Multi-level digital-twin models of pulmonary mechanics: correlation analysis of 3D CT lung volume and 2D Chest motion.

Creating multi-level digital-twin models for mechanical ventilation requires a detailed estimation of regional lung volume. An accurate generic map between 2D chest surface motion and 3D regional lung volume could provide improved regionalisation and clinically acceptable estimates localising lung damage. This work investigates the relationship between CT lung volumes and the forced vital capacity (FVC) a surrogate of tidal volume proven linked to 2D chest motion. In particular, a convolutional neural network (CNN) with U-Net architecture is employed to build a lung segmentation model using a

The Effect of Tidal Volumes of Mechanically Ventilated Patients on Lung Sliding Amplitude in Point-Of-Care Lung Ultrasound.

OBJECTIVES: Several factors influencing lung sliding amplitude have been identified. However, the effect of tidal volume changes on lung sliding amplitude in the same patient remains unclear. In this study, we investigated the relationship between lung sliding amplitude, tidal volume, lung compliance, and mortality. METHODS: This was a single-center, observational, and prospective study. Lung sliding amplitudes were measured at four points using ultrasound in patients connected to mechanical ventilators in the emergency department of a tertiary hospital. In patients in whom tidal volume adjus

Effect of hyperinflation on the diaphragm.

Acute hyperinflation causes the inspiratory muscles to operate at shorter than normal lengths. The ability of these muscles, in particular the diaphragm, to lower intrathoracic pressure is therefore reduced. Skeletal muscles, however, adapt to chronic shortening, and animals models of emphysema have shown that with chronic hyperinflation, the diaphragmatic muscle fibres lose sacromeres. As a result, the force-generating ability of these fibres is relatively preserved. In patients with hyperinflation due to chronic obstructive pulmonary disease, the ability of the diaphragm to generate pressure

Lung function abnormalities in patients with primary biliary cirrhosis.

Twenty-five patients with primary biliary cirrhosis (PBC) in different stages were investigated with respect to pulmonary function abnormalities. The results were compared with a reference sample of 17 sex- and age-matched healthy subjects. A high prevalence of lung function impairment was found in the PBC patients (14/25 [56%]). Bronchial asthma was present in three patients, and severe lung emphysema in one. These four patients had an abnormal lung function, mainly of obstructive type. There was a statistically significant difference between the remaining 21 PBC patients without chronic obst

Infants with cystic fibrosis: pulmonary function at diagnosis.

Meconium ileus (MEC), failure to thrive (FTT), and a combination of FTT and pulmonary symptoms (COMB) are the most frequent symptoms of cystic fibrosis (CF) at the time of diagnosis. The purpose of this study was to compare to normal controls (NC) the pulmonary function of CF infants at the time of diagnosis, when grouped by these symptoms. The measurements of pulmonary function included oxygen saturation (SaO2), functional residual capacity (FRC), mixing index (MI), total respiratory system compliance (Crs), and maximal flow at FRC (VmaxFRC). Compared to NC (n = 33), the MEC group (n = 5) had

Physiological basis and interpretation of indices of pulmonary mechanics.

Tests of pulmonary mechanical function provide information about the state of the lungs, both airways and parenchyma. This information can be extracted from measurements made in experimental animals, especially the combined determination of pulmonary resistance and dynamic compliance. This report discusses the rationale upon which effects of an intervention on the lung periphery can be distinguished from those on more central airways. Further, practical considerations involved in making these measurements are discussed.

Late pulmonary function in survivors of adult respiratory distress syndrome.

Early recognition and improvements in treatment have brought an appreciable decline in mortality from the adult respiratory distress syndrome. Physiologic changes during the acute illness include restriction of lung volumes and a defect in gas transfer. Until recently, pulmonary function of long-term survivors has not been discussed. This report concerns the pulmonary function of two patients studied 19 and 22 months, respectively, after surviving an episode. No residual changes in lung volume or compliance were found. A mild decrease in diffusing capacity was noted, and alveolar-arterial oxyg

Pulmonary function in advanced pulmonary hypertension.

Pulmonary mechanical function and gas exchange were studied in 33 patients with advanced pulmonary vascular disease, resulting from primary pulmonary hypertension in 18 cases and from Eisenmenger physiology in 15 cases. Evidence of airway obstruction was found in most patients. In addition, mean total lung capacity (TLC) was only 81.5% of predicted and 27% of our subjects had values of TLC less than one standard deviation below the mean predicted value. The mean value for transfer factor (TLCO) was 71.8% of predicted and appreciable arterial hypoxaemia was present, which was disproportionate t

Lung and thorax development during adolescence: relationship with pubertal status.

The aim of the present study was to define reference values for lung volumes and the lung transfer factor for carbon monoxide (TL,CO) for an adolescent population using thoracic volume index (TVI) and an index of pubertal stage in order to account for the variation in growth pattern between adolescents. TVI, pubertal stage by Tanner scale (PST), time since menarche, functional residual capacity measured using the helium-dilution technique, vital capacity, total lung capacity and TL,CO measured using a steady-state method were determined in 51 males (aged 13-20 yrs; PST T3-T5) and 52 females (a

Respiratory mechanics and control of the ventilatory cycle in the garter snake.

We measured respiratory airflow and intrapulmonary pressure in spontaneously breathing garter snakes (Thamnophis sirtalis) before and after bilateral vagotomy. Each breathing cycle consisted of expiration, inspiration, and a breath-holding period with the glottis closed, as previously reported. Respiratory system compliance was very high compared with that of mammals (0.042 ml/cm H2O per g), but was considerably reduced when the animal was coiled. Resistance to airflow averaged 6.35 cm H2O per ml/sec. Lung volume during the breath-hold was usually above the relaxed volume of the system. Analys

Alpha1-antitrypsin deficiency: genetic, clinical and functional correlations in a three generation family.

alpha1-antitrypsin (alpha1-AT) deficiency is a hereditary condition transmitted as an autosomal codominant trait. Fully deficient homozygotes develop chronic obstructive pulmonary disease at an early age. It remains controversial whether heterozygotes develop some degree of airway disease at an early age which results in severe obstructive disease later in life. We studied 12 alpha1-AT-deficient members of a family spanning three generations by extensive pulmonary function tests, clinical history and genetic determinations. 3 of the subjects were homozygotes and showed important clinical and l

Computational simulations of respiratory-laryngeal interactions and their effects on lung volume termination during phonation: Considerations for hyperfunctional voice disorders.

Glottal resistance plays an important role in airflow conservation, especially in the context of high vocal demands. However, it remains unclear if laryngeal strategies most effective in controlling airflow during phonation are consistent with clinical manifestations of vocal hyperfunction. This study used a previously validated three-dimensional computational model of the vocal folds coupled with a respiratory model to investigate which laryngeal strategies were the best predictors of lung volume termination (LVT) and how these strategies' effects were modulated by respiratory parameters. In cystic fibrosis, lung clearance index is sensitive to detecting abnormalities appearing at exercise in children with normal spirometry.

Symptom-limited incremental cardiopulmonary exercise test was performed in school-age children with clinically stable cystic fibrosis (CF), all with normal spirometry. Physiological parameters were compared between patients with normal lung clearance index (LCI; n=6) and patients with elevated LCI (n=6). Dyspnoea ratings during exercise were similar in both groups. Although no patient had significant dynamic hyperinflation, end-expiratory lung volumes were higher throughout exercise in patients with elevated LCI. In addition, alveolar-arterial oxygen gradient at peak exercise was higher, and S
