Prolonged second stage of labor and risk of postpartum hemorrhage in nullipara with epidural anesthesia and vaginal delivery: A cohort study with propensity score analysis.

OBJECTIVE: To conduct an analysis using propensity score methods, exploring the association between a prolonged second stage (>3 h) and the risk of postpartum hemorrhage (PPH) in a diverse population. METHODS: We conducted a prospective cohort study involving nullipara with epidural anesthesia and vaginal delivery, aged ≥18 years, presenting cephalically, and with a gestational age (GA) of ≥24 weeks at a tertiary maternity hospital in China (chictr.org.cn identifier: ChiCTR2200063094). Women undergoing emergency cesarean section in labor were excluded. The primary outcome was PPH, with second

Cervical dilation at the time of epidural catheter insertion is not associated with the degree of prolongation of the first or second stages of labor, or the rate of instrumental vaginal delivery.

INTRODUCTION: Epidural analgesia (EA) is an established option for efficient intrapartum analgesia. Meta-analyses have shown that EA differentially affects the first stage of labor but prolongs the second. The question of EA timing remains open. We aimed to investigate whether EA prolongs delivery in total and whether the EA administration timing vis-à-vis cervical dilation at catheter insertion is associated with a modulation of its effects on the duration of the first and second stages, as well as the rate of instrumental vaginal delivery in primiparas and multiparas. MATERIAL AND METHODS:

New technique for automatic sonographic measurement of change in head-perineum distance and angle of progression during active phase of second stage of labor.

OBJECTIVE: To evaluate the performance of a new ultrasound technique for the automatic assessment of the change in head-perineum distance (delta-HPD) and angle of progression (delta-AoP) during the active phase of the second stage of labor. METHODS: This was a prospective observational cohort study including singleton term pregnancies with fetuses in cephalic presentation during the active phase of the second stage of labor. In each patient, two videoclips of 10 s each were acquired transperineally, one in the axial and one in the sagittal plane, between rest and the acme of an expulsive effo

[Effects of the combined therapy of the auricular-point pressure at the free position and the unprotected perineal delivery technique during the second stage of labor in the primiparas].

OBJECTIVE: To observe the effects of the combined therapy of the auricular-point pressure at the free position and the unprotected perineal delivery technique during the 2nd stage of labor in the primiparas so as to improve the clinical delivery quality. METHODS: A total of 146 primiparas who accepted the natural delivery willingly were collected. According to the visit sequence, they were divided into an observation group (72 cases) and a control group (74 cases). The free position and the unprotected perineal delivery were adopted in combination during the labor in both of the two groups. A

Effect of intermittent versus continuous bladder catheterization on duration of the second stage of labor among nulliparous women with an epidural: a randomized controlled trial.

INTRODUCTION AND HYPOTHESIS: Catheterization type among women laboring with epidural analgesia who develop bladder retention has been reported to affect labor duration and mode of delivery. We aimed to compare the effect of continuous bladder catheterization (CC) with that of intermittent bladder catheterization (IC) on the duration of the second stage of labor. METHODS: In a randomized trial, term nulliparous women with singleton gestation who requested epidural analgesia and were unable to void spontaneously were eligible and randomized to either CC or IC. Epidural analgesia was maintained

A randomized trial of the effects of coached vs uncoached maternal pushing during the second stage of labor on postpartum pelvic floor structure and function.

OBJECTIVE: The purpose of this study was to determine if refraining from coached pushing during the second stage of labor affects postpartum urogynecologic measures of pelvic floor structure and function. STUDY DESIGN: Nulliparous women at term were randomized to coached (n = 67) vs uncoached (n = 61) pushing. At 3 months' postpartum women underwent urodynamic testing, pelvic organ prolapse examination (POPQ), and pelvic floor neuromuscular assessment. RESULTS: Urodynamic testing revealed decreased bladder capacity (427 mL vs 482 mL, P = .051) and decreased first urge to void (160 mL vs 202

Epidural Analgesia and Its Impact on the Duration of the Second Stage of Labor, Vacuum Extraction Delivery, and Neonatal Apgar Scores: A Retrospective Cohort Study.

Background and Objectives: Epidural analgesia is widely used for intrapartum pain relief; nonetheless, its associations with labor progression and neonatal outcomes remain incompletely characterized in diverse obstetric populations. This retrospective cohort study aimed to investigate the association between labor epidural analgesia and obstetric outcomes, with a focus on the duration of the second stage of labor and the vacuum extraction delivery (VED) rate. Materials and Methods: Maternal and neonatal outcomes were compared between 173 women without epidural analgesia and 386 women with epid

Correlation between clinical and sonographic station during the second stage of labor: Which role for the occiput position? secondary analysis of a prospective dataset.

BACKGROUND: The assessment of the occiput position and station of the fetal head are required for managing the second stage of labor. Clinical assessment has shown limitations in terms of accuracy and reproducibility, and previous sonographic data have demonstrated a different pattern of head descent in the event of occiput posterior compared to non-occiput posterior position. OBJECTIVE: To evaluate the relation between the position of the fetal occiput and the clinical and sonographic assessment of fetal head station during the second stage of labor. METHODS: Single center prospective obser

Just go with your body? A conversation analytic study of the transition from first to second stage of labor in UK midwife-led care.

BACKGROUND: The transition from first to second stage of labor is poorly understood. While the onset of second stage is defined by cervical measurement, dilation cannot be directly sensed or externally observed. Thus, uncertainty exists when women report pushing urges before dilation is confirmed. This study aimed to explore how sensations of pushing and uncertainty over progress are interactionally managed. METHODS: We audio/video recorded the labors of 37 women in two midwife-led units in England. Our analysis focused on a subset of 28 recordings that featured discussion of transition from

Moderate or intensive management of the active phase of second-stage labor and risk of urinary and anal incontinence: results of the PASST randomized controlled trial.

BACKGROUND: Incontinence occurs frequently in the postpartum period. Several theoretical pathophysiological models may underlie the hypothesis that different types of management of the active phase of the second stage of labor have different effects on pelvic floor muscles and thus perhaps affect urinary and anal continence. OBJECTIVE: This study aimed to evaluate the impact of "moderate pushing" on the occurrence of urinary or anal incontinence compared with "intensive pushing," and to determine the factors associated with incontinence at 6 months postpartum. STUDY DESIGN: This was a planne

Estimation of Perineal Length in First and Second Stage of Labor Among South Indian Women and Its Association With Perineal Tears-A Prospective Observational Study.

AIM: To estimate average perineal length during first and second stages of labor and its association with composite perineal tears. METHODS: A prospective observational study was conducted at a tertiary institute in Southern India. Perineal length was measured using a sterile flexible tape; an episiotomy was made when the perineum appeared stretched and thinned out. Composite tears and obstetric anal sphincter injuries (OASI) were detected clinically. Incidence of tears and their association with perineal length was analyzed using a chi-squared test and log binomial regression was used to est

Visual Biofeedback (VB) through Trans-Perineal Ultrasound (TPU) during the active second stage of labor to improve maternal childbirth satisfaction: protocol of a randomized controlled trial.

BACKGROUND: Childbirth can be psychologically traumatic for up to 44% of women. Determinants of a negative childbirth experience include loss of control and lack of emotional support or interaction with obstetric caregivers. Vaginal childbirth also predisposes to somatic trauma, including trauma to the pelvic floor and perineum, which may lead to pelvic floor dysfunctions (PFDs). Providing Visual Biofeedback through Trans-Perineal Ultrasound (VB-TPU) during the expulsive phase of labor has been shown to increase efficacy of pushing efforts and may reduce perineal trauma. The primary aim of thi

Efficacy of an Ergonomic Ankle Support Aid for Squatting Position in Improving Pushing Skills and Birth Outcomes During the Second Stage of Labor: A Randomized Controlled Trial.

BACKGROUND: The physical positions that are adopted by women during childbirth significantly impact their childbirth outcomes and experiences. Literature studies have associated using a squatting position with reduced childbirth pain and increased comfort and pushing efficiency. However, the major disadvantage of the squatting position is that women may lack the muscular fitness and stamina necessary to sustain this position for a long period. PURPOSE: The aim of this study was to compare the pushing experiences and birth outcomes of three different pushing positions during the second stage o
