Clinical Effects of Posterior Longitudinal Ligament Removal and Wide Anterior Cervical Corpectomy for Spondylosis.

BACKGROUND: Removing the posterior longitudinal ligament in cervical corpectomy is a controversial issue. It is unclear whether the risks are counterbalanced by clinical benefits. Another unexplored topic is whether the width of the corpectomy affects outcome. METHODS: This cross-sectional retrospective study included consecutive patients who underwent cervical corpectomy for spondylosis by 6 different neurosurgeons. We compared 2 groups, where the posterior longitudinal ligament was either removed (N = 15 patients) or preserved (N = 21 patients). The posterior width of the corpectomy was ass

First sacral nerve and anterior longitudinal ligament anatomy: clinical applications during sacrocolpopexy.

BACKGROUND: The recommended location of graft attachment during sacrocolpopexy is at or below the sacral promontory on the anterior surface of the first sacral vertebra. Graft fixation below the sacral promontory may potentially involve the first sacral nerve. OBJECTIVE: The objectives of this study were to examine the anatomy of the right first sacral nerve relative to the midpoint of the sacral promontory and to evaluate the thickness and ultrastructural composition of the anterior longitudinal ligament at the sacral promontory level. STUDY DESIGN: Anatomic relationships were examined in 1

Rapid Progression of Ossification of the Posterior Longitudinal Ligament After Anterior Cervical Discectomy and Fusion.

BACKGROUND: Ossification of the posterior longitudinal ligament (OPLL) has a reported incidence of 1.9%-4.3%. Disease progression is associated with surgery, with most studies focusing on OPLL progression after laminoplasty. The continued range of motion following surgery is believed to place strain on adjacent levels, driving calcification of the ligament. We present a case of marked progression of OPLL at levels adjacent to a previous anterior cervical discectomy and fusion. CASE REPORT: A 59-year-old man initially presented for progressive loss of balance and dexterity and underwent a C4-6

Identification of susceptibility loci for thoracic ossification of the posterior longitudinal ligament by whole-genome sequencing.

Ossification of the posterior longitudinal ligament (OPLL) is a myelopathy commonly observed in the cervical spine. By contrast, thoracic OPLL (T‑OPLL) is rare but more severe. Previous studies have identified several polymorphisms in osteogenic genes that are associated with the occurrence and development of cervical OPLL. However, few genetic studies have evaluated T‑OPLL. The present study aimed to identify the genetic factors for OPLL by performing whole‑genome sequencing (WGS) in 30 unrelated northern Chinese Han patients with T‑OPLL. Using bioinformatics analyses and damaging‑variant pre

The H2 blocker famotidine suppresses progression of ossification of the posterior longitudinal ligament in a mouse model.

BACKGROUND: Ossification of the posterior longitudinal ligament (OPLL) of the spine is a common human myelopathy that leads to spinal cord compression. No disease-modifying drug for OPLL has been identified, whereas surgery and conservative management have been established. OBJECTIVES: To evaluate the therapeutic potential of the H2 blocker famotidine for ectopic ossification in the cervical spine in an OPLL mouse model. METHODS: The H2 blocker famotidine was orally administered to Enpp1 (ttw/ttw) mice, a model of OPLL, at either 4 or 15 weeks of age. Radiological and survival rate analyses

Surgical outcome in patients with cervical ossified posterior longitudinal ligament: A single institutional experience.

OBJECTIVE: Ossification of the posterior longitudinal ligament (OPLL) is a complex multi-factorial disease process having both metabolic and biomechanical factors. The role of surgical intervention as well as the choice of approach weather anterior or posterior is ambiguous. The objective of this study was to assess the surgical out come and post operative functional improvement in patients with cervical OPLL at a tertiary care centre. PATIENTS AND METHODS: This prospective study included 63 patients of cervical OPLL who underwent either anterior and/or posterior surgeries in Department of Ne

Association of NPP1 polymorphism with postoperative progression of ossification of the posterior longitudinal ligament in Chinese patients.

Ossification of the posterior longitudinal ligament (OPLL) is a condition of the spine that can cause paralysis by compressing the spinal cord. The aim of this study was to evaluate the possible role of nucleotide pyrophosphatase phosphodiesterase 1 gene (NPP1) polymorphism in the etiology and pathology of the OPLL in Chinese patients. DNA from patients with OPLL (N = 95) and without OPLL (N = 90) were genotyped for 4 NPP1 single-nucleotide polymorphisms (SNPs): A533C, C973T, IVS15-14T→C, and IVS20-11delT. An association study evaluated the relationship between specific SNP genotypes and susce

Suture-button construct for interosseous ligament reconstruction in longitudinal radioulnar dissociations: a biomechanical study.

PURPOSE: Longitudinal radioulnar dissociation is a triad of injuries consisting of distal radioulnar joint disruption, interosseous ligament complex (IOLC) tear, and radial head fracture. This renders the forearm longitudinally unstable, resulting in proximal migration of the radius and ulnar-sided wrist degeneration. We hypothesized that reconstruction of the central band of the IOLC in cadaver forearms using a Mini-TightRope suture-button construct would restore native forearm stability. METHODS: We implanted 8 fresh-frozen cadaver arms with steel beads into the distal radius and ulna, moun

Diagnosis and surgery of ossification of posterior longitudinal ligament associated with dural ossification in the cervical spine.

Direct removal of the ossified mass via anterior approach carries good decompression to ossification of the posterior longitudinal ligament (OPLL) in the cervical spine. Ossification occasionally involves not only the posterior longitudinal ligament but also the underlying dura mater, which increased the opportunity of the cerebrospinal fluid (CSF) leakage or neurological damage. The surgeon was required to recognize the dural ossification (DO) and need more cautious manipulation. Hida et al. first described the computed tomography (CT) findings that indicated the association with DO, and sugg

Choice of surgical approach for ossification of the posterior longitudinal ligament in combination with cervical disc hernia.

Ossification of the posterior longitudinal ligament (OPLL) is a common spinal disorder that presents with or without cervical myelopathy. Furthermore, there is evidence suggesting that OPLL often coexists with cervical disc hernia (CDH), and that the latter is the more important compression factor. To raise the awareness of CDH in OPLL for spinal surgeons, we performed a retrospective study on 142 patients with radiologically proven OPLL who had received surgery between January 2004 and January 2008 in our hospital. Plain radiograph, three-dimensional computed tomography construction (3D CT),

Ossified posterior longitudinal ligament with an atypical clinical presentation in an Indian male. A case report.

Ossification of the posterior longitudinal ligament is an uncommon disorder mostly seen in the Japanese population and hence termed by some the "Japanese disease". Ossification of the posterior longitudinal ligament is more common in the cervical spine (2). Clinically it is usually asymptomatic, but serious neurological deficits have been seen in some patients (2). The ossified mass composed of lamellar bone and focal calcified cartilage expands in volume causing spinal canal stenosis and cord compression. Morphologically, four forms of ossification of the posterior longitudinal ligament have

Symptomatic ossification of the anterior longitudinal ligament with stenosis of the cervical spine: a report of seven cases.

Seven men with a mean age of 63.9 years (59 to 67) developed dysphagia because of oesophageal compression with ossification of the anterior longitudinal ligament (OALL) and radiculomyelopathy due to associated stenosis of the cervical spine. The diagnosis of OALL was made by plain lateral radiography and classified into three types; segmental, continuous and mixed. Five patients had associated OALL in the thoracic and lumbar spine without ossification of the ligamentum flavum. All underwent removal of the OALL and six had simultaneous decompression by removal of ossification of the posterior l

Osteoplastic anterolateral vertebrotomy without fusion for multilevel cervical ossification of the posterior longitudinal ligament.

OBJECTIVE: To introduce a new method for osteoplastic anterolateral vertebrotomy without fusion to resect multilevel cervical ossification of the posterior longitudinal ligament. METHODS: A multilevel vertebral column graft containing portions of the intervertebral discs is cut with a microsurgical saw from the anterolateral part of the vertebra via a thin lateral gutter placed in advance. A sufficiently wide oblique operative field is provided for resection of the ossified ligament with this method, and only a narrow bony defect remains after simple replacement of the vertebral graft. Cervical laminoplasty to enlarge the spinal canal in multilevel ossification of the posterior longitudinal ligament with myelopathy.

We studied 23 patients with severe myeloradiculopathy involving multiple (more than three) levels of ossification of the posterior longitudinal ligament (OPLL) of the cervical spine, who were treated with laminoplasty to enlarge the spinal canal. The resected spinous processes were used as bone grafts to support the opened laminae. These patients were analyzed pre- and postoperatively with a neurological evaluation according to the Japanese Orthopedic Association (JOA) score system for cervical myelopathy. Follow-up was from 2.0 to 5.3 years with an average of 31.5 months. The results were com

The correlation between loss of cervical lordosis and the severity of ligament ossification in patients with cervical ossification of posterior longitudinal ligament: a cross-sectional observational study.

BACKGROUND: Both genetic and environmental (e.g., mechanical stress) factors have been considered multifactorial components contributing to the development and progression of ossification of posterior longitudinal ligament (OPLL). Importantly, loss of cervical lordosis may change the mechanical stress on posterior longitudinal ligament. However, few studies have discussed the relationship between the cervical sagittal alignment and OPLL. The aim of this study is to quantify the cervical sagittal alignment and severity of ligament ossification in patients with cervical OPLL and to analyze their
