[A new case of tibial stress fracture as a complication of knee osteoarthritis].

PURPOSE OF STUDY: Transverse stress fractures of the proximal tibia are associated with osteoarthritis of the knee. This uncommon complication is presented in this case report as a non-union form along with a literature review. CASE REPORT: A fifty nine year old black African woman who had a thirteen-year history of osteoarthritis in both knees, was seen for increasing pain in the proximal part of the left leg without demonstrative traumatism. This homemaker measured 155 cm for 85 kgs. Clinical and radiological examination revealed a non-united stress fracture of the proximal part of the tibi

[Damage to the knee ligament as a concomitant injury in femoral shaft fractures].

Knee ligament injuries associated with femoral shaft fractures can severely influence the functional results of treatment. In a follow-up of 59 operatively treated femoral shaft fractures 18.6% of the patients revealed ligamentous damages. In all cases one or both of the cruciate ligaments were injured. Especially polytraumatized car front-seat passengers are at higher risk with 30.8% ligament ruptures. While only 61.6% of the ligament injuries were primarily diagnosed, the testing of knee joint stability after osteosynthesis of a femoral fracture is essential. Furthermore arthroscopy should b

Classification and Outcomes of 214 Periprosthetic Patellar Fractures in Primary Total Knee Arthroplasty.

BACKGROUND: Long-term data following periprosthetic patellar fractures around primary total knee arthroplasty (TKA) are lacking. The purpose of this study was to describe the classification, management, and outcomes of periprosthetic patellar fractures in, to our knowledge, the largest series to date. METHODS: We identified 214 (213 cemented and one uncemented) periprosthetic patella fractures (205 patients) around a primary TKA sustained between 1989 and 2020. There were eight intraoperative fractures and 206 postoperative fractures. Postoperative fractures were classified according to the O

Is Severe Knee Osteoarthritis Associated with Intertrochanteric Rather Than Femoral Neck Hip Fractures?

We have not identified any research in the literature that explores the severity of knee osteoarthritis in relation to the type of hip fracture. In our study, we examined the severity of knee osteoarthritis in patients with hip fractures affecting the intertrochanteric (Group 1) and femoral neck region (Group 2). In our study, patients over 50 years of age were analyzed and Kellgren-Lawrence classification was applied to bilateral knee radiographs. The correlation between the severity of knee osteoarthritis and the classification of hip fractures was analyzed, and subgroups were compared. The

Anterior femoral notching during total knee arthroplasty: a predictor of periprosthetic femoral fractures?

INTRODUCTION: Anterior femoral notching during TKA is thought to weaken the distal femur, potentially increasing periprosthetic fracture risk, but its clinical impact remains debated. This study examines whether notching independently predicts such fractures and assesses related clinical outcomes using the Tayside classification system for cortical violations. METHODS: A retrospective cohort study was conducted on 715 patients who underwent primary TKA. Patients were divided into Group 1 without notching (control group) and Group 2 with any presence of notching according to Tayside classifica

High Prevalence of Symptomatic Knee Osteoarthritis Among Patients Who Have Fragility Hip Fractures.

BACKGROUND: Knee osteoarthritis (OA) is a common degenerative musculoskeletal condition that impairs mobility and balance, increasing fall risk. When combined with osteoporosis, it further increases the risk of fragility fractures. Despite its prevalence, the frequency of knee OA in patients who have fragility hip fractures (FHFs) is not well established. This study aimed to determine the prevalence of knee OA among FHF patients. METHODS: We conducted a cross-sectional analysis of patients who underwent surgical treatment for FHF. The knee OA diagnoses followed the American College of Rheumat

High-energy fractures of the tibial plateau. Knee function after longer follow-up.

BACKGROUND: Studies of the long-term outcomes of treatment of fractures of the tibial plateau have included wide mixtures of fracture types and mostly low-energy split and split-depression fractures. The long-term results of treatment of high-energy intra-articular proximal tibial fractures are unknown. The purpose of this study was to assess the function of the knee and the development of arthrosis at a minimum of five years after injury in a consecutive series of patients in whom a high-energy fracture of the tibial plateau had been treated with a uniform technique of external fixation. Supracondylar fractures of the femur above total knee arthroplasties with cortical allograft struts.

Ten supracondylar fractures of the femur above a total knee arthroplasty were treated with open reduction and internal fixation using a metal plate and cortical allograft struts. Failure with nonunion of previous operative treatments was seen in 5 patients. The other 5 patients had either severe osteopenia or comminution of the fracture above the knee prosthesis. At an average of 38 months' follow-up, all the fractures healed with an average union time of 17.6 weeks. The postoperative knee alignment averaged 5.1 degrees valgus. Infection occurred in 1 patient, which resulted in a poor outcome.

Avulsion fractures and chronic avulsion injuries of the knee: role of MR imaging.

Avulsion fractures and chronic avulsion injuries of the knee are common lesions in sports-related trauma, especially among adolescents. Magnetic resonance imaging may prove useful in detecting and characterizing such lesions, and has several advantages with regard to other imaging modalities. We review, illustrate, and discuss the MR imaging features of some of the more frequent avulsion fractures and chronic avulsion injuries of the knee, including avulsion fractures of the cruciate ligaments, avulsion fractures of lateral and medial stabilizers, avulsion fractures and chronic avulsion injuri

Techniques for Fixing Anterior Cruciate Ligament Tibial Avulsion Fractures in Multiligament Knee Injuries.

BACKGROUND: In patients with multiligament knee injuries, anterior cruciate ligament (ACL) tears are often reconstructed. Recent studies have shown good results when the ACL tibial avulsions are repaired. The advantages of ACL tibial avulsion repair are the preservation of the native anatomy, reduction in donor site morbidity, and lower risk of tunnel convergence. INDICATIONS & TECHNIQUE DESCRIPTION: We show 2 techniques for repairing ACL tibial avulsion fractures. The first case describes the use of hybrid fixation (screw and suture), with staged repair and reconstruction in a patient with h

Open reduction and polyaxial plating for stemmed knee periprosthetic fractures: A case series.

INTRODUCTION: Stemmed total knee arthroplasty (STKA) periprosthetic fractures (PPFs) are an emerging problem affecting frail patients. Their surgical fixation is challenging, due to intramedullary involvement and poor bone stock. Polyaxial locking plating has yielded good results in implant-related femur fractures. We hypothesized that this treatment would provide similar results for STKA PPFs. METHODS: Retrospective analysis of consecutive patients with a femoral PPF or inter-implant fracture around a knee revision stem who had undergone open reduction and periprosthetic-specific polyaxial p

Sister, Sister! Siblings With Simultaneous Early Femoral Insufficiency Fractures After Total Knee Arthroplasty.

Early periprosthetic fractures after total knee arthroplasty are rare but devastating complications which require revision surgery and lead to poor patient satisfaction. We present 2 siblings who underwent primary total knee arthroplasty on the same day and then both presented 2 weeks after surgery with atraumatic periprosthetic femur fractures. The first patient underwent revision for a cemented distal femoral replacement with stemmed tibial fixation. The second patient underwent an isolated femoral component revision with a stemmed femoral component and distal augment. Histological evaluatio

Periprosthetic Tibial Fractures After Total Knee Arthroplasty: Early and Long-Term Clinical Outcomes.

BACKGROUND: Although periprosthetic fractures are increasing in prevalence, evidence-based guidelines for the optimal treatment of periprosthetic tibial fractures (PTx) are lacking. Thus, the purpose of this study is to assess the clinical outcomes in PTx after a total knee arthroplasty (TKA) which were treated with different treatment options. METHODS: A retrospective review was performed on a consecutive series of 34 nontumor patients treated at 2 academic institutions who experienced a PTx after TKA (2008-2016). Felix classification was used to classify fractures (Felix = I-II-III; subgrou

Distal Femoral Replacement for Periprosthetic Fractures around Total Knee Arthroplasty: When and How?

Periprosthetic fractures around total knee arthroplasty have become more common with the increasing incidence of total knee arthroplasty. These fractures are challenging to treat, especially in older patients with poor bone stock. Although open reduction internal fixation remains the most common treatment, revision with distal femoral replacement has become popular in recent years. Careful evaluation and planning are required to have successful outcomes. We present a review of distal femoral replacement in periprosthetic fractures around the total knee arthroplasty, along with surgical techniq

Outcomes of fixation for periprosthetic tibia fractures around and below total knee arthroplasty.

INTRODUCTION: The incidence of periprosthetic fractures after total knee arthroplasty (TKA) is rising due to an increasing number of TKAs performed annually and the growing elderly population. Like periprosthetic fractures of the distal femur, periprosthetic tibia fractures are primarily treated with operative fixation; however, there is limited scientific literature that has reported outcomes of periprosthetic tibia fractures treated with modern plating techniques. To our knowledge, this is the largest series of non-intraoperative periprosthetic tibia fractures treated with open reduction int
