[Staghorn renal lithiasis treated with shock waves. Bacteriologic aspects].

Struvite renal stones are caused by infection of the urine with bacteria that synthesize the enzyme urease. Ammonium is released by the breakdown of urea by urease, the urine becomes highly alkaline, and magnesium ammonium phosphate (struvite) and carbonate apatite crystallize. Incorporation of the infecting bacteria within the developing stone, results in a focus of infection that is resistant to conventional antimicrobial therapy, and which is manifested clinically by repeated urinary tract infection caused by persistent bacteriuria. Extracorporeal shock wave lithotripsy (ESWL) currently is

Extracorporeal shock wave lithotripsy for vesical lithiasis: initial experience.

The classical treatment for vesical lithiasis consists of surgery or endoscopic transurethral disintegration. These procedures require anaesthesia and hospitalisation. We report our experience with the Siemens Lithostar in 18 male patients with bladder stones. Complete fragmentation was achieved after a single session in 14 patients and 4 required 2 sessions. A 3-way Foley catheter with saline irrigation was used during treatment. Patients were treated in the prone position followed by immediate evacuation of stone fragments in 9 cases. Morbidity was minimal. Two patients developed impacted fr…

Radiological treatment of common bile duct lithiasis in infancy.

The authors report a series of 10 infants aged from 20 days to 11 months, presenting with CBD lithiasis, explored and cured by radiological procedure. US showed BD dilatation in 9 cases, sludge in the GB in 5 and in the CBD in 2. Histological findings of cholangitis were present in 4 infants. PTC was performed by GB puncture in 6 and BD puncture in 4. There was evidence of a filling defect in CBD but no anatomical anomaly. Blackish concretions were removed through a side-holes catheter or pushed in the duodenum by washing with saline. An external drainage was left a few days to allow control c…

[Results of choledoco-duodenostomy for common bile duct lithiasis].

One hundred and sixty-seven choledoco-duodenostomies for lithiasis of the common bile duct (CBD) were performed. The mean age of the patients was 74 +/- 1 years. Sixty p. 100 of the patients presented with at least one operative risk factor, usually related to their general condition. An anastomosis of at least 20 mm in diameter was possible as the diameter of the CBC was always larger than 10 mm. Immediate postoperative complications (within one month) were observed in 29 cases (17 p. 100). In 14 cases (8 p. 100), these complications were directly related to the operative procedure and, in 15…

Tips & tricks for transoral approach for submandibular lithiasis (TASL).

INTRODUCTION: Sialolithiases mainly affect the submandibular gland. More often, the lithiasis is large and located at the junction of the middle and the posterior third of the duct, in the hilum region. In such situation, transoral approach is recommended to avoid sialadenectomy because of its lower morbidity. TIPS AND TRICKS: Because of our experience, with over 300 cases operated with this transoral approach, we have decided to describe the tips and tricks that can help the surgeon who operates large stones impacted in the hilum of the submandibulary gland. DISCUSSION: The benefits of thes…

Total Hip Lithiasis: A Rare Sequelae of Spilled Gallstones.

Laparoscopic cholecystectomy is a surgical treatment for acute cholecystitis or symptomatic cholelithiasis. One potential complication, the spillage of gallstones into the peritoneal cavity, can form a nidus for infection and may be associated with hepatic, retroperitoneal, thoracic, and abdominal wall abscesses. We report a case of a patient presenting with a right iliopsoas abscess and an infected right hip prosthesis status postlaparoscopic cholecystectomy. A CT demonstrated that the acetabular shell was overmedialized and perforated through the medial wall. The patient was taken to the ope…

Lithiasis in 1,313 kidney transplants: incidence, diagnosis, and management.

BACKGROUND: Renal transplantation remains the optimal treatment of patients with end-stage renal disease. Urinary lithiasis represents an unusual urologic complication in renal transplantation, with an incidence of <1%. Today, recipients of kidneys from deceased donors are more likely to receive grafts with undiagnosed lithiasis, which does not occur in patients from living donors, owing to screening with computerized tomography. OBJECTIVE: The aim of this study was to evaluate the incidence, diagnosis, and therapeutic management of renal lithiasis in transplanted kidneys at a single institut…

[Bladder lithiasis secondary to intrauterine device migration. Case report].

OBJECTIVE: To report one clinical case of bladder lithiasis secondary to the migration of an intrauterine device, its symptoms, diagnostic tests employed and treatment. METHODS/RESULTS: 30-year-old female patient who presented lower urinary tract irritative symptoms. One year before she underwent insertion of an intrauterine device (copper T) after endometrial curettage for pregnancy interruption. She received treatment for infection cystitis over two years, with various antibiotic cycles. The persistence of symptoms led her to the urology clinics, and a 5 x 3 cm intravesical lithiasis with a…

[Videolaparoscopic treatment of biliary lithiasis. Evolution of our experience].

Videolaparoscopic cholecystectomy is considered the treatment of choice for simple cholelithiasis. Now many surgeons consider the laparscopic procedure usable also in the complicated biliary lithiasis like acute cholecystitis and choledocholithiasis. The authors report their recent experience of the laparoscopic treatment of biliary lithiasis, regarding 221 non-selected patients (69% symptomatic cholelithiasis, 20% chronic cholecystitis, 4.5% acute cholecystitis, 4.5% coledocolithiasis, 2% hydrops). The diagnostic-therapeutic protocol and the results are described and compared with the beginni…

[Bladder lithiasis formed from an IUD. A rare case].

OBJECTIVE: To describe an uncommon case of bladder calculus arising from an intrauterine device (IUD). METHODS/RESULTS: A female patient with an IUD for the past 10 years consulted for frequent episodes of cystitis. Patient evaluation demonstrated a bladder calculus that warranted surgery. Treatment was by cystolithotomy, which disclosed a 6 cm calculus attached to the IUD. CONCLUSION: Bladder calculus arising from an IUD is rare. The case described herein was successfully managed by cystolithotomy.

[Ureteral resection of ureteral inflammatory polyp associated with lithiasis].

We report on a fibroepithelial polyp case with inflammatory characteristics located in the pelvian ureter, and related with an ureteral calculus, which was treated in a conservative way with ureteroscopic resection, after the pincer extraction of the stone. Our team found two fibroepithelial polyp cases in the bibliography reviewed treated with ureteroscopic resection. We consider that the polyp's reactive inflammatory nature is due to the urothelial chronic irritation caused by the calculus.

Urinary infection, renal lithiasis and bladder cancer in Spain.

A case-control study on bladder cancer was carried out in 12 hospitals located in 4 regions of Spain. The study included 497 cases and 530 population controls, matched by sex, age and residence. The present paper reports the results regarding the risk for bladder cancer in relation to history of infections and lithiasis of the urinary tract. Increased risk was found for infections starting 4 years or less before diagnosis (OR = 15.00; 95% CL: 6.07-51.66) but no statistically significant increase in risk was observed for infections starting 5 or more years before (OR = 1.44; 95% CL: 0.86-2.47).…

[Lithiasis in small salivary ducts. Report of 3 cases].

Three cases with sialolithiasis of minor salivary gland are presented. The sialoliths were found on the left side of the upper lip (two patients) with local infection and on the right side of buccal mucosa (one patient) without any symptom. The histologic findings resulted from our own cases and the etiology are discussed extensively and a literature review is made as well.
