patients were female and were given the drug from 8 to 16 weeks (200mg/day). The clinical condition of the four patients was similar to that of viral hepatitis. Normalization of clinical and laboratory findings occurred 60 to 90 days after discontinuation of the drug and no patient died. One patient received ketoconazole again after her clinical and laboratory results were normalized, and a new toxic hepatitis appeared within 15 days; the second discontinuation of the drug lead to cure of the hepatitis.

Efficacy of itraconazole in blastomycosis in a murine model and comparison with ketoconazole.

Mice were infected intranasally with B. dermatitidis yeasts, and after infection treated orally. Itraconazole 50-150 mg/kg/day was protective (prolonged survival) against lethal infection, although the infection was not sterilized. Itraconazole was approximately 3 times as potent as ketoconazole. These results suggest advantages for itraconazole therapy clinically.

Interaction of miconazole, ketoconazole and itraconazole with rat-liver microsomes.

The interaction of the antimycotics miconazole, ketoconazole and itraconazole with liver microsomes from untreated rats or from rats pretreated with phenobarbital or 3-methylcholanthrene, gave rise to type II difference spectra. The interactions of the antimycotics with control, phenobarbital-induced or 3-methylcholanthrene-induced microsomes were biphasic, except for the monophasic binding of ketoconazole to phenobarbital-induced microsomes. The N-demethylation of N,N-dimethylaniline, the O-demethylation of p-nitroanisole and the hydroxylation of aniline in microsomes from untreated and induc…

[Orbital zygomycosis (mucormycosis) in a healthy child. Treatment with ketoconazole].

A case of orbital zygomycosis, probably mucormycosis, histologically demonstrated was observed in a healthy host. The etiologic agent could not be identified because of the unsuccessful cultures. No clinical antecedent allowed us to suppose that an immunological deficiency statement was present. It seemed that the fungus did not penetrates by the classical nasal and sinusal pathways. The fungus inoculation probably resulted from a subocular traumatism. The ketoconazole treatment, the only one possible in our case, was efficient.

Experience with ketoconazole in three major manifestations of progressive coccidioidomycosis.

Forty-seven courses of oral ketoconazole therapy administered to 46 patients with progressive coccidioidomycosis were evaluated. Responses, of varying degrees evaluated by a point system, occurred in 81, 94, and 91 percent of courses evaluated for skeletal, chronic pulmonary, and cutaneous disease, respectively. Presumed side effects occurred in 26 percent of courses and were generally minor and reversible. Relapse occurred in 33, 33, and 11 percent of evaluable responding patients with skeletal, chronic pulmonary and cutaneous disease, respectively. From this experience, ketoconazole appears …

Treatment of paracoccidioidomycosis, candidiasis, chromomycosis, lobomycosis, and mycetoma with ketoconazole.

The authors present the use of ketoconazole in 27 cases of paracoccidioidomycosis, eight of mycetoma, seven of chromomycosis, four of systemic candidiasis and one of lobomycosis. The drug was administered orally in a dosage of 200 to 400 mg per day within a period of up to 90 days. The results of the treatment for paracoccidioidomycosis were of cicatrization of the cutaneous lesions in three to four weeks in 24 patients and in two, from six to seven weeks. Out of 27 patients, 14 presented pulmonary lesions. The evolution within a 90-day period showed radiological cure in one case, improvement …

Sulfur cream blowing and parching combined with compound ketoconazole cream versus compound ketoconazole cream alone in patients with palmoplantar keratotic eczema: a pilot study.

BACKGROUND: Sulfur cream is reported to relieve itching and treat skin diseases caused by bacterial, fungal, and scabies infections. However, there is limited data on the efficacy of sulfur cream blowing and parching combined with compound ketoconazole ointment (ketoconazole, clobetasol propionate, and neomycin sulfate) in curing palmoplantar keratotic eczema. This prospective study proposed to investigate the clinical efficacy of sulfur cream on people who suffer with palmoplantar keratotic eczema. METHODS: In total, 116 patients with palmoplantar keratotic eczema (those who met the diagnost…

Comparative efficacy and tolerability of Ketomousse (ketoconazole foam 1%) and ketoconazole cream 2% in the treatment of pityriasis versicolor: results of a prospective, multicentre, randomised study.

Ketomousse (K), a new thermophobic formulation (ketoconazole 1%), has proven its efficacy in the treatment of dandruff, caused by the same agent as pityriasis versicolor (PV). The objective of this study was to compare the efficacy and tolerability of K thermophobic foam vs. ketoconazole cream 2% (N) in the treatment of PV. Forty-six patients (22 in K and 24 in N group) with PV involving no more than 15% of the total trunk surface were randomly assigned for treatment either with K or N once daily for 14 days. Three weeks after the completion of treatment, improvement rate and side-effects were…

Effects of long-term ketoconazole therapy on serum lipid levels.

Serum cholesterol and triglycerides were determined in 36 patients receiving an 8-month course of oral ketoconazole 200 mg/day in order to study its effect on lipid metabolism. The mean serum cholesterol concentration had decreased by 15% (p less than 0.001) after 1 month, but on continued medication it returned to the pretreatment state; after discontinuation of therapy it increased transiently by 13% (p less than 0.001). Triglycerides increased during ketoconazole administration and at the end of the trial the mean triglyceride concentration was 48% higher than the baseline value (p less tha…

A highly sensitive LC-MS/MS method for determination of ketoconazole in human plasma: Application to a clinical study of the exposure to ketoconazole in patients after topical administration.

A simple, rapid and highly sensitive LC-MS/MS method was developed and validated for the determination of ketoconazole in human plasma. Sample preparation was accomplished through a single step liquid-liquid extraction by ethyl acetate. The chromatography separation was carried out on a Hedera CN (150mm×2.1mm, 5μm) column with isocratic elution using acetonitrile and 10mM ammonium acetate containing 0.1% formic acid (45:55, v/v) as the mobile phase. The flow rate was 0.5mL/min. Detection was performed in the positive ion electrospray ionization mode using multiple reaction monitoring of the tr…

Putative contact ketoconazole shampoo-triggered pemphigus foliaceus in a dog.

A 10-year-old spayed female cocker spaniel dog was referred for an evaluation of acute-onset generalized pustular cutaneous lesions following application of ketoconazole shampoo. Cytologic and histopathologic examinations of the lesions revealed intra-epidermal pustules with predominantly neutrophils and acantholytic cells. This is the first description of putative contact ketoconazole shampoo-triggered pemphigus foliaceus in a dog.

Multiple oral dosing of ketoconazole increases dog exposure to ivermectin.

PURPOSE: The parasiticide ivermectin and the antimicrobial drug ketoconazole are macrolides that interact with P-glycoprotein. We investigated the effects of ketoconazole at a clinical dose on the pharmacokinetics of ivermectin, a CYP3A substrate with low hepatic clearance. METHODS: Beagle dogs received a single subcutaneous injection of ivermectin at 0.05 mg/kg alone (n=6) or in combination with a daily oral dose of ketoconazole 10 mg/kg over 5 days before and after ivermectin administration (n=6). The plasma kinetics of ivermectin and its metabolite were followed over 15 days by HPLC analys…
