Changes in occlusal force after intraoral one-stage curved osteotomy of the prominent mandibular angle.

BACKGROUND: Curved osteotomy of a prominent mandibular angle is widely used in narrowing the width of the lower face, but its influence on occlusal force has not been evaluated. METHODS: The maximum occlusal forces of five different teeth areas in 20 consecution patients with an average age of 26.5 years were studied. The examinations were performed both before the operation and 6 months afterward. RESULTS: The values of maximum occlusal force had reached the preoperative levels or slightly above for the incisor teeth, bilateral premolar teeth, and bilateral molar teeth 6 months after operat

[Obstructive sleep-apnoea syndrome: good results with maxillo-mandibular osteotomy after failure of conservative therapy].

A 36-year-old woman and a 49-year-old man with symptoms of an obstructive sleep-apnoea syndrome benefited insufficiently from the therapy of choice, i.e. treatment with continuous positive airway pressure. Minor surgical procedures to improve the upper airways did not have the desired effect. Subsequently, maxillo-mandibular osteotomy was performed. Thereafter, both the daytime somnolence and the snoring during sleep disappeared. Indications for osteotomy of the maxilla and mandible are: failure of conservative therapy or other forms of treatment with a lower morbidity, severe mandibular hypop…

Evaluation of soft tissue profile following intraoral ramus osteotomy in Chinese adults with mandibular prognathism.

Twenty-five Chinese adults with mandibular prognathism were treated with either the intraoral vertical subcondylar osteotomy or the bilateral sagittal split ramus osteotomy. The patients were kept in maxillomandibular fixation for 6 to 8 weeks while osteosynthesis was achieved with the use of intraosseous wiring. Serial lateral cephalograms were taken presurgery and between 12 and 26 months postsurgery, and specific soft and hard tissue points were digitized on a computer. The mean mandibular setback postsurgically was 8.4 +/- 3.2 mm, with a 5.2-degree reduction in point A-nasion-point B angle

Autonomous surgical planning of mandibular angle reduction based on anatomical landmarks and osteotomy plane detection.

In this paper, a method of automatic surgical planning for mandibular osteotomy based on automatic identification of mandibular landmark points was proposed with facial skeletal contour surgery. A mandibular osteotomy plane optimization model was established, allowing key anatomical measurement parameters to be obtained. The analysis of the preoperative CT images of 100 patients with mandibular osteotomy showed that the average RMS error of the automatic identification algorithm was 1.87 ± 0.33 mm. To verify the effectiveness of the mandibular angle osteotomy surgical planning system on real c…

Evaluation, prevention, and treatment of inferior alveolar nerve injury in bilateral sagittal split mandibular osteotomy.

BACKGROUND: Bilateral sagittal split osteotomy (BSSO) is a widely adopted surgical procedure for correcting mandibular deformities, yet neurosensory disturbance (NSD) of the inferior alveolar nerve (IAN) remains a significant postoperative complication. This complication adversely impacts patients' quality of life due to persistent sensory abnormalities in the lower lip and chin region. MAIN BODY: This narrative review summarizes anatomical risks and prevention/management strategies. Cone-beam CT (CBCT) may clarify canal anatomy and support risk stratification. Nerve-sparing osteotomy modific

Impact of the lateral skeletal stability following bilateral sagittal split ramus osteotomy for mandibular asymmetry.

This study evaluated the stability of bilateral sagittal split ramus osteotomy (BSSRO) associated with positional plagiocephaly and temporal and masseter muscles using posteroanterior cephalogram analysis and three-dimensional computed tomography (3D-CT). This retrospective cohort study included 31 patients who underwent BSSRO for mandibular asymmetry. The cranial vault asymmetry index (CVAI) and the cephalic index were used as indicators of positional plagiocephaly. The distance from the vertical reference line to the menton (Me) was measured on posteroanterior cephalograms immediately and 1

Biomechanical Analyses of Porous Designs of 3D-Printed Titanium Implant for Mandibular Segmental Osteotomy Defects.

Clinically, a reconstruction plate can be used for the facial repair of patients with mandibular segmental defects, but it cannot restore their chewing function. The main purpose of this research is to design a new three-dimensionally (3D) printed porous titanium mandibular implant with both facial restoration and oral chewing function reconstruction. Its biomechanical properties were examined using both finite element analysis (FEA) and in vitro experiments. Cone beam computed tomography images of the mandible of a patient with oral cancer were selected as a reference to create 3D computation

The three-dimensional evaluation of positional change in mandibular condyle after intraoral vertical ramus osteotomy.

OBJECTIVES: The purpose of this study was to compare the changes of the condylar axis, the anteroposterior condylar position relative to the glenoid fossa, after intraoral vertical ramus osteotomy (IVRO). STUDY DESIGN: 21 patients diagnosed as skeletal class III malocclusion underwent IVRO and were followed according to the authors' postoperative management regimen. The three-dimensional positions of the condyles were evaluated by cone-beam computerized tomography (CBCT) at pre-op, post-op, and at follow-up. CBCT images were referenced to assess the condylar axis change and the anteroposterio

Mandibular Access Osteotomy for Tumor Ablation: Could a More Tissue-Preserving Technique Affect Healing Outcome?

PURPOSE: Paramedial mandibulotomy facilitates access for the resection of tumors in the oral and oropharyngeal space; however, severe complications related to surgical techniques and radiotherapy have been reported for this procedure. This study evaluated whether preservation of the periosteum during a mandibulotomy would decrease postoperative complications owing to the increased healing capacity provided by preserving more tissue. MATERIALS AND METHODS: Patients who underwent mandibulotomy for surgical tumor ablation from 2007 through 2012 were included in a retrospective controlled cohort

Two-stage mandibular setback employing modified Dingman's mandibular body osteotomy with step for large antero-posterior dentofacial deformity correction: a report of two cases.

Mandibular prognathism is a well-known skeletal discrepancy with varying prevalence rates demographically. Patients with severe prognathism may experience difficulties speaking and chewing food as well as problems with psychosocial adjustment, indicating the need for surgical correction. In cases of single-stage larger mandibular setbacks, complications such as reduction in the pharyngeal airway and instability can be encountered. Therefore, two-stage orthognathic surgery could be recommended. Two patients (17-year-old female and 16-year-old female) reported with large mandibular antero-poster
