Evaluating high dose laxatives via nasogastric tube versus enemas in children with severe constipation: a prospective cohort study.

OBJECTIVES: Children with severe constipation often require disimpaction when standard treatments fail, typically involving high doses of oral laxatives or rectal enemas. Practice guidelines for nurses lack clear directives. This study compared the effectiveness of nasogastric laxatives versus rectal enemas in children aged 0-18 years and aimed to assess the pain and burden experienced by children and parents. METHODS: A prospective cohort study was conducted between December 2018 and June 2022 at a Dutch pediatric outpatient clinic. Of 111 children with severe constipation, 70 participated:

Severe Symptomatic Hypermagnesemia Associated with Over-the-Counter Laxatives in a Patient with Renal Failure and Sigmoid Volvulus.

Hypermagnesemia is an uncommon but a potentially serious clinical condition. Over-the-counter magnesium containing products are widely used as antacids or laxatives. Although generally well tolerated in patients with normal renal function, their unsupervised use in the elderly can result in severe symptomatic hypermagnesemia, especially in those patients with concomitant renal failure and bowel disorders. We report a case of severe symptomatic hypermagnesemia associated with over-the-counter laxatives in a 70-year-old male patient with renal failure and sigmoid volvulus, who was successfully t

[The miraculous effects of taking laxatives. Success and failure of pre-modern medical treatment from the patients' perspective].

The massive use of laxatives, emetics and blood-letting in pre-modern medicine has often intrigued historians. Why did generations of prominent and often quite empirically minded physicians resort to methods which, in hindsight, were usually much more likely to do harm than good? And why did the patients not rebel?Based on patients' personal documents and, in particular, on the letter consultations many of them wrote ot renowned early modern physicians, this essay traces, how the sick and their relatives experienced and responded to this kind of treatment. These sources provide ample evidence,

Laxative use after cardiac arrest: exposure timing, treatment selection, and outcome interpretation.

We commend Duan et al. for examining laxative use after cardiac arrest. However, because follow-up began at return of spontaneous circulation, whereas exposure timing was not reported, the observed association may reflect both immortal time and treatment selection: patients who survived and recovered sufficiently may have been more likely to receive laxatives. A time-varying exposure defined by first administration, with landmark analysis as a sensitivity analysis, would better align treatment and risk time. The reported time-varying-effect model does not estimate the effect of initiating trea

Using supermarket loyalty card data to investigate seasonal variation in laxative purchases in the UK.

While laxatives are designed to manage the symptoms of constipation, they are also known to be misused for weight management, particularly by individuals with eating disorders. This study investigates the relationship between laxative purchases and weight management by examining seasonal trends. Using real-world loyalty card data from a major UK pharmacy retailer spanning December 2013 to December 2014, we analyse self-medication purchasing patterns from 748,375 buyers to explore potential links with weight management behaviours. In pre-registered analysis, we use regression models to investig

Anti-Malignancy and Anti-Inflammation Properties of Emodin, an Essential Constituent in Popular Laxative Botanicals.

The rhubarb root and the senna leaf are two commonly used laxative herbs for functional constipation, containing dianthrones and free anthraquinones. As a free anthraquinone, emodin is a crucial constituent isolated from laxative botanicals, and it is particularly abundant in the rhubarb root, a key ingredient in the classic Chinese medicine formula MaZiRenWan (or herbal medicine CDD-2101). This study explored emodin's anti-malignancy, anti-inflammation, and underlying mechanisms. We examined emodin and other anthraquinones on human colorectal cancer cell proliferation, then used ApcMin/[Formu

Prophylactic laxative use for postoperative constipation in urological cancer: a real-world cohort study.

PURPOSE: To assess the effectiveness of prophylactic laxative use compared to symptom-based management for preventing constipation in patients undergoing surgery for urological cancers. METHODS: Of 2,024 screened patients (aged 18-80) with urological tumors, 1,724 eligible (no severe gastrointestinal disease, non-pregnant, scheduled for surgery) were non-randomly assigned to prevention (prophylactic laxatives, n = 965) or non-prevention groups (symptom-based, n = 748). Propensity score matching with a 1:1 optimal matching strategy was used to minimize baseline differences between the two grou

Treatment of intestinal and liver features in cystic fibrosis mice by the osmotic laxative polyethylene glycol.

BACKGROUND: Cystic Fibrosis (CF) is a genetic disease affecting multiple organs, primarily the lungs and digestive system. Improved pulmonary management significantly improved life expectancy of CF patients. As a result, extrapulmonary manifestations, including gastrointestinal and liver-related symptoms, have become more relevant. We previously reported that the osmotic laxative polyethylene glycol (PEG), which hydrates the CF gut, decreased fecal bile acid loss in a CF knockout mouse model. In the current study we investigated the effect of PEG on intestinal fat and cholesterol absorption an

Evaluations of the in vivo laxative effects of aqueous root extracts of Euclea racemosa L. in mice.

BACKGROUND: Management of constipation with currently available modern medicines is costly and chances of side effects are high. This limits their clinical usefulness and remain to be solved, and calls for investigations of new and better compounds. The experimental plant, Euclea racemosa L. (E. racemosa L) is among plants, which are used for management of constipation traditionally but its effect is not yet experimentally validated. Therefore, the aim of the present study is to investigate the laxative effects of this plant. METHODS: The laxative effects of aqueous root extracts of E. racemo

Post-repair laxative management in obstetric anal sphincter injury guidelines: A narrative review.

BACKGROUND: Childbirth is a common factor which increases the risk of obstetric anal sphincter injuries (OASIS). Damage to the anal sphincters increases the risk of anal incontinence, which has a debilitating impact on the quality of life. Post-repair laxatives are prescribed in this group of women. However, there is no consensus regarding the type or frequency with which they are used, and available guidelines lack consistency and evidence to support the recommendations. AIM: The aim was to review and compare the international, national and local Australian management guidelines for recommen

Unhealthy life habits associated with self-induced vomiting and laxative misuse in Brazilian adolescents.

Adolescence is a stage in life characterized by important social, cognitive, and physical changes. Adolescents are vulnerable to various psychosocial disorders, including eating disorders. We aimed to investigate the association between unhealthy habits, sociodemographic characteristics, and the practice of self-induced vomiting or laxative misuse in a representative sample of Brazilian adolescent girls and boys. Data from 102,072 students who participated in the National Adolescent School-based Health Survey were analyzed using the dependent variable: presence or absence of self-induced vomit

Severe edema after cessation of laxative abuse and use of a loop diuretic: Case report.

Stimulant laxatives are the class of laxatives most often abused by patients with eating disorders. The abrupt cessation of high-dose stimulant laxatives is known to cause edema. We present the case of a patient with anorexia nervosa with binge-purge subtype who was taking ∼100 stimulant laxatives per day. Upon discontinuation of stimulant laxatives, she experienced severe peripheral edema with rapid gain of 11.6 kg over 1 week. Unique features of this case include the high quantity of stimulant laxatives consumed per day, the amount of weight gained due to edema, and the 3 month duration of e

Consuming Genistein Improves Survival Rates in the Absence of Laxative in ΔF508-CF Female Mice.

Genistein is a naturally occurring isoflavone found in soy. Genistein has been shown to increase the open probability of the most common cystic fibrosis (CF) disease-associated mutation, ∆F508-CFTR. Mice homozygous for the ∆F508 mutation are characterized with severe intestinal disease and require constant laxative treatment for survival. This pathology mimics the intestinal obstruction (meconium ileus) seen in some cystic fibrosis patients. This study tested whether dietary supplementation with genistein would reduce the dependence of the ∆F508 CF mouse model on laxatives for survival, thereb

Agarwood induced laxative effects via acetylcholine receptors on loperamide-induced constipation in mice.

Agarwood (Aquilaria sinensis, Aquilaria crasna) is well known as an incense in the oriental region such as Thailand, Taiwan, and Cambodia, and is used as a digestive in traditional medicine. We investigated the laxative effects and mechanism of agarwood leaves extracted with ethanol (EEA-1, Aquilaria sinensis; EEA-2, Aquilaria crasna). EEA-1, EEA-2, the main constituents of EEAs (mangiferin, and genkwanin-5-O-primeveroside), and senna increased the frequency and weight of stools in loperamide-induced constipation model mice. EEA-1 and EEA-2 did not induce diarrhea as a side effect, but senna i

A randomized controlled trial of enemas in combination with oral laxative therapy for children with chronic constipation.

BACKGROUND & AIMS: After 5 years of intensive oral laxative use, up to 30% of constipated children still have an unsuccessful outcome. Children refractory to oral laxatives might benefit from regular rectal evacuation by enemas. This randomized controlled trial compared the effects of additional treatment with rectal enemas (intervention) with conventional treatment alone (oral laxatives, control) in severely constipated children. METHODS: In a tertiary hospital in the Netherlands, 100 children, aged 8-18 years, with functional constipation for at least 2 years were randomly assigned to inter
