Larynx osteosarcoma: case report.

We present the case of a laryngeal osteosarcoma in a 69-year-old man, which generated many diagnostic and treatment difficulties. The patient was admitted to the hospital because of persisting hoarseness and a laryngeal tumor was seen on laryngoscopy. Total laryngectomy was performed and the pathological examination of the resected material showed osteosarcoma of the larynx. Postoperative radiotherapy was planned but the patient declined any treatment. He was readmitted to the hospital 5 months later with a neck tumor and enlarged lymph nodes on the right side of the neck. Chemotherapy started…

[Larynx preservation: nonsurgical approaches].

The question of larynx preservation is central in the management of patients with a carcinoma of the larynx or hypopharynx, especially to preserve the main functions of the larynx. In early stages (T1-earlyT2) Larynx preservation can generally be obtained with partial surgery or radiotherapy. Some other approaches such as exclusive chemotherapy require further investigations. In locally advanced and infiltrating larynx/hypopharynx carcinomas, (advancedT2-T3), several ways have been used to preserve the larynx including exclusive radiotherapy which can be improved by modified fractionation and …

Solitary extramedullary plasmacytoma - larynx.

Solitary Plasmacytoma of the larynx is a rare tumor with male predominance and most patients are between 50-70 years of age. The paper reports a case of Solitary Extramedullary' Ipasmacytoma. (EMP) ljirynx in subglottic region, in a 65-year-old man successfully treated with complete response to radiotherapy.

[CBO guideline 'larynx carcinoma'].

The guideline 'Larynx carcinoma' aims at preventing divergent views on the treatment of different stages of larynx carcinoma between the major referral centres. Strict criteria are proposed for the radiological diagnostic procedures and the pathology report in order to accurately classify the patient in the appropriate prognostic category. Taking into consideration possible subsequent treatments, surgery--preferably by laser--is proposed for severe dysplasia and carcinoma in situ. For all other stages the treatment of choice is radiotherapy as a larynx conserving therapy. Only in cases of mass…

Larynx cancer risk factors.

The larynx cancer relative risk has been evaluated (328 cases) in the stationary-native Upper Silesians (63.7%) and migrating (36.3%) male population. The particular control groups (656 men) included 418 and 238 men respectively not suffering from malignant neoplasms. The essentially higher larynx cancer incidence risk has been shown for men manual labor and exposed to the influence of various dusts, gases and vapors and other pollutants in their place of work. The increased substantial incidence risk is also true for men who smoke tobacco and are professionally exposed to the pollution. The r…

Prognostic factors for survival and nonfunctional larynx in patients with squamous cell carcinoma of the larynx.

OBJECTIVES/HYPOTHESIS: Prognostic variables upon presentation can assist in recommending the optimal treatment for patients and may help in management of patient's expectations and side effects. Our objective was to evaluate prognostic factors for survival and nonfunctional larynx in patients with laryngeal squamous cell carcinoma (SCC). STUDY DESIGN: Retrospective chart review. METHODS: All patients diagnosed as having laryngeal SCC from January 2007 through December 2016 in a tertiary, university-affiliated medical center were reviewed. Main outcomes were survival and nonfunctional larynx …

Spindle cell carcinoma of the larynx with a verrucous carcinoma component: combined carcinoma of larynx.

Squamous cell carcinoma (SCC) is the most common neoplasm of the larynx. There are three variants of SCC. Each variant presents distinctive biologic behaviors and clinical courses. Therefore reaching the exact diagnosis and distinguishing the variants is fateful. We describe a case of spindle cell carcinoma of the larynx with a verrucous carcinoma component. To our knowledge, this entity has never been documented before.

Larynx motion considerations in partial larynx volumetric modulated arc therapy for early glottic cancer.

INTRODUCTION: To assess laryngeal motion in early glottic cancer in order to determine safe margins for partial larynx volumetric modulated arc therapy (PL-VMAT), and to quantify dosimetric advantages of PL-VMAT. METHODS: This prospective study included T1-2N0 glottic cancers treated with whole larynx VMAT (WL-VMAT). Pre- and mid-treatment 4D-computed tomography (4D-CT) and dynamic magnetic resonance imaging (MRI) allowed for assessment of larynx swallowing and respiratory motion. For 10 patients with lateralized lesions, PL-VMAT plans were calculated using margins derived from 4D-CT analysis…

The impact of operability status on outcomes in patients with T4 larynx cancer undergoing larynx preservation.

BACKGROUND: Large analyses of T4 larynx cancer (LC) have raised concerns that larynx preservation (LP) contributes to reduced survival compared with laryngectomy (LGX). The role of operability has not been previously considered as a confounder. METHODS: We queried the National Cancer Database for T4M0 LC diagnosed 2004-2015. Patients were categorized as undergoing LGX, chemoradiotherapy but operable (LP-operable), and chemoradiotherapy inoperable (LP-inoperable). Overall survival (OS) was estimated by Kaplan-Meier. Cox multivariate analysis (MVA) identified variables associated with OS. RESU…

Work-associated irritable larynx syndrome.

PURPOSE OF REVIEW: The purpose of this study is to review the relevant literature concerning work-associated irritable larynx syndrome (WILS), a hyperkinetic laryngeal disorder associated with occupational irritant exposure. Clinical symptoms are variable and include dysphonia, cough, dyspnoea and globus pharyngeus. WILS is a clinical diagnosis and can be difficult to differentiate from asthma. Treatment options for WILS include medical and behavioural therapy. RECENT FINDINGS: Laryngeal-centred upper airway symptoms secondary to airborne irritants have been documented in the literature under…

Work-associated irritable larynx syndrome.

BACKGROUND: Work-associated respiratory symptoms may be caused by disorders of both the lower and upper respiratory tract. We propose that occupational exposures may initiate and/or trigger recurrent hyperkinetic laryngeal symptoms, predominantly episodic dyspnoea, dysphonia, cough and sensation of tension in the throat-work-associated irritable larynx syndrome (WILS). AIMS: To examine characteristics of individual and work-related factors that are associated with WILS, occupational asthma (OA) and work-exacerbated asthma (WEA). METHODS: Subjects with WILS, OA and WEA were identified from an…

Granular cell tumor on larynx.

Granular cell tumors (GCTs) are uncommon neoplasm. They can originate in any part of the body. The most common sites of origin are in the head and neck, while the larynx is a relatively uncommon location. Patients affected with a laryngeal GCT typically present with persistent hoarseness, stridor, hemoptysis, dysphagia, and otalgia but, the tumor may be asymptomatic. Care must be taken to differentiate this lesion from others due to the presence of pseudo-epitheliomatous hyperplasia which overlies the GCT and may occasionally mimic squamous cell carcinoma. Therefore, a confirmative diagnosis s…
