Adult Scheuermann kyphosis: evaluation, management, and new developments.

Scheuermann kyphosis is a structural hyperkyphosis defined radiographically as anterior wedging of ≥5° of at least three consecutive vertebral bodies. Typically, the disease develops during adolescence but may not present until adulthood. The etiology remains unknown. Indications for management include progressive deformity, pain, cosmesis, neurologic deficit, and cardiopulmonary compromise. Surgical intervention is indicated in patients with persistent pain and unacceptable deformity caused by significant kyphosis. Surgery can be performed through posterior-only, anterior-only, or combined an…

Direct internal kyphectomy for severe angular tuberculous kyphosis.

We describe a direct internal kyphectomy through a modified costotransversectomy, an extrapleural approach to the kyphus that does not jeopa … reated five patients with paraparesis of healed disease with this approach. Preoperatively the mean kyphosis was 114 degrees. Neurological improvement was obtained in two patients. At a mean followup of 5 years, solid anterior fusion was achieved in four patients. One patient died 5 months after surgery because of chest infection.

Cervical kyphosis in patients who have Larsen syndrome.

Four patients who had Larsen syndrome and cervical kyphosis were managed operatively and followed for an average of seventy months (range, forty to ninety-two months). The preoperative cervical kyphosis ranged from 35 to 65 degrees. The patients had had a posterior cervical arthrodesis alone when they were infants, at an average age fo fourteen months (range, ten to sixteen months). In three infants, the kyphosis either stabilized (one patient) or reversed into lordosis (two patients). Thus, the kyphosis corrected gradually by continued anterior growth in the presence of a solid posterior fusi…

Oligosegmental correction of post-traumatic thoracolumbar angular kyphosis.

Seventeen patients with rigid throacolumbar angular kyphosis due to neglected fractures or dislocations were treated by a standardized single-stage monosegmental or bisegmental anterior discectomy and posterior closing extension wedge osteotomy. The two- or three-level reduction-fixation (RF) instrumentation was used posteriorly for correction and fixation. Choosing angled pedicle screws according to preoperative measurements, the method can always correct the kyphosis to the approximate sagittal curvature that is planned to create preoperatively. The average precorrection of thoracolumbar kyp…

Scheuermann kyphosis. Follow-up of Milwaukee-brace treatment.

From 1960 through 1978, a total of 274 patients who had a diagnosis of Scheuermann kyphosis were treated with a Milwaukee brace at the Twin Cities Scoliosis Center. We analyzed the long-term results in 120 patients who had used the Milwaukee brace and had been followed for at least five years after the completion of treatment. The average age of the patients at the initiation of treatment was twelve years and five months, the average age at the completion of treatment was sixteen years and one month, and the average age at the last follow-up examination was twenty-four years. Of the patients w…

Two hereditary spinal diseases producing kyphosis during adolescence.

Familial accumulation of spinal osteochondrosis (Scheuermann's disease) and hereditary juvenile anterior fusion of the vertebral bodies in the thoraco-lumbal area are reported for the first time in the same family. Radiological examination of the spine in 2 planes of 73 persons formed the basis for the study. Nine cases of spinal osteochondrosis and 5 cases of hereditary juvenile anterior fusion were found. Proliferation and increased height of the anterior surface of the body of the vertebrae and pronounced reduction in the anterior aspect of the intervertebral spaces were characteristic duri

Novel radiological predictors for the progression of proximal junctional kyphosis in osteoporotic vertebral compression fracture with kyphosis following posterior corrective surgery.

OBJECTIVE: This study aimed to identify the effect of some novel risk factors associated with L1 vertebrae and parameters closely related to the sagittal alignment for the occurrence of proximal junctional kyphosis (PJK) following surgery for patients with osteoporotic vertebral compression fractures (OVCF) kyphosis. METHODS: 74 OVCF patients undergoing posterior corrective surgery between January 2008 and June 2021 with a minimum 2-year follow-up were included. These patients were divided into PJK and non-PJK groups. Spinopelvic parameters, including thoracolumbar slope (TLS) and the L1 plum…

Operative choices matter: the role of UIV and sagittal balance in the development of proximal junctional kyphosis following posterior instrumentation for Scheuermann's kyphosis.

PURPOSE: This study sought to investigate associations between upper instrumented vertebra (UIV) location and the risk of proximal junctional kyphosis (PJK) at 2 years following posterior spinal fusion (PSF) for Scheuermann's kyphosis (SK). METHODS: In this retrospective cohort study, SK patients who underwent PSF and reached 2 years postop were identified in a multicenter international registry, excluding those with anterior release, prior spine surgery, neuromuscular comorbidity, post-traumatic kyphosis, or kyphosis apex below T11-T12. Location of UIV as well as the number of levels between…

Proximal junctional kyphosis in adult reconstructive spine surgery results from incomplete restoration of the lumbar lordosis relative to the magnitude of the thoracic kyphosis.

BACKGROUND: PROXIMAL JUNCTIONAL KYPHOSIS (PJK) IS DEFINED AS: 1) Proximal junction sagittal Cobb angle >≥10°, and 2) Proximal junction sagittal Cobb angle of at least 10° greater than the pre-operative measurement PJK is a common complication which develops in 39% of adults following surgery for spinal deformity. The pathogenesis, risk factors and prevention of this complication are unclear. METHODS: Of 54 consecutive adults treated with spinal deformity surgery (age≥59.3±10.1 years), 19 of 54 (35%) developed PJK. The average follow-up was 26.8 months (range 12 - 42). Radiographic parameters …

The Effects of Corrective Exercises and Respiratory Biofeedback on the Kyphosis Angle and Electrical Activity of Trapezius Muscles in Male Adolescents With Kyphosis: A Randomized Controlled Trial.

BACKGROUND AND AIMS: Thoracic kyphosis is a prevalent postural abnormality among adolescents, which can impair musculoskeletal alignment and affect neuromuscular function. Despite the known benefits of corrective exercises, limited attention has been given to the role of respiratory biofeedback. This study aimed to investigate the effects of corrective exercises and respiratory biofeedback on the kyphosis angle and the electrical activity of the trapezius muscles in male adolescents with kyphosis. METHODS: A randomized clinical trial was conducted on 45 male adolescents with postural kyphosis…

How Can We Prevent Postoperative Kyphosis in Cervical Laminoplasty?

Background and Objectives: This study aimed to evaluate changes in cervical sagittal alignment after open-door laminoplasty and identify any specific preventable risk factors associated with postoperative kyphotic deformity. Materials and Methods: We retrospectively reviewed patients who underwent open-door laminoplasty for degenerative cervical stenosis between 2018 and 2021. Radiological assessment included pre- and postoperative C2-C7 Cobb angles, cervical alignment categories (lordosis, straight, sigmoid, kyphosis), and K-line status. Early postoperative CT scans were analyzed for lamina f…

Cervicothoracic Kyphosis and Spinal Cord Compression in Hurler Syndrome.

INTRODUCTION: Hurler syndrome is characterized by significant spinal abnormalities, including cervical instability, odontoid dysplasia, and spinal cord compression. Spinal compression and kyphosis at the cervicothoracic (C-T) junction are also observed but less commonly reported. The present study was conducted to determine the prevalence of C-T kyphosis and spinal cord compression at the C-T junction in patients with Hurler syndrome. In addition, we aimed to evaluate the significance of C-T kyphosis and spinal cord compression in these patients in terms of clinical symptoms or neuromonitoring…

Correlation between Degenerative Thoracolumbar Kyphosis and Lumbar Posterior Muscle.

UNLABELLED: The relationship between spinal alignment, particularly degenerative thoracolumbar kyphosis (DTLK) combined with lumbar spine stenosis (LSS), and paraspinal muscle content remains underexplored. This study aimed to elucidate the characteristics of paraspinal muscle distribution in DTLK patients and its association with lumbar lordosis (LL) and body mass index (BMI). METHODS: A case-control study was conducted comparing 126 patients with DTLK and LSS against 87 control patients. The lumbar crossing indentation value (LCIV) was introduced as a novel measurement for paraspinal muscle…

Kyphosis After Thoracolumbar Spine Fractures: WFNS Spine Committee Recommendations.

Thoracolumbar fractures change the biomechanics of the spine. Load distribution causes kyphosis by the time. Treatment of posttraumatic kyphosis is still controversial. We reviewed the literature between 2010 and 2020 using a search with keywords "thoracolumbar fracture and kyphosis." We removed osteoporotic fractures, ankylosing spondylitis fractures, non-English language papers, case reports, and low-quality case series. Up-to-date information on posttraumatic kyphosis management was reviewed to reach an agreement in a consensus meeting of the World Federation of Neurosurgical Societies (WFN…
