Smartphone capture of flexible laryngoscopy: Optics, subsite visualization, and patient satisfaction.

OBJECTIVE: To determine whether a smartphone adaptor can record laryngoscopic videos of adequate quality for clinical diagnosis and communication among otolaryngologists and assess the impact of recorded exams on patient satisfaction. METHODS: Twenty adult inpatients undergoing flexible laryngoscopy in a tertiary care medical center were prospectively enrolled. Each subject's larynx was visualized with the standard laryngoscope eyepiece and with an attached mobile phone adaptor with video recording capabilities. A 5-point Likert scale was used by the resident performing the scope to grade the

Vertebral artery dissection after direct laryngoscopy: case report and literature review.

OBJECTIVE: We report a case of vertebral artery dissection following direct laryngoscopy, and we provide an update on current knowledge regarding this condition and its relationship to movements of the neck. METHOD: A case report and review of the world literature are presented. RESULTS: Vertebral artery dissection is an uncommon event leading to stroke. It has been associated with many risk factors, particularly extreme movement of the cervical spine. The pathogenesis of the condition and the true aetiological significance of neck movement are not known. CONCLUSION: To our knowledge, we pr

Voice changes and thyroid surgery: is pre-operative indirect laryngoscopy necessary?

BACKGROUND: Indirect laryngoscopy (IDL) is often performed prior to thyroid surgery to detect pre-existing vocal cord pathology. METHODS: A retrospective chart review of 201 patients undergoing thyroid surgery at the Prince of Wales Hospital was undertaken in order to study the patterns of pre-operative and postoperative voice changes and IDL findings. RESULTS: A total of 9% of patients had pre-operative voice symptoms, and 22% of this group had abnormalities detected on pre-operative IDL. Of 160 documented IDL, 4% revealed vocal cord pathology in asymptomatic patients, including an asymptom

Vision-Based Assistance for Vocal Fold Identification in Laryngoscopy with Knowledge Distillation.

Laryngoscopy images play a vital role in merging computer vision and otorhinolaryngology research. However, limited studies offer laryngeal datasets for comparative evaluation. Hence, this study introduces a novel dataset focusing on vocal fold images. Additionally, we propose a lightweight network utilizing knowledge distillation, with our student model achieving around 98.4% accuracy-comparable to the original EfficientNetB1 while reducing model weights by up to 88%. We also present an AI-assisted smartphone solution, enabling a portable and intelligent laryngoscopy system that aids laryngos

The Clinical Utility of Continuous Laryngoscopy During Exercise: A Report of Two Cases.

Exertional dyspnea is a common and disabling symptom in otherwise healthy children and adolescents, as well as in children with baseline airway abnormalities. It impairs the quality of life and may be associated with fatigue and underperformance in sports. Exertional dyspnea can be caused by a wide variety of structural and psychogenic causes. Exercise-induced laryngeal obstruction (EILO) is a relatively prevalent entity in young people that usually presents with exertional stridor, coughing, and dyspnea caused by transient closure of the larynx. In more complex cases where conventional tests

Tracheal intubation in microgravity: a simulation study comparing direct laryngoscopy and videolaryngoscopy†.

BACKGROUND: The risk of severe medical and surgical events during long-duration spaceflight is significant. In space, many environmental and psychological factors may make tracheal intubation more difficult than on Earth. We hypothesised that, in microgravity, tracheal intubation may be facilitated by the use of a videolaryngoscope compared with direct laryngoscopy. METHODS: In a non-randomised, controlled, cross-over simulation study, we compared intubation performance of novice operators and experts, using either a direct laryngoscope or a videolaryngoscope, in weightlessness and in normogr

Direct laryngoscopy assisted fiberoptic intubation: A novel technique for the pediatric airway.

OBJECTIVE: To introduce a novel intubation technique for difficult pediatric airways. METHODS: This two-provider technique requires a direct laryngoscope and a flexible fiberoptic laryngoscope. One provider performs direct laryngoscopy which allows for introduction of the flexible laryngoscope. The second provider inserts the flexible laryngoscope with the endotracheal tube loaded, through the oropharynx in to the subglottis. RESULTS: We report three pediatric patients that were initially unable to be intubated by conventional methods. We were ultimately able to successfully intubate these p

Routine Preoperative Laryngoscopy for Thyroid Surgery Is Not Necessary Without Risk Factors.

Background: Routine preoperative vocal cord (VC) assessment with laryngoscopy in patients undergoing thyroidectomy allows clear documentation of baseline VC function, aids in surgical planning in patients with palsies, and facilitates interpretation of intraoperative neuromonitoring (IONM) findings. We aimed to determine the incidence of preoperative vocal cord palsy (VCP); to evaluate the associated risk factors for preoperative VCP; and to calculate the cost-savings potential of implementing a selective approach. Methods: Patients with a pre-thyroidectomy VC assessment by fiberoptic laryngos

Assessment of cervical spine movement during laryngoscopy with Macintosh and Truview laryngoscopes.

BACKGROUND: Truview laryngoscope provides an indirect view of the glottis and will cause less cervical spine movement since a ventral lifting force will not be required to visualize the glottis compared to Macintosh laryngoscope. MATERIALS AND METHODS: A randomized crossover study to assess the degree of movement of cervical spine during endotracheal intubation with Truview laryngoscope was conducted in 25 adult ASA-I patients. After a standard anesthetic technique laryngoscopy was performed twice in each patient using in turn both the Macintosh and Truview laryngoscopes. A baseline radiograp

A comparison of the forces applied to a manikin during laryngoscopy with the GlideScope and Macintosh laryngoscopes.

The force applied during laryngoscopy can cause local tissue trauma and can induce cardiovascular responses and cervical spine movement in susceptible patients. Previous studies have identified numerous operator and patient factors that influence the amount of force applied during intubation. There are few studies evaluating the effect of different laryngoscope blades and no study involving video laryngoscopes. In this study we measured the forces using two laryngoscopic techniques. Three FlexiForce Sensors (A201-25, Tekscan, Boston, MA, USA) were attached to the concave blade surface of a Mac
