Flexible nephroscopy while using carbon dioxide to insufflate the collecting system has been described as a "good trick" to remove caliceal stones during laparoscopic pyelolithotomy. We found this true only for those calices with undisturbed urine drain into the renal pelvis, while it provided suboptimal view of stones that were located in dependent lower pole calices. We describe how we dealt successfully with a group of lower pole caliceal stones during laparoscopic pyeloplasty.

In a series of experiments with pigs the completely obstructed renal pelvis was filled with Ringer's lactate at a constant rate of 2 ml/min. At various pressure levels lower than 50 mm Hg the relations pressure vs infused volume and pressure vs actual intrapelvic volume were repeatedly measured. It was concluded that the measurements were reproducible and that the pelvicalyceal system is -under these experimental conditions- a stable elastic unit.

The following case describes the clinical course of a patient with bilateral infundibulopelvic stenosis from her initial presentation at age 2 through the age of 14 years. A 54-year-old women reported recurrent pain in one side. Radiological and ultrasonographic examination revealed a mass in the left renal pelvis and the lower calyx. With percutaneous calycopyeloscopy keratinous fragments of a cholesteatoma were found and extracted during the same procedure. Up to now, malignant degeneration of a cholesteatoma has never been reported in the literature. Therefore, a conservative surgical treatment is acceptable.

Percutaneous removal of renal stones is becoming an established procedure, especially for stones lying free in the renal pelvis. However, some renal stones, particularly caliceal stones, are less accessible and require special techniques for removal. Uretero calicostomy is a valuable technique in patients with uretero-pelvic obstruction, if a reconstruction to obtain anatomical drainage cannot be achieved. Ten cases of inferior and one of superior uretero-calicostomy, in a total of ten patients, are reported. The operation was performed eight times as a primary and three times as a secondary procedure. Good drainage of the kidney was achieved in eight of the 11 cases. The indication for ureterocalicostomy is discussed. It is rare especially in cases of malignancy.

Upper ureteral stricture is a relatively rare but increasingly encountered condition in clinical practice. Percutaneous nephrolithotomy - PCNL - was made possible by technological progress, with the introduction of nephroscopes, lithotomy forceps, and mechanical, ultrasonic or electrohydraulic lithotriptors. The procedure includes four stages. First needle puncture of the inferior or middle calyx. Secondly, dilatation of the pathway and creation of a tunnel. Thirdly, investigation of the collecting system. And, finally, extraction of the stone. The results of 302 published cases show a success rate of 87 per cent, a complete failure rate of 13 per cent, and residual stones in 10 per cent of cases.

25 nephrectomized kidneys of patients with vesicoureteral reflux (VUR) were examined during the period 1977 to 1985. In 13 cases, VUR was associated with other anomalies of the urinary tract. 14 patients were children aged 6 months to 15 years. The oldest of the 11 adult patients was 50 years old.
